摘要

目的 探讨血清淀粉样蛋白A(SAA)、胰岛素样生长因子-1(IGF-1)、金属蛋白酶抑制剂-1(TIMP-1)与非小细胞肺癌病理特征及预后的相关性。方法 选取2021年3月~2023年7月就诊于商丘市立医院的84例非小细胞肺癌患者为研究组,根据不同预后分为预后不良及预后良好患者,另选取本院同期收治的80例良性肺部病变患者作为对照组。对比2组入院时血清SAA、IGF-1、TIMP-1水平;对比非小细胞肺癌患者不同病理学参数及不同预后患者入院时血清各指标水平;ROC分析非小细胞肺癌患者入院时血清各指标水平联合检测对预后不良的预测价值。结果 研究组血清SAA、IGF-1、TIMP-1水平显著高于对照组(P<0.05);不同病理学参数非小细胞肺癌患者血清SAA、IGF-1、TIMP-1水平比较:Ⅰ~Ⅱ期低于Ⅲ~Ⅳ期、无淋巴结转移低于有淋巴结转移(P<0.05);预后不良患者血清SAA、IGF-1、TIMP-1水平显著高于预后良好患者(P<0.05);入院时联合检测血清SAA、IGF-1、TIMP-1水平对非小细胞肺癌预后不良的预测价值较高。结论 非小细胞肺癌患者中血清SAA、IGF-1、TIMP-1的表达与患者病理特征及预后相关,检测其表达对判断患者病情程度及评估预后具有重要价值。

Abstract

Objective To investigate the correlation between serum amyloid A (SAA), insulin-like growth factor-1 (IGF-1), and tissue inhibitor of metalloproteinase-1 (TIMP-1) and the pathological characteristics and prognosis of non-small cell lung cancer. Methods Eighty-four cases of non-small cell lung cancer patients who attended Shangqiu Municipal Hospital from March 2021 to July 2023 were selected as the study group, which were categorized into patients with poor and good prognosis according to different prognoses, and 80 cases of benign lung lesions admitted to the same hospital during the same period were selected as the control group. The levels of serum SAA, IGF-1 and TIMP-1 at the time of admission were compared between the two groups; the levels of serum indicators at the time of admission were compared between non-small cell lung cancer patients with different pathologic parameters and patients with different prognoses. ROC was used to analyze the predictive value of poor prognosis of non-small cell lung cancer patients by the combined detection of serum levels of various indexes at the time of admission. Results The levels of SAA, IGF-1 and TIMP-1 of the study group were significantly higher than those in the control group (P< 0.05). The levels of SAA, IGF-1 and TIMP-1 in patients with non-small cell lung cancer with different pathological parameters were lower in stages Ⅰ and Ⅱ than in stages Ⅲ and Ⅳ, and no lymph node metastasis was lower than having lymph node metastasis (P<0.05). Serum levels of SAA, IGF-1 and TIMP-1 in patients with poor prognosis were significantly higher than those in patients with good prognosis (P<0.05). The combined detection of serum SAA, IGF-1 and TIMP-1 levels on admission had high predictive value for poor prognosis of NSCLC. Conclusion The expression of serum SAA, IGF-1, and TIMP-1 in patients with non-small cell lung cancer is associated with their pathological characteristics and prognosis, and the detection of its expression is of great value in determining the degree of the patient's condition and evaluating the prognosis.
