血液透析患者动静脉内瘘“互联网+”随访信息平台的构建及临床应用

Construction and Clinical Application of an “Internet+” Follow-Up Information Platform for Arteriovenous Fistulas in Hemodialysis Patients

摘要

目的:构建“互联网+”信息平台并评价其在维持性血液透析(MHD)患者动静脉内瘘(AVF)中的应用效果。方法:选取2023年10月—2024年10月在宜昌市中心人民医院两院区血液净化中心行MHD的80例患者,分为采用“互联网+”信息平台的观察组(n=40)和对照组(n=40),收集两组患者术后随访资料,比较两组患者干预前后AVF初级通畅率、辅助初级通畅率、次级通畅率、并发症发生率及自我管理能力。结果:观察组患者初级通畅率、辅助初级通畅率显著高于对照组(均P<0.05)。与对照组相比,观察组患者并发症总发生率明显降低(22.50% vs 45.00%),自我管理能力显著高于对照组(均P<0.05)。结论:“互联网+”随访平台的应用有效提升了患者自我管理能力、降低AVF并发症的发生率,提高AVF通畅率,可作为血管通路随访管理的重要措施。

Abstract

Objective: To construct an “Internet+” information platform and evaluate its application effects in maintaining arteriovenous fistula (AVF) in patients undergoing maintenance hemodialysis (MHD). Methods: A total of 80 patients undergoing MHD at the Department of Blood Purification Center of Yichang Central People's Hospital from October 2023 to October 2024 were selected and divided into an intervention group (n=40) using the “Internet+” information platform and a control group (n=40). Postoperative follow-up data of both groups were collected, and the AVF primary patency rate, assisted primary patency rate, secondary patency rate, incidence of complications, and self-management ability of patients in both groups were compared before and after intervention. Results: The primary patency rate and assisted primary patency rate of patients in the intervention group were significantly higher than those in the control group (both P<0.05). Compared with the control group, the total incidence of complications in the intervention group was significantly reduced (22.50% vs 45.00%), and the self-management ability was significantly higher than that of the control group (both P<0.05). Conclusion: The application of the “Internet+” follow-up platform effectively improves patients' self-management ability, reduces the incidence of AVF complications, and improves the patency rate of AVF, which can be used as an important measure for vascular access follow-up management.
