Analysis of independent risk factors for poor prognosis after transnasal-intestinal obstruction catheterization under endoscopic ultrasound and construction and verification of nomogram

摘要

目的 探讨影响超声内镜下经鼻-肠梗阻导管置管术后预后不良的独立危险因素,构建预测术后预后不良的列线图,并对列线图进行外部验证。 方法 收集2019年2月-2022年2月于该院行超声内镜下经鼻-肠梗阻导管置管术的肠梗阻患者451例,根据患者的临床资料建立列线图模型,收集条件相同的2022年2月-2024年2月的194例患者作为外部验证组,对模型进行外部验证。观察术后30 d的恢复情况,将患者分为预后良好组和预后不良组,采用多因素Logistic回归模型,分析影响超声内镜下经鼻-肠梗阻导管置管术后预后不良的独立危险因素。采用R 3.6.3软件和RMS程序包,构建预测超声内镜下经鼻-肠梗阻导管置管术后预后不良风险的列线图模型。采用受试者操作特征曲线(ROC curve)和校准曲线评估模型的区分度和一致性。 结果 预后不良组年龄大于预后良好组,C反应蛋白(CRP)、降钙素原(PCT)和中性粒细胞与淋巴细胞比值(NLR)水平高于预后良好组,住院时间长于预后良好组,糖尿病、腹痛进行性加重和应用激素比例高于预后良好组,体重指数(BMI)、术前白蛋白水平和术前营养支持比例低于预后良好组,差异均有统计学意义(P < 0.05)。多因素Logistic回归模型(引入水准为0.05,排除水准为0.107)结果显示:年龄 ≥ 68岁(OR^ = 2.631,95%CI:1.927~3.593)、BMI < 22.31 kg/m2OR^ = 2.142,95%CI:1.436~3.195)、术前白蛋白 < 32.47 g/L(OR^ = 1.962,95%CI:1.506~2.556)、无术前营养支持(OR^ = 2.814,95%CI:1.401~5.654)是影响超声内镜下经鼻-肠梗阻导管置管术后预后不良的独立危险因素(P < 0.05)。列线图模型显示:年龄、BMI、术前白蛋白和无术前营养支持会增加相应权重,内部和外部验证结果提示模型一致性和区分度均较好。 结论 年龄 ≥ 68岁、BMI < 22.31 kg/m2、术前白蛋白 < 32.47 g/L、无术前营养支持是超声内镜下经鼻-肠梗阻导管置管术后预后不良的影响因素,以此4项因素为基准建立的列线图模型,可靠且实用。

Abstract

Objective To explore the independent risk factors influencing the poor prognosis after transnasal-intestinal obstruction catheterization under endoscopic ultrasound, construct a nomogram for predicting poor postoperative prognosis, and conduct external validation of the nomogram. Methods Clinical data of 451 patients with intestinal obstruction who underwent endoscopic ultrasound transnasal-intestinal obstruction catheterization from February 2019 to February 2022 were collected to establish a nomogram. Then, 194 sets of data with the same conditions from February 2022 to February 2024 were collected as the external validation group to validate the model externally. The recovery at 30 d after operation was observed and divided into good prognosis group and poor prognosis group. Multivariate Logistic regression model was used to analyze the independent risk factors influencing the poor prognosis after transnasal-intestinal obstruction catheterization under endoscopic ultrasound. Using R 3.6.3 software and the RMS package, a nomogram model for predicting the risk of poor prognosis after intestinal obstruction catheterization under endoscopic ultrasound was constructed.. The discrimination and consistency of the model were evaluated using receiver operator characteristic curve (ROC curve) and calibration curve. Results The patients in the poor prognosis group were older than those in the good prognosis group, the levels of C-reactive protein (CRP), procalcitonin (PCT) and neutrophil to lymphocyte ratio (NLR) were higher than those in the good prognosis group, the length of hospital stay was longer than that in the good prognosis group, and the proportion of diabetes, abdominal pain and hormone using were higher than those in the good prognosis group, body mass index (BMI), preoperative albumin level and preoperative nutritional support ratio were lower than those of the good prognosis group, with statistical significance (P < 0.05). Multivariate Logistic regression analysis (introduction level was 0.05, exclusion level was 0.107) showed that: age ≥ 68 years (OR^ = 2.631, 95%CI: 1.927 ~ 3.593), BMI < 22.31 kg/m2 (OR^ = 2.142, 95%CI: 1.436 ~ 3.195), preoperative albumin < 32.47g/L (OR^ = 1.962, 95%CI: 1.506 ~ 2.556) and preoperative nutritional non-support (OR^ = 2.814, 95%CI: 1.401 ~ 5.654) were independent risk factors affecting the poor prognosis after endoscopic transnasal-intestinal obstruction catheterization (P < 0.05). The column nomogram showed that old age, low BMI, low preoperative albumin, and no preoperative nutritional support all increased their corresponding weights. Internal and external validation results indicated good consistency and discrimination of the model. Conclusion age ≥ 68 years, BMI < 22.31 kg/m2, preoperative albumin < 32.47 g/L, and no preoperative nutritional support are all independent risk factors affecting the ineffective of intestinal obstruction catheterization under endoscopic ultrasound. The nomogram model established in this study based on these four factors has high reliability and practicality.

肠梗阻是临床中最常见的疾病之一。患者肠道发生阻塞或麻痹,阻止了肠内容物向前通过,导致其在阻塞附近发生机械性或功能性积聚[1]。肠梗阻的常见表现包括:恶心呕吐、痉挛腹胀和大便潴留,若不及时治疗,可能引起各种并发症[2-3]。手术治疗可彻底地解决梗阻,但创伤大[4]。因此,临床首先考虑保守治疗,经鼻-肠梗阻导管置管术可直接到达空肠,快速减压引流,针对梗阻部位进行蓖麻油注入,可迅速疏通梗阻,达到治疗目的[5]。置管方法包括:透视、内镜或两者结合,透视可迅速找到导管所处位置,但具有放射损伤,而内镜无此缺点,且内镜可在直视下操作,便捷且直观,但内镜无法精确地显示导管[6-7]。因此,超声内镜下经鼻-肠梗阻导管置管术开始应用于肠梗阻的治疗中[8]。临床关于超声内镜下经鼻-肠梗阻导管置管术疗效和影响因素的研究较少,而分析预后不良的风险因素是,提前制定相关预防策略和提高疗效的重要步骤。本研究纳入接受超声内镜下经鼻-肠梗阻导管置管术的肠梗阻患者451例,旨在分析不同疗效患者临床资料的差异,并探讨导致差异的原因,建立列线图模型,以期为肠梗阻术后疗效的改善,提供依据。

1 资料与方法

1.1 一般资料

收集2019年2月-2022年2月于本院行超声内镜下经鼻-肠梗阻导管置管术的451例肠梗阻患者的临床资料。以451例符合要求的患者资料为基础,建立列线图模型后,再收集条件相同的2022年2月-2024年2月本院收治的194例患者的临床资料作为外部验证组,并对模型进行外部验证。

纳入标准:年龄≥18岁;确诊为肠梗阻[9];行超声内镜下经鼻-肠梗阻导管置管术者;存在腹痛和呕吐等症状;临床资料完整。排除标准:合并肿瘤;有心、肝和肾等器官功能严重障碍;存在感染者;腹部有外伤;有促胃动力药使用史。本研究经医院伦理委员会批准,伦理批件号:2023-R333。

1.2 手术方法

采用超声内镜下经鼻-肠梗阻导管置管术治疗。在肠梗阻导管头端固定细线,留出2 cm线头,再将肠梗阻导管插入鼻腔,用胃镜的活检钳固定细线,并借助细线引导肠梗阻导管至十二指肠降部,随后,将导管的导丝置入导管,并在超声内镜辅助下,逐步送入导丝,直至空肠后方,确认导管位置后,去除导丝,导管前部用气囊充气,并接入负压吸引。置管过程中,动作尽量轻柔,避免造成肠穿孔。

1.3 观察指标

检索医院病历系统,观察患者年龄、性别、体重指数(body mass index,BMI)、既往史(高血压、糖尿病和腹腔手术史)、体征(贫血、休克、便血、肠鸣音消失和腹痛进行性加重)、肠梗阻类型、术前营养支持、应用抗生素、应用激素、住院时间和实验室指标[术前白蛋白、C反应蛋白(C-reactive protein,CRP)、降钙素原(procalcitonin,PCT)和中性粒细胞与淋巴细胞比值(neutrophil to lymphocyte ratio,NLR)]。

1.4 疗效判断

记录患者术后30 d的恢复情况,包括:消化道症状(腹胀腹痛、呕吐压痛和肠鸣音消失)、CT检查结果。若患者消化道症状消失,影像学提示肠管无扩张、积气或积液平面减轻,可将患者纳入预后良好组(n = 387),剩余患者纳入预后不良组(n = 64),术后发现感染和休克等并发症的,也纳入预后不良组。

1.5 统计学方法

采用SPSS 25.0和R 3.6.3统计学软件分析数据。计量资料以均数±标准差(x¯±s)表示,比较采用独立样本t检验;计数资料以例(%)表示,比较采用χ2检验。P < 0.05为差异有统计学意义。采用多因素Logistic回归模型,分析影响超声内镜下经鼻-肠梗阻导管置管术后预后不良的独立危险因素,并构建列线图模型;绘制受试者操作特征曲线(receiver operator characteristic curve,ROC curve)和校准曲线,评估模型的区分度和一致性(H-L拟合优度检验)。

2 结果

2.1 超声内镜下经鼻-肠梗阻导管置管图示

2.2 影响超声内镜下经鼻-肠梗阻导管置管术后预后不良的危险因素分析

2.2.1 单因素分析

预后不良组年龄大于预后良好组,CRP、PCT和NLR水平高于预后良好组,住院时间长于预后良好组,糖尿病、腹痛进行性加重和应用激素比例高于预后良好组,BMI、术前白蛋白水平和术前营养支持比例低于预后良好组,差异均有统计学意义(P < 0.05)。见表1

2.2.2 多因素Logistic回归分析

以患者术后是否出现预后不良(否 = 0,是 = 1)为因变量,年龄(≥ 68岁 = 1, < 68岁 = 0)、BMI( < 22.31 kg/m2 = 1,≥ 22.31 kg/m2 = 0)、糖尿病(否 = 0,是 = 1)、腹痛进行性加重(否=0,是=1)、术前营养支持(否 = 0,是 = 1)、术前白蛋白( < 32.47 g/L = 1,≥ 32.47 g/L = 0)、应用激素(否 = 0,是 = 1)、住院时间(≥11 d = 1, < 11 d = 0)、CRP水平(≥ 5.21 mg/L = 1, < 5.21 mg/L = 0)、PCT水平(≥ 1.21 μg/L = 1, < 1.21 μg/L= 0)和NLR水平(≥ 2.10 = 1, < 2.10 = 0)为自变量,进行多因素Logistic回归分析(引入水准为0.05,排除水准为0.107),结果显示:年龄≥ 68岁(OR^ = 2.631,95%CI:1.927~3.593)、BMI < 22.31 kg/m2OR^ = 2.142,95%CI:1.436~3.195)、术前白蛋白 < 32.47 g/L(OR^ = 1.962,95%CI:1.506~2.556)、无术前营养支持(OR^ = 2.814,95%CI:1.401~5.654)均是影响超声内镜下经鼻-肠梗阻导管置管术后预后不良的独立危险因素(P < 0.05)。见表2

2.3 预测超声内镜下经鼻-肠梗阻导管置管术后预后不良风险的列线图模型及验证

2.3.1 列线图模型

表2中的年龄、BMI、术前营养支持和术前白蛋白引入R软件,建立预测超声内镜下经鼻-肠梗阻导管置管术后预后不良的列线图模型。结果显示:年龄每增加5岁,会增加4.45分的权重,BMI每降低0.50 kg/m2,会增加8.33分的权重,术前白蛋白每降低1.00 g/L,会增加5.00分的权重,无术前营养支持,会增加13.50分的权重。见图3

2.3.2 模型效能验证

采用H-L拟合优度和ROC curve检验模型效能,H-L拟合优度检验显示:该模型一致性较好(χ2 = 2.65,P = 0.954)。ROC curve显示:该模型区分度较好(曲线下面积为0.887,95%CI:0.844~0.929)。见图4

2.3.3 效能外部验证

外部验证人群中,预后不良组年龄大于预后良好组,BMI和术前白蛋白水平低于预后良好组,术前营养支持比例低于预后良好组,差异均有统计学意义(P < 0.05)。H-L拟合优度检验和ROC curve结果均提示:一致性和区分度均较好[曲线下面积0.927(95%CI:0.903~0.952)]。见表3图5

3 讨论

3.1 肠梗阻的治疗方法

肠梗阻是由于肠腔内压升高时,液体气体积聚、肠壁微循环异常和黏膜屏障损坏导致的体液移位、移行性腹膜炎和低血容量性疾病[10]。肠梗阻治疗的基本原则为胃肠减压,随着减压后积聚的体液减少,自然恢复肠道功能,从而达到消除梗阻的目的[11]。目前,针对肠梗阻患者,治疗的首要手段为留置经鼻减压管,有胃管和肠梗阻导管两种。其中,经鼻胃管长度有限,减压作用较小,经鼻肠梗阻导管的应用则更加可靠。而超声内镜综合了透视与内镜的优点,简便可靠[12]。因此,本研究以超声内镜下经鼻-肠梗阻导管置管术治疗肠梗阻患者,分析导致疗效差异的可能因素,结果发现:451例行手术的患者中,有64例术后预后不良,占比14.19%,略低于王萌等[13]的研究,可能与疗法、患者年龄和病因等有关。

3.2 超声内镜下经鼻-肠梗阻导管置管术后预后不良的独立危险因素

本研究先对64例预后不良患者与387例预后良好患者的临床资料进行单因素分析,对单因素分析中差异有统计学意义的危险因素进行多因素Logistic回归分析,发现:年龄≥ 68岁(OR^ = 2.631,95%CI:1.927~3.593)、BMI < 22.31 kg/m2OR^ = 2.142,95%CI:1.436~3.195)、术前白蛋白 < 32.47g/L(OR^ = 1.962,95%CI:1.506~2.556)、无术前营养支持(OR^ = 2.814,95%CI:1.401~5.654)为影响超声内镜下经鼻-肠梗阻导管置管术后预后不良的独立危险因素。不同年龄会影响列线图得分,年龄越大,预后不良的可能性越高。分析原因为:患者年龄越大,机体基本功能越差,且年龄越大,越可能伴随各种基础疾病,患者体质较弱,机体基本的储备功能较低,肠道功能减弱。因此,更可能出现各种并发症,术后恢复也更慢,出现预后不良的概率更高[14-15]。针对年龄较大的置管术者,应在术前评估患者身体机能,基于检查结果,及时调整身体指标,减少使用刺激胃肠道的药物,以便提高术后恢复能力,同时,应避免受到自身基础疾病的影响,造成疗效不佳的情况。BMI的降低,亦会造成列线图权重的增加。分析原因为:BMI越低,机体能量储备能力和营养状况可能越差,其应对外界刺激的能力也越弱,在接受置管术后,其抵抗营养不足的性能会更差[16-17],术后机体更加虚弱,免疫能力更差,出现消化道症状恢复不佳等情况的可能性亦更高。针对BMI处于较低水平的手术患者,应注意术前和术后能量的补充,以提高患者应对损伤的能力,降低并发症率。术前白蛋白水平越低,列线图权重越高。究其原因为:营养不良是导致术后出现并发症的一个重要诱因,白蛋白水平为营养状况的体现,其水平越低,表示机体营养水平越低下,可能对机体压力产生负面影响,如:造成血浆渗透压的升高[18],引发组织水肿,降低术后恢复能力等。因此,白蛋白水平可以初步预测疗效情况。无术前营养支持,可增加13.50分的权重,已知白蛋白低水平,即机体营养状况较差时,预后不良的可能性更高,针对需行置管术的患者,为了降低其感染风险,应于术前行充足的营养储备。肖田等[19]研究表明,营养支持可有效改善肠梗阻新生儿术后营养状态和并发症情况,接受营养支持的新生儿,术后营养状态比无营养支持的新生儿更好,并发症更少,与本研究结果一致。由此可见,术前营养支持,不仅可以改善患者营养状况,还能够提高患者的抵抗力,增强营养吸收能力,及时纠正术后消化液不足和营养缺乏等状况[20],降低术后恢复不良的可能性。因此,针对白蛋白水平较低的患者,及时行术前营养支持,可改善疗效,同时,白蛋白水平处于正常情况时,亦可结合患者身体状况,及时进行营养支持,以降低并发症的发生率。

3.3 预测模型的建立及验证

本研究基于年龄、BMI、术前白蛋白和术前营养支持建立的列线图模型,可为行超声内镜下经鼻-肠梗阻导管置管术的肠梗阻患者疗效的预测,提供理论基础。为验证模型可靠性,本研究进行了H-L检验,并绘制ROC curve,内部和外部结果验证均证实了该模型的实用性。对于需行置管术的肠梗阻患者,首先需关注其年龄和BMI,若年龄较大,BMI较低,则应给予术前干预,纠正身体机能,确定用药方案和是否需要术前营养支持,若患者白蛋白水平较低,应及时补充营养,以降低预后不良的概率。

综上所述,年龄 ≥ 68岁、BMI < 22.31 kg/m2、术前白蛋白 < 32.47 g/L和无术前营养支持均是行超声内镜下经鼻-肠梗阻导管置管术的肠梗阻患者发生预后不良的危险因素,本研究以此4项因素为基准建立的列线图模型,可靠性和实用性均较高。
