摘要

目的 分析基质金属蛋白酶组织抑制因子3(TIMP-3)、DNA甲基转移酶1(DNMT1)与乳腺癌患者病情进展及预后情况的关联。方法 回顾性分析信阳市中心医院2022年3月—2024年3月收治的102例乳腺癌患者临床资料,分析TIMP-3、DNMT1与乳腺癌患者肿瘤标志物的相关性,及对乳腺癌不良预后的预测效能。结果 晚期组的TIMP-3表达评分低于中期组、早期组,DNMT1表达评分高于中期组、早期组(P<0.05)。晚期组的癌胚抗原(CEA)、糖类抗原15-3(CA153)、糖类抗原125(CA125)、糖类抗原19-9(CA199)、组织多肽抗原(TPA)均高于中期组、早期组(P<0.05)。TIMP-3与CEA、CA153、CA125、CA199、TPA负相关(P<0.05)。DNMT1与CEA、CA153、CA125、CA199、TPA正相关(P<0.05)。预后不良组的TIMP-3表达评分低于预后良好组,DNMT1表达评分高于预后良好组(P<0.05)。TIMP-3、DNMT1联合检测对乳腺癌不良预后的预测AUC值高于TIMP-3、DNMT1单独检测。结论 联合检测TIMP-3、DNMT1可作为识别患者不良预后的重要辅助手段。

Abstract

Objective To analyze the association between matrix metalloproteinase tissue inhibitor 3 (TIMP-3), DNA methyltransferase 1 (DNMT1) and the disease progression and prognosis of breast cancer patients. Methods A retrospective analysis was conducted on the clinical data of 102 breast cancer patients admitted to Xinyang Central Hospital from March 2022 to March 2024. The correlations between TIMP-3, DNMT1 and tumor markers in breast cancer patients were analyzed, as well as their predictive efficacy for adverse prognosis of breast cancer. Results The expression score of TIMP-3 in the late stage group was lower than that in the middle stage group, and the expression score of DNMT1 in the early stage group was higher than that in the middle stage group and the early stage group (P<0.05). The levels of carcinoembryonic antigen (CEA), carbohydrate antigen 15-3 (CA153), carbohydrate antigen 125 (CA125), carbohydrate antigen 19-9 (CA199) and tissue polypeptide antigen (TPA) in the late group were higher than those in the middle group and early group (P<0.05). TIMP-3 was negatively correlated with CEA, CA153, CA125, CA199 and TPA (P<0.05). DNMT1 was positively correlated with CEA, CA153, CA125, CA199 and TPA (P<0.05). The poor prognosis group had a significantly lower expression score of TIMP-3 and a significantly higher expression score of DNMT1 than the good prognosis group (P<0.05). The AUC value of TIMP-3 and DNMT1 combined detection in predicting poor prognosis of breast cancer was higher than that of TIMP-3 or DNMT1 alone. Conclusion The combined detection of TIMP-3 and DNMT1 can serve as an important auxiliary method for identifying patients with poor prognosis.
