多模态监测在听神经瘤手术面神经保护中的应用效果
Application effect of multimodal monitoring in facial nerve protection in acoustic neuroma surgery
目的 探讨多模态监测在听神经瘤(Acoustic neuroma,AN)手术面神经保护中的应用效果。 方法 选取2021年5月—2024年5月赣州市人民医院收治的90例行手术治疗的AN患者,依据是否采用多模态监测将其分成观察组(行多模态监测)和对照组(未行多模态监测),每组45例。比较2组一般资料、手术相关指标、面神经解剖保留和功能情况、术后不良事件发生情况。 结果 2组年龄、性别、肿瘤位置、病程、肿瘤大小、ASA分级情况比较,差异无统计学意义(P>0.05);观察组手术时间短于对照组(P<0.05);2组术中出血量、肿瘤切除情况比较,差异无统计学意义(P>0.05);2组面神经解剖保留率比较,差异无统计学意义(P>0.05);观察组术后2周、术后3个月面神经功能保留率均高于对照组(P<0.05);观察组术后不良事件总发生率低于对照组(P<0.05)。 结论 多模态监测辅助AN手术可在一定程度上缩短手术时间,保护患者面神经功能,同时减少术后不良事件发生。
Objective To explore the application effect of multimodal monitoring in facial nerve protection in acoustic neuroma (AN) surgery. Methods A total of 90 patients with AN treated by operation in Ganzhou People's Hospital from May 2021 to May 2024 were divided into observation group (with multimodal monitoring) and control group (without multimodal monitoring) according to whether multimodal monitoring was adopted or not, with 45 cases in each group. The general data, surgical related indexes, anatomical preservation and function of the facial nerve and postoperative adverse events were compared between the two groups. Results There was no statistically significant difference in age, sex, tumor location, course of disease, tumor size and ASA grade between the two groups (P>0.05). The operation time in the observation group was shorter than that in the control group (P<0.05). There was no statistically significant difference in intraoperative blood loss and tumor resection between the two groups (P>0.05). There was no statistically significant difference in the anatomical retention rate of the facial nerve between the two groups (P>0.05). The retention rate of the facial nerve function in the observation group was higher than that in the control group 2 weeks and 3 months after operation (P<0.05). The total incidence of postoperative adverse events in the observation group was lower than that in the control group (P<0.05). Conclusion Multimodal monitoring assisted AN operation can shorten the operation time, protect the facial nerve function of patients and reduce the occurrence of postoperative adverse events to some extent.
