摘要

目的 分析S1PR2、ApoH、Let-7d在主动脉夹层中的表达及临床意义。方法 选择2021年1月~2023年9月在南阳市中心医院利用胸腹主动脉CTA确诊主动脉夹层患者60例作为研究组,另选取同时期50例健康体检者作为对照组,以数字减影血管造影检查结果为金标准,将研究组患者按照病情严重程度分为轻度组(n=21)、中度组(n=19)、重度组(n=20)。比较2组人群S1PR2、ApoH、Let-7d表达、不同程度下各组人群S1PR2、ApoH、Let-7d表达、各种检测方式与三者联合检测检测对比、ROC曲线分析S1PR2、ApoH、Let-7d及三者联合诊断价值、S1PR2、ApoH、Let-7d水平与主动脉夹层疾病中的相关性。结果 与对照组相比,研究组人群S1PR2、Let-7d表达较低、ApoH表达较高(P<0.05);重度组人群S1PR2、Let-7d表达均低于轻度、中度人群,重度组人群ApoH表达高于轻度、中度人群;本次研究中使用S1PR2阳性为5例、ApoH阳性为6例、Let-7d阳性为4例均低于联合检测阳性例数;三者联合AUC值(0.743)均高于S1PR2(0.552)、ApoH(0.343)、Let-7d(0.493);且三项联合灵敏度、特异度、准确度较高(P<0.05);以S1PR2、ApoH、Let-7d与主动脉夹层疾病中的相关性分析,S1PR2与主动脉夹层疾病的相关性呈现出负相关(r=-5.137,P=0.027)、ApoH与主动脉夹层疾病的相关性呈现出正相关(r=4.211,P=0.044)、Let-7d与主动脉夹层疾病的相关性呈现出负相关(r=-6.291,P=0.016)。结论 S1PR2、ApoH、Let-7d在主动脉夹层疾病中的表达水平存在一定的相关性,将这三种指标结合起来,能够指导主动脉夹层疾病治疗及预后评估判断提供一定的理论基础。

Abstract

Objective To analyze the expression and clinical significance of S1PR2, ApoH and Let-7d in aortic dissection. Methods A total of 60 patients with thoracic and abdominal aortic dissection diagnosed by CTA in Nanyang Central Hospital from January 2021 to September 2023 were selected as the study group, and another 50 healthy subjects during the same period were selected as the control group. The results of digital subtraction angiography (DSA) were used as the gold standard. The patients in the study group were divided into mild group (n=21), moderate group (n=19) and severe group (n=20) according to the severity of the disease. The expressions of S1PR2, ApoH and Let-7d in the two groups were compared, the expressions of S1PR2, ApoH and Let-7d in different levels of the groups, the detection comparison between various detection methods and the combined detection of the three groups, and the ROC curve analysis of S1PR2, ApoH, Let-7d and the combined diagnostic value of the three groups, S1PR2, ApoH and Le Association of t-7d levels with aortic dissection disease. Results Compared with the control group, the expression of S1PR2 and Let-7d in the study group was lower and the expression of ApoH was higher (P<0.05). The expression of S1PR2 and Let-7d in severe group was lower than that in mild and moderate groups, and the expression of ApoH in severe group was higher than that in mild and moderate groups. In this study, 5 cases were positive for S1PR2, 6 cases were positive for ApoH and 4 cases were positive for Let-7d, all of which were lower than the number of positive cases for combined testing. The combined AUC value of the three components (0.743) was higher than that of S1PR2 (0.552), ApoH (0.343) and Let-7d (0.493). The combined sensitivity, specificity and accuracy were higher (P<0.05). Based on the correlation analysis of S1PR2, ApoH, Let-7d and aortic dissection disease, the correlation between S1PR2 and aortic dissection disease showed a negative correlation (r=-5.137, P=0.027), and the correlation between ApoH and aortic dissection disease showed a positive correlation (r=4.211, P=0.044) and Let-7d were negatively correlated with aortic dissection disease (r=-6.291, P=0.016). Conclusion There is a certain correlation between the expression levels of S1PR2, ApoH and Let-7d in aortic dissection diseases. The combination of these three indicators can provide a theoretical basis for guiding the treatment and prognosis evaluation of aortic dissection diseases.
