Role and mechanism of aerobic exercise in delaying cognitive decline

摘要

年龄相关认知功能衰退(age-associated cognitive decline,AACD)是一种增龄性的记忆、注意、思维能力的下降,增加罹患阿尔茨海默病(Alzheimer’s disease,AD)、帕金森病(Parkinson’s disease,PD)的风险,其潜在的生理学机制可能是由于年龄增加使得神经元内的信号传导通路失衡,神经元损伤,认知功能下降。因此,清除受损或衰老的细胞,有助于促进神经元修复,维持健康的细胞环境具有重要作用。来自骨骼肌、肝脏和脂肪组织在运动负荷刺激下分泌运动因子,细胞外囊泡(extracellular vesicles,EVs)作为运动因子的有效载体能够透过血脑屏障,维持大脑神经元的内稳态,进而起到延缓认知衰退的目的。本文综述了肌肉、肝脏和脂肪组织衍生的运动因子通过EVs介导修复受损神经元,改善大脑的代谢和认知功能,为有氧运动延缓认知衰退的作用及机制提供理论基础。

Abstract

Age-associated cognitive decline is characterized by a progressive deterioration of memory,attention,and thinking abilities associated with age and may increase the risk of Alzheimer’s disease and Parkinson’s disease,with the potential physiological mechanism of age-induced imbalance in neuronal signal transduction pathways,thereby leading to neuronal damage and subsequent cognitive decline. Therefore,the clearance of damaged or senescent cells and the promotion of neuronal repair are critical for maintaining a healthy cellular environment. Exercise factors secreted by skeletal muscle,liver,and adipose tissue in response to physical activity play a pivotal role in modulating brain homeostasis,and as efficient carriers of these exercise factors,extracellular vesicles(EVs) can cross the blood-brain barrier(BBB) and maintain neuronal homeostasis,thereby delaying cognitive decline. This article reviews the mechanisms by which the exercise factors derived from muscle,liver,and adipose tissue improve brain metabolism and cognitive function through the repair of damaged neurons mediated by EVs,which provides a theoretical basis for the role and mechanism of aerobic exercise in delaying cognitive decline.

运动缺乏及不良的饮食结构等不健康的生活方式增加罹患中枢神经系统疾病的风险,可引起大脑记忆、思维、认知加工、情绪调节等受损,加速年龄相关认知功能衰退(age-associated cognitive decline,AACD)的病理进程。运动作为一种无创的干预手段,有利于维持机体-大脑的代谢稳态。规律性运动刺激机体产生适应性改变,这一系列经典的运动效应包括血管生成增加、线粒体生物合成增强、心脏与免疫细胞功能改善,以及脂肪组织与肝脏对底物的代谢能力的增强。这些适应性改变最终使训练个体形成预适应状态,从而对后续病理或生理性刺激(如运动或慢性疾病)产生保护作用。尽管运动训练具有明显的健康效益,但导致组织结构与功能适应的精确分子信号转导机制目前仍未完全阐明。研究表明,机体受到运动负荷刺激将生物活性物质释放到循环血液中,这些生物活性物质被称为运动因子。按照来源可分为肌动蛋白因子、脂肪因子、肝因子;按照功能可以分为神经营养因子、血管生长因子、组织代谢因子。细胞外囊泡(extracellular vesicles,EVs)是运动产生生物学效应的有效载体。本文以运动因子学说为理论框架,旨在系统梳理由运动直接刺激骨骼肌、肝脏及脂肪组织等外周器官分泌的活性因子。本文聚焦于那些能够穿越血脑屏障,并最终对神经发生与突触重塑过程发挥关键调控作用的运动因子[1-2]

1 “运动因子”是运动延缓认知功能衰退的重要介质

早在21世纪初,Pedersen BK[3]发现了多种运动诱导性的信号分子。在急性运动过程中,骨骼肌、心肌、肝脏及脂肪组织等多种器官会分泌生物活性物质,包括多肽、蛋白质、组织代谢物、脂质及核酸等成分,其中部分分子同时具有激素或细胞因子功能,这类物质被统称为“运动因子”。迄今为止,仅5%肌动因子的生物学和生理学功能被明确定义。大量研究表明这些运动因子通过EVs实现大脑-肌肉通信 [4]。阐明运动因子的整体动态特征对于AACD的运动处方提供理论依据。

1.1 肌动因子通过影响神经元线粒体生物合成起到神经保护作用

肌动因子被定义为“由肌纤维产生、表达和释放的细胞因子或肽”[5]。运动诱导的肌动因子,包括脑源性神经营养因子(brain derived neurotrophic factor,BDNF)、组织蛋白酶B(cathepsin B,CTSB)、鸢尾素、乳酸(lactic acid,LA),白细胞介素-6(interleukin,IL-6)等,这些分子统称为“肌动蛋白组”[6]。它们都参与运动信号传导,调节成人神经发生和认知功能。运动刺激肌纤维收缩,产生并释放肌动蛋白,其释放多少由运动量、强度和频率等因素决定[7],在这个过程中骨骼肌与其他器官组织相互作用共同促进神经发生。

1.1.1 BDNF

BDNF主要来源于骨骼肌和大脑,运动负荷刺激骨骼肌产生BDNF,BDNF能通过血脑屏障进入大脑,促进神经细胞的生长和存活,并增强突触可塑性。临床研究已经发现,神经退行性疾病如帕金森病(Parkinson’s disease,PD)等大脑中的神经元丢失与BDNF减少有关,运动能够增加PD患者血液中的BDNF含量。BDNF可以维持神经元的功能和小胶质细胞的分化与存活,并调控神经可塑性、参与神经发生及突触形成。在神经系统发育的全过程中,BDNF在神经元中持续表达,覆盖了从早期形成到成熟完善的各个阶段[8]。BDNF激活过氧化物酶体增殖物激活受体γ共激活因子1α(peroxisome proliferator-activated receptor gamma coactivator-alpha,PGC-1α),从而增加线粒体三磷酸腺苷(adenosine triphosphate,ATP)的生物合成,促进突触的形成。相反,PGC-1α能增加BDNF的表达水平,PGC-1α敲低后,突触形成减少,且BDNF刺激突触形成的能力减弱[9]。其次,研究表明,BDNF与线粒体生物合成直接相关[10]。上调BDNF可以促进小鼠海马神经元线粒体的生物合成[11]。在阿尔茨海默病(Alzheimer’s disease,AD)患者中其大脑BDNF水平降低,运动可以上调BDNF的表达,改善AD患者的认知功能。人体实验研究表明,急性运动和长期运动均上调BDNF水平的表达。急性运动即刻增加了循环血液中BDNF,之后便恢复到原有水平,这可能是由于运动终止,研究对象的个体差异造成。长期有氧运动通过激活AMP活化蛋白激酶[adenosine 5’-monophosphate(AMP)-activated protein kinase,AMPK]信号转导通路,促进突触可塑性,增强了记忆与学习的直接内部机制[12]。因此,优化的运动策略应兼具持续性与优质性,即在保障长期运动累积效应的同时,通过精细化设计确保每次运动的质量与短期收益。

1.1.2 Irisin

Irisin是一种运动诱导的肌动蛋白,是纤连蛋白Ⅲ型结构域5(recombinant fibronectin type Ⅲ domain,FNDC5)的一个裂解蛋白,通过激活PGC-1α/FNDC5/BDNF信号轴发挥明显的神经生物学效应。该通路可促进胚胎干细胞向神经元分化,并上调BDNF及其他神经保护相关基因的表达。研究表明,在皮质培养物及小鼠海马中,过表达FNDC5可提高BDNF水平,而其敲除则产生相反效果,由此证实Irisin具有增强突触可塑性及神经保护的作用。重要的是,这些机制构成了运动所产生益处的生物学基础,Irisin参与改善神经退行性病理进程,在AD大鼠模型中运动能够通过FNDC5/Irisin增强突触可塑性并改善记忆功能,从而确立了FNDC5/Irisin作为运动诱导神经保护作用的关键介质[13-15]

1.1.3 IL-6

IL-6由骨骼肌分泌,在运动的刺激下释放进入外周循环,是影响内分泌的重要因素[16]。IL-6是发现最早且研究最为广泛的肌因子之一,已在多项研究及综述中被深入探讨[17-19]。在肌因子概念确立前,该分子曾被称为β2干扰素。1989年,该分子被正式命名为“IL-6”[20]。Pedersen BK[21]首次提出其可能作为“运动因子”的理论。

尽管IL-6通常被视为促炎细胞因子,但骨骼肌收缩时释放的IL-6却表现出明显的抗炎特性。其血浆浓度可在运动后迅速升高至基线水平的100倍,并在短期内恢复至基础水平。这种抗炎作用具有肌肉组织特异性及快速达峰的特点[22-25]。此外,IL-6能诱导其他抗炎细胞因子的产生,同时抑制肿瘤坏死因子-α和白细胞介素-1β的生成。在代谢调控方面,IL-6通过增强胰岛素敏感性和促进葡萄糖转运蛋白4聚集发挥重要调控作用[26-28]。其作用机制包括在肌肉局部通过激活AMPK进入循环后以激素样方式作用于外周组织[29-30]

在中枢神经系统中,这些外周循环中的IL-6通过影响神经元线粒体代谢,进而起到神经保护作用[31]。在啮齿类动物模型中,通过增强肌肉来源的IL-6来调节下丘脑肽的表达。在体外培养的星形胶质细胞中上调IL-6的表达,可提高PGC-1α蛋白水平[32]。大脑中海马区IL-6的表达可以抑制神经元的凋亡,长期运动通过激活AMPK信号通路调控IL-6的表达,其表达量与运动强度和时间呈正相关。具体而言,能够持续激活AMPK的中等强度有氧运动(如持续跑步、骑行)和高强度间歇训练是诱导IL-6水平明显升高的有效方式。研究表明,敲除大鼠大脑中的IL-6受体后则会抵消运动带来的神经保护作用[33]。因此,运动诱发的肌源性IL-6的升高是实现神经保护作用的重要介质[34-37]

1.1.4 CTSB

CTSB是一种溶酶体半胱氨酸蛋白酶,由肌肉在运动负荷的刺激下而分泌,对认知具有有益影响。已证明其释放与PGC-1α,BDNF的水平呈正相关。在神经元中,CTSB通过调节促凋亡分子的释放参与神经元线粒体细胞死亡信号的传导[38]。研究表明,在小鼠中进行为期3个月的有氧跑台运动(每周5次,每次30 min)所诱导的CTSB增加,导致神经发生和记忆改善[39]。诸多研究证实,CTSB是介导运动改善认知功能的关键中介因子。在AD等多种脑部疾病模型中,运动诱导的认知改善作用均被发现依赖于CTSB。机制上,为期30 d的跑台运动可明显提升小鼠腓肠肌和血浆中的CTSB水平;而外源性应用重组CTSB则能直接增强成年海马祖细胞的BDNF表达,提示其可能通过调控脑内神经营养环境来发挥作用。来自基因敲除小鼠的研究表明:在CTSB缺失时,跑步运动对海马神经发生和空间记忆的促进作用完全消失。这一从动物到临床的完整证据链(恒河猴与人类研究均显示运动后血浆CTSB水平升高,且其与体能和记忆功能正相关)共同确立了CTSB作为核心肌脑因子的重要地位。

1.1.5 LA

LA从骨骼肌中释放,导致血液中LA水平升高,影响大脑代谢以及初级运动皮层兴奋性。相较于传统意义上LA是无氧代谢的副产物,依据最新“LA穿梭理论”,LA是线粒体功能调节的信号分子,且能够调节神经元的兴奋性[40-44]。在大脑中,LA不仅是能量来源,还参与谷氨酸能信号传导。此外,人体研究表明,运动引起的LA升高或外源性LA血管输注均可刺激BDNF的释放,该因子对神经发生和认知功能具有重要作用[45-47]

有氧跑台运动刺激大鼠骨骼肌产生的LA能够通过血脑屏障,并参与海马体中BDNF介导的AMPK信号传导,从而改善学习和记忆能力[48-49]。研究表明,下丘脑对循环血液中的LA具有敏感性,这种敏感性维持了大脑葡萄糖代谢的内稳态,例如,机体缺氧的条件下,会出现LA的增高,而循环中的LA可以在血糖不足时满足大脑兴奋性活动。Taha HB等[50]用LA处理原代海马神经元,证明了LA可增强氧化磷酸化相关基因,提高海马ATP水平。因此,运动刺激机体产生的LA对神经元能量代谢至关重要。

高强度运动(high intensity intermittent training,HIIT)通过PGC-1α和G蛋白偶联受体通路,调控线粒体的分裂与融合动态平衡。这一过程有助于维持神经元内线粒体网络的稳态、及时清除受损线粒体,从而优化能量代谢并减少氧化应激,最终实现对神经元的保护作用。研究表明,HIIT诱导肌肉中LA的水平升高,改善大脑线粒体功能,促进神经元生成[51-53]。相较于有氧运动和抗阻运动,HIIT运动中高的运动负荷更能够有效刺激骨骼肌中LA的生成。在运动处方中,在个体化运动及疾病状况的基础上,加入HIIT运动要素,同时避免过量运动造成LA堆积,所产生的负面效应。

1.2 肝因子通过调节神经元线粒体酶活性和过氧化水平起到神经保护作用

肝脏衍生的因子包括β-羟基丁酸(B-hydroxybutyrate,BHB),成纤维细胞生长因子21(fibroblast growth factor 21,FGF21)等。运动负荷刺激肝脏产生并释放肝因子,调节线粒体的功能[54]。并对远端大脑产生有益影响。

1.2.1 BHB

BHB是一种由肝脏合成和分泌的物质。在葡萄糖供应有限的情况下,如禁食和运动时,脂肪细胞会分泌脂肪酸,然后由肝脏将其转化为BHB。运动过程中代谢需求增加,肝脏分泌和合成BHB水平随之提高,循环血液中的BHB能够通过血脑屏障,在海马体中积累,增加BDNF启动子中的组蛋白乙酰化,进而增加BDNF的表达。BHB是运动增加大脑BDNF表达的重要中介因素,对神经元起到保护作用。研究表明,长时间的有氧运动相较于短时间急性运动在刺激脂肪细胞方面更具有优势,长时间的有氧运动能够刺激脂肪细胞分泌脂肪酸,促进BHB的生成[55-56]。因此,应合理安排运动时间,避免运动时间两极化所带来的负面影响。

大脑可氧化BHB产生ATP,增加神经元线粒体呼吸能力。Tieu V等[57]证明,在PD模型中,BHB通过复合体Ⅱ依赖性方式增加神经元线粒体耗氧量。Marosi C[58]发现,经BHB处理的神经元线粒体呼吸速率、ATP产生均增加;阻止BHB诱导的线粒体有氧呼吸,会抵消BHB对黑质致密部多巴胺能神经元的神经保护作用。对原代培养的大鼠皮层神经元进行BHB处理,可增加线粒体呼吸能力,这表明外源性BHB的神经保护作用可能通过调节线粒体呼吸能力来实现。

其次,BHB通过改善线粒体功能介导运动对认知的积极影响。其机制在于BHB通过提升线粒体复合物Ⅰ和Ⅳ的活性、增强抗氧化能力并抑制动力蛋白相关蛋白1(dynamin-related protein 1,Drp1)介导的过度分裂,从而逆转神经元线粒体碎片化。在衰老大鼠模型中,这一系列线粒体层面的改善被证实是BHB增强记忆功能的基础[59]

1.2.2 FGF21

FGF21是一种运动介导的肝因子,能够通过血脑屏障,降低活性氧(reactive oxygen species,ROS)水平,起到神经保护作用。在建立氧化应激模型的大鼠多巴胺能神经元中,与未处理的应激组相比,FGF21预处理可有效逆转由应激诱导的ROS水平升高[60]

在高脂饮食6周后,大鼠腹腔注射FGF21持续6周可降低ROS水平,并减少神经元线粒体肿胀。这些结果表明,FGF21通过减少ROS的表达,进而改善线粒体内环境,对肥胖大鼠发挥神经保护作用 。

其次,运动能够提高肝脏释放进入血清的FGF21,且FGF21水平与运动强度呈现正相关[61]。研究表明,FGF21水平与体力活动的运动量存在相关性,单次运动干预后未观察到健康成年人血清FGF21水平差异有统计学意义。2周每日规律体力活动后,受试者血清FGF21水平明显升高,研究表明,在抑郁小鼠模型中,运动通过FGF21介导了认知改善效应。敲除FGF21不仅阻断了运动抗抑郁的作用,也消除了其对学习记忆的促进;而外源性补充FGF21则能模拟运动效益,通过改善海马区的氧化应激状态、增强神经发生,最终提高小鼠的空间学习与记忆能力[62]。因此,在运动过程中,渐进性地增加运动强度,使得机体逐渐适应运动负荷的刺激,发挥运动的积极效用。

1.3 脂肪因子脂联素(adiponectin,ADPN)通过抑制神经元线粒体的损伤起到神经保护作用

ADPN是一种由脂肪细胞分泌到循环血液中的蛋白质,作为信号分子发挥神经保护作用[63]。ADPN可以抑制线粒体碎裂,起到神经保护作用。研究表明,在缺血性体外模型中ADPN可以降低缺血条件下星形胶质细胞中的线粒体碎裂。在衰老大鼠模型中,Fang PH等[64]通过腹腔注射ADN受体激动剂,发现其激活可提升大脑ATP水平、减轻线粒体损伤,并通过上调PGC-1α等蛋白的表达促进神经元线粒体生物合成。此外,在脑出血动物模型中,激活ADPN受体可增加整体神经元线粒体功能并减弱线粒体膜的超极化,ADPN能够穿过血脑屏障,促进细胞增殖并改善小鼠的抑郁样行为[65]

运动诱导ADPN的水平变化具有剂量效应 [66]。中等至高强度运动方案可能对ADPN水平产生最明显的影响,但在确立确切的剂量-效应关系之前,尚须开展更高质量的随机对照试验。Simpson KA 等[67]在对有氧运动(51项研究)和抗阻训练(35项研究)的研究中均显示出中等至较大运动强度的干预效应,这表明达到足够强度的运动可能是调控ADPN水平的关键因素。现有证据显示,运动对认知的改善作用结论一致,这可能是社会心理(如运动作为社会交往)与生理机制共同作用的结果。但其背后的生理机制因运动方式而异。Fatouros IG等[68]研究发现抗阻训练能通过体成分改变剂量依赖性的提升ADPN等肌源性因子来发挥效用。而有氧运动的相关效应则较弱且与强度无关。这提示不同运动模式可能通过截然不同的生理途径(如抗阻训练依赖ADPN等肌源性因子,而有氧运动可能更依赖血管与直接的细胞应激反应)最终共同作用于大脑。未来需在统一认知评估的前提下,厘清不同运动方式特异性的神经保护路径。

2 细胞外囊泡是运动刺激骨骼肌、肝脏、脂肪释放生物活性物质延缓认知功能衰退的有效载体

运动过程中,肌肉、脂肪、肝脏释放的生物活性物质,通过 EVs透过血脑屏障,实现运动负荷刺激下通过“外周”实现对“中枢”的调节作用。有氧运动增强大脑中有害物质的清除能力,通过影响全身参数来促进远隔部位的“重编程”,这些参数被压力感受器、化学感受器感知,促进生物活性物质的重编程[69]

EVs可以将运动因子中的线粒体组分包裹运输,间接影响大脑功能[70]。EVs包括外泌体、微囊泡、微颗粒、凋亡小体。EVs能够绕过生物屏障将生物活性物质输送到受体细胞[71]。EVs如外泌体和微囊泡,可以包裹线粒体或其组分,并通过体液,如血液或组织液,运输到远处的细胞。因此,运动因子可以包装在纳米大小的EVs中,并独立于经典的分泌释放机制。运动负荷刺激,骨骼肌中的EVs会释放到全身循环中[72]。然而,目前评估EVs的来源具有挑战性,因此线粒体转运在运动诱导的大脑效应中的作用仍未明确。有研究发现,向血液中注射分离的线粒体可导致其在大脑中重新定位,并能改善AD和PD啮齿类动物模型的神经内稳态及认知行为。其有益机制在于,这些外源性线粒体可被神经元摄取,直接补充缺陷细胞的能量供应,同时通过降低局部氧化应激水平、并可能替代或修复受损的线粒体网络,从而发挥神经保护作用[73]

骨骼肌细胞可释放EVs并进入血液循环系统。目前已鉴定的肌因子中高达75%存在于EVs内,自由活动状态下,骨骼肌纤维来源的EVs约占循环EVs总量的4%~5%。有研究观察到,受试者完成1 h骑行运动后,其血浆中肌因子及EVs高富集蛋白的浓度明显升高;肌肉组织在运动刺激下会向血液中释放携带微小RNA的EVs,跑步运动可促使骨骼肌释放含有数百种多肽的EVs[74]。多项研究表明,EVs可能介导运动诱导的肌因子释放,延缓AACD病理过程。

运动诱导的EVs释放是运动延缓认知障碍发生的关键中介机制。研究表明,运动负荷与EVs的变化之间存在明确的剂量效应,高强度急性运动能作为有效刺激,使循环EVs水平呈现快速、瞬时的上升,且其增幅与运动强度呈正相关。这一现象在动物实验中得到进一步证实,小鼠的内皮细胞源性EVs水平也呈现运动剂量依赖性升高。更重要的是,这些由运动调控的EVs并非孤立变化,它们直接参与了认知功能的改善过程。实验表明,来自经过运动训练的健康供体小鼠的EVs,能够在AD模型小鼠中介导运动的认知益处——具体表现为在巴恩斯迷宫测试中,AD小鼠的长期记忆能力得到明显增强,找到目标孔的时间和错误次数减少,同时其大脑皮层内的淀粉样β蛋白病理沉积也明显减少。因此,运动处方中采用渐增负荷以优化EVs释放,并探索其剂量效应拐点,对于最大化认知保护效益并避免过度运动的负面作用至关重要[75-76]。表明运动能够通过EVs延缓认知功能的衰退。

3 小结与展望

基于整合的生理学视角,机体处于运动因子形成的动态平衡的网络中。运动作为一个外部刺激作用于该网络,诱导形成新的动态平衡的运动因子网络系统,实现新的更为健康的动态平衡状态。①运动刺激内分泌器官释放的运动因子改善认知功能衰退尚处于初级阶段,运动因子网络动态平衡中所涉及的器官因子有待挖掘,其分子路径有待明晰。②应充分考虑运动因子的释放受到运动强度、类型、运动方式的综合影响,且要充分考虑年龄、性别、体力活动水平,制定个性化的运动处方。
