肿瘤组织T细胞亚群与结直肠癌术后化疗疗效的相关性

摘要

探讨肿瘤组织T细胞亚群与结直肠癌患者术后化疗疗效的相关性。选取2021年1月—2024年5月于保定市第二中心医院进行化疗的95例结直肠癌患者为研究对象,化疗完成后参照RECIST疗效评价标准评估患者化疗疗效,将发生疾病进展的患者纳入进展组(n=54),未出现疾病进展的患者纳入未进展组(n=39),2例未完成化疗不计入组内。采用免疫组织化学方法测量术后CD3 +T细胞、CD4 +T细胞、CD8 +T细胞、FoxP3 +Tregs细胞表达状况,通过多因素Logistic分析影响结直肠癌术后化疗疗效的影响因素,采用受试者工作特征(ROC)曲线评估肿瘤T细胞亚群对结直肠癌术后化疗疗效的预测能力。两组CEA水平及CD3 +、CD4 +、CD8 +、FoxP3 +Tregs等T淋巴细胞亚群的比例,差异均有统计学意义(P<0.05)。单因素及多因素Logistic回归分析证实,CD8 +T细胞(OR=0.394,P=0.003)与FoxP3 +Tregs细胞(OR=0.257,P=0.001)是结直肠癌术后化疗疗效的保护因素。ROC曲线显示,CD8 +T细胞(AUC=0.901,95%CI:0.837~0.966)和FoxP3 +Tregs细胞(AUC=0.940,95%CI:0.892~0.989)能够预测结直肠癌术后化疗疗效(均P<0.05)。CD8 +T细胞、Foxp3 +Tregs细胞水平与结直肠癌术后化疗疗效密切相关,且可预测结直肠癌患者术后化疗疗效。
