摘要

老年抑郁症(late-life depression,LLD)是老年人群中常见的精神障碍,具有高复发率和认知损伤等特点,严重影响患者生活质量和社会功能。近年来,非侵入性神经调节技术(non-invasive brain stimulation,NIBS)因其安全性高、依从性好和可推广性强,逐渐成为抑郁障碍非药物治疗的重要手段。本文旨在系统综述NIBS技术在LLD治疗中的研究进展,综述了经颅磁刺激(repetitive transcranial magnetic stimulation,rTMS)、经颅直流电刺激(transcranial direct current stimulation,tDCS)、经颅交流电刺激(transcranial alternating current stimulation,tACS)、经颅随机噪声刺激(transcranial random noise stimulation,tRNS)以及经颅聚焦超声刺激(transcranial focused ultrasound stimulation,tFUS)在LLD 中的应用现状,重点讨论其作用机制、临床应用现状及疗效评价。现有研究显示,rTMS、tDCS与tACS在改善LLD抑郁症状及部分认知功能方面疗效明确,安全性良好,而tRNS与tFUS尚处于探索阶段。总体而言,NIBS技术为LLD提供了新的干预途径,未来的研究应聚焦于长期随访、参数标准化、引入客观疗效指标以及多模态联合应用等关键领域,以推动NIBS在LLD治疗中的精准化和规范化发展。

Abstract

Late-life depression(LLD) is a common mental disorder in elderly people,characterized by a high recurrence rate and cognitive impairment,which severely impacts the quality of life and social functioning of patients. In recent years,noninvasive brain stimulation(NIBS) techniques have emerged as an important non-pharmaceutical treatment method for depressive disorders due to their favorable safety profiles,good compliance,and strong potential for widespread application. This article systematically reviews the research advances in NIBS techniques in the treatment of LLD and summarizes the current status of the application of repetitive transcranial magnetic stimulation(rTMS),transcranial direct current stimulation(tDCS),transcranial alternating current stimulation (tACS),transcranial random noise stimulation(tRNS),and transcranial focused ultrasound stimulation(tFUS) in LLD,with a focus on mechanism of action,clinical application,and efficacy evaluation. Current studies show that rTMS,tDCS,and tACS have demonstrated good efficacy and safety in improving depressive symptoms and certain aspects of cognitive function,while tRNS and tFUS are still in the exploratory stage. Overall,NIBS techniques provide new interventions for LLD,and future research should focus on key areas such as long-term follow-up,parameter standardization,introduction of objective outcome measures,and multimodal combined applications,in order to promote the precise and standardized development of NIBS in LLD treatment.

抑郁症是一种高患病率、低识别与治疗率、反复发作且致残负担高的常见精神障碍。本文所称“抑郁症”主要指《精神障碍诊断与统计手册(第5版)》(diagnostic and statistical manual of mental disorders,fifth edition,DSM-5)界定的重性抑郁障碍(major depressive disorder,MDD)。老年抑郁症(late-life depression,LLD)指年龄在60岁及以上的抑郁症患者[1],根据首次抑郁发作的年龄不同,可以分为早发型老年抑郁(early onset depression,EOD),指在60岁之前首次抑郁发作,以及晚发型老年抑郁症(late onset depression,LOD),即60岁以后首次抑郁发作[2]。老年人群受生理老化与社会心理因素共同影响,其抑郁症的患病形势尤为突出。1项Meta分析结果显示,我国老年人抑郁症状患病率为20.0%(95%CI=17.5%~22.8%)[3]。社区老年样本中重性抑郁障碍约为5%,明显抑郁症状为8%~16%。1项基于中国健康与养老追踪调查项目的最新数据分析显示,我国老年抑郁症的患病率呈现上升趋势,且存在明显的城乡、性别及慢性病共病差异[4]。同时,我国人口老龄化问题日渐突出,老年人群中抑郁人数剧增,值得重视。
流行病学数据显示,LLD与认知受损高度并存:在我国老年抑郁人群中,约36.4%存在轻度认知功能障碍,10%~15%出现认知损伤,涉及学习记忆、注意与执行功能的下降等方面。这些认知缺陷不仅加重功能受限和照护负担,也与更高的复发风险及较差的长期预后相关[5-7],提示情绪与认知双重靶点的干预需求迫切。目前,抗抑郁药物仍是临床一线治疗的首选。然而在老年期,受生理老化、慢性躯体共病与多药联用、社会支持减少及认知老化等因素影响,LLD的临床表现更具异质性:典型情绪症状常被躯体化主诉与认知投诉所掩盖,易漏诊或误诊;同时药物不良反应敏感性增加、药物相互作用风险更高,导致常规药物治疗的有效性与可耐受性受限,进一步加重复发与致残风险。有研究发现,老年患者的难治比例与复发风险相对更高,即便抑郁症状缓解,残留的认知障碍仍可能延续并影响功能恢复[8]。基于此,探索兼顾疗效、安全与认知结局的非药物或联合治疗策略,具有明确的临床必要性。
非侵入性神经调节技术(non-invasive brain stimulation,NIBS),是一类通过物理手段(如磁、电、超声、激光等)无创调节神经系统功能的技术,包括经颅磁刺激(transcranial magnetic stimulation,TMS)、经颅直流电刺激(transcranial direct current stimulation,tDCS)、经颅交流电刺激(transcranial alternating current stimulation,tACS)、经颅随机噪声刺激(transcranial random noise stimulation,tRNS)和经颅聚焦超声刺激(transcranial focused ultrasound stimulation,tFUS)等[9]。近年来,NIBS在多种神经精神疾病治疗及脑功能研究中显示出潜在优势。针对抑郁障碍认知结局的证据提示,NIBS对总体认知功能与学习记忆等维度可能带来改善[10-11];同时,NIBS具有不良反应少、安全性高、操作简单、成本低,甚至部分技术已可实现居家操作等优势,鉴于老年群体在安全性与依从性方面的特殊考量,NIBS有望成为兼顾情绪缓解与认知改善的治疗方案[12-14]。本文以常见的NIBS为切入点,综述其在LLD中的治疗作用及潜在机制。

1 重复经颅磁刺激(repetitive transcranial magnetic stimulation, rTMS)

TMS是基于电磁转换原理,通过线圈产生的快速变化磁场诱发皮层电流,从而调节神经元兴奋性与网络活动[15]。根据不同的脉冲频率可分为单脉冲经颅磁刺激、双脉冲经颅磁刺激以及rTMS。其中rTMS以特定频率、强度、脉冲数与序列结构反复施加,能诱导更持久的皮层兴奋性变化与突触可塑性效应,近20年来多项临床试验已证实rTMS治疗抑郁障碍的有效性,并已广泛用于抑郁障碍的干预与机制研究[16-18]

rTMS治疗抑郁障碍的效果主要与刺激的频率、刺激靶点、刺激方式等相关。根据刺激频率的不同可分为高频(5~20 Hz)和低频(≤1 Hz),其中高频rTMS被认为可以增加皮层兴奋性而低频rTMS则起到抑制的效果[19]。在靶点选择上,左侧背外侧前额叶(dorsolateral prefrontal cortex,DLPFC)的高频刺激与右侧DLPFC的低频刺激是抑郁干预中应用最广的2类方案;多项研究与Meta分析提示二者均可改善抑郁症状[20-22]。DLPFC是情绪认知的重要枢纽,在调节情绪和执行认知控制方面发挥重要作用[23]。通过刺激DLPFC,rTMS可经前额叶-边缘系统环路改善前额叶的执行控制,下调杏仁核对负性刺激的过度反应,从而减少负面情绪。也有研究者提出应该关注其他刺激靶点。有研究发现,与DLPFC相比,背内侧前额皮质(dorsomedial prefrontal cortex,DMPFC)、额极皮质(frontopolar cortex,FPC)、腹内侧前额皮质(ventromedial prefrontal cortex,VMPFC)和腹外侧前额皮质(ventrolateral prefrontal cortex,VLPFC)等区域在重度抑郁症的情绪调节中发挥作用,是未来新刺激靶点研究的主要方向[24]

目前已有不少使用rTMS治疗LLD患者的临床试验研究。有研究将30例 LLD 患者随机分为3组,分别接受高频rTMS(20 Hz)、低频rTMS(1 Hz)或假刺激,在此期间,所有被试均持续服用相同抗抑郁药物。结果显示,3个组的抑郁症状均明显降低,高频rTMS组在治疗第5天即显示出明显改善,而低频组和假刺激组则在治疗结束时才显示出明显改善,但高、低频组对于抑郁症状改善没有明显差异[25],提示二者在LLD抑郁症状缓解方面可能疗效相近、但起效时程与耐受性存在差别。另一项研究发现,在共病帕金森的LLD患者中,低频rTMS刺激不仅能有效改善抑郁症状,还能有效改善认知功能,蒙特利尔认知评估得分提高并持续至6个月[26]。但是,也有Meta分析发现,虽然左侧高频和右侧低频的治疗效果相似,但高频刺激易诱发癫痫等并发症,安全性不及低频刺激,因此对于治疗LLD可考虑首选低频刺激治疗[27]。在刺激范围方面,部分研究提示,双侧刺激治疗LLD效果优于单侧刺激,其原因可能与老年人群双侧DLPFC皮质体积减小有关[28]。因此,双侧rTMS刺激DLPFC对于治疗老年抑郁可能是更理想的治疗方案,但仍需大量研究证实。另外,有研究发现,采用双侧序贯治疗(先进行低频1 Hz右侧DLPFC刺激,随后立即接受高频10 Hz左侧DLPFC刺激),每周5次,共3周比仅采用高频10 Hz刺激左侧DLPFC对老年抑郁症状缓解率更高[29]

目前国内研究仍多采用左侧DLPFC、刺激频率为10 Hz的刺激参数。与国外研究主要探索超高脉冲数(如单次3 000脉冲)不同的是,国内研究在保障安全前提下,通过增加频次(而非单次极高剂量)来累积疗效。同时,在治疗特点方面,因国内LLD患者多为住院治疗,故易于实施“rTMS+药物/认知训练”的综合方案[30-31]

近年来,rTMS的一种特殊形式——间歇性θ爆发刺激(intermittent theta burst stimulation,iTBS)正成为治疗抑郁症的热点之一。iTBS通过模拟自然脑电活动(θ节律)来诱导长时程增强(long-term potentiation,LTP)效应[32],单次治疗时间仅需192 s,其基础参数为每串脉冲频率50 Hz,重复频率为5 Hz,每次刺激持续2 s,休息8 s,总脉冲数600个。相较常规rTMS(约26 min),iTBS明显缩短疗程时间,其耐受性和治疗效率、依从性都更高,而对抑郁情绪的改善效果相当[33]。近年,Cole EJ等[34]采用斯坦福智能加速神经疗法研究发现,iTBS组的缓解率高达90.48%,蒙哥马利抑郁量表(Montgomery-Asberg Depression Rating Scale,MADRS)评分较基线下降≥50%。同时,多项研究证实该方案在治疗MDD 方面效果明显[35-36]。此外,1项研究将iTBS与10 Hz rTMS 在治疗难治性抑郁症的疗效进行对比研究,结果发现iTBS在改善抑郁症方面非劣效于标准10 Hz rTMS。并且2组的汉密尔顿抑郁评分(Hamilton Depression Scale,HAMD)在治疗后均明显降低,其非劣效性在第12周的随访中仍得到维持[37]。提示可考虑将iTBS作为10 Hz rTMS的高效替代方案。

目前已经有若干针对LLD的iTBS临床研究,都发现其在治疗抑郁情绪方面效果明显。比如1项研究刺激右侧DLPFC,显示抑郁评分下降≥50%,缓解率为20%[38]。Blumberger DM等[39]进行的1项随机非劣效临床试验证实,双侧iTBS治疗LLD非劣效于双侧rTMS,2种双侧治疗方案均采用了美国食品药品监督管理局(Food and Drug Administration,FDA)批准的左侧治疗(10 Hz rTMS或iTBS),然后进行右侧启动(1 Hz rTMS或iTBS),双侧iTBS组的缓解率为35.4%,与双侧rTMS组33%的缓解率相当。提示未来可考虑将 iTBS作为治疗LLD的方案之一。

除了抑郁情绪,iTBS在改善LLD认知功能方面同样也有着积极的效果,认知维度附加获益明显。1项对LLD进行双侧DLPFC刺激的开放标签研究显示,除了MADRS评分明显下降,Flanker任务得分也有明显提高,表示对于执行功能也有一定改善[40]。另外一项研究表明,iTBS组在接受治疗4周后,在工作记忆、执行功能、言语学习等认知领域均有所提高[41]

鉴于LLD患者常表现出情绪与认知的双重损害,探索并确定更适宜的刺激方式协同改善LLD的认知功能具有至关重要的意义。有学者先后进行了2项研究来探讨这个问题。在2020年,对伴有执行功能障碍的LLD患者进行单侧 DLPFC的 iTBS干预,结果显示抑郁症状明显改善之余,其主观及客观执行功能水平均有明显改善,抑郁与执行功能改善呈正相关[42]。但随后2022年的另一项小样本随机对照试验(randomized controlled trial,RCT)研究发现,双侧iTBS治疗虽使抑郁评分明显下降,但与假刺激组相比,抑郁症状及认知功能(如Flanker任务和工作记忆评估)的组间差异均不明显。提示双侧iTBS治疗LLD可能有效,但在改善认知功能方面可能并非最优刺激方式[43]。但这2项研究均为小样本(分别为11名和17名受试者),且刺激参数标准化不足。目前针对iTBS治疗LLD方面的治疗机制尚不明确,治疗参数并未标准化,临床方案仍需完善。因此,未来的研究需要更大的样本量、更精细的个性化靶向策略以及对认知功能影响的进一步分析。

同时,为了客观评价rTMS的疗效,克服量表评估的主观性,近年来研究引入了多模态神经指标。功能性近红外光谱成像(functional near-infrared spectroscopy,fNIRS)通过检测脑区氧合血红蛋白、脱氧血红蛋白浓度的变化为抑郁疗效提供了代谢信息支持。Shinba T等[44]研究发现,15例MDD中多数在rTMS治疗时额叶血红蛋白浓度持续增加。与额叶血红蛋白浓度减少组相比,增加组在治疗结束时的MADRS得分明显降低。证明了rTMS引发脑皮质血流动力学变化,可能有助于更好了解rTMS治疗效果。此外,国内1项大样本(纳入MDD组和健康对照组各40人)设计研究发现,经过2 Hz、90%MT、刺激靶点为右侧DLPFC、治疗时间为4周的刺激后,MDD组右侧腹外侧前额叶氧合血红蛋白浓度变化与rTMS治疗后HAMD评分降低呈正相关[45]。再次证明运用fNIRS与rTMS协同作用为抑郁症治疗机制及疗效优化提供创新方案。

此外,静息态功能磁共振成像(resting-state functional MRI,rs-fMRI)显示,LLD的默认模式网络(default-mode network,DMN)与执行控制网络之间的功能连接减弱会导致认知灵活性和情绪调控能力下降。国内1项研究表明,rTMS组DMN内的局部一致性值的变化与HAMD评分呈负相关[46]

在治疗安全性方面,rTMS及iTBS在老年群体中耐受性较好、不良反应轻度且持续时间短暂;与药物治疗相比,不涉及药物相互作用与依从性受限等问题,适合作为药物治疗与心理干预的补充或替代选项[47]。需要强调的是,高频刺激的癫痫诱发风险相对更需关注,部分研究提示在老年患者中可优先考虑低频方案或个体化以平衡疗效与风险。

2 tDCS

tDCS通过头皮将微弱的直流电传入大脑,改变神经元的静息膜电位从而调节大脑皮质的兴奋性。刺激方式可以分为阴极刺激(又称作抑制性刺激)和阳极刺激(又称作兴奋性刺激),分别会引起神经细胞膜电位的超极化和去极化,从而起到降低以及增加局部皮质兴奋性[48]。抑郁障碍研究中最常见的布置为阳极置于左侧 DLPFC,阴极置于对侧额区(如右侧DLPFC/眶额区/乳突等)[49]。常用参数多为2 mA、20~30 min/次、每周5次、2~4周的集中疗程(具体以各研究方案为准)。

多项研究表明 tDCS 在治疗抑郁障碍方面具有一定临床疗效,尤其与抗抑郁药物联合应用时效果更为明显。1项RCT研究表明,联合治疗组(tDCS与药物同步启动)在MADRS评分的下降幅度上优于单舍曲林组(50 mg/d)、单 tDCS组和安慰刺激组[50],提示二者可能存在协同作用。有研究纳入15名平均年龄为73.27岁的LLD患者,给予2 mA、20 min/d、为期2周的tDCS干预。结果显示,HAMD评分明显下降,这种改善效果在1个月和3个月的随访中仍然维持。此外,简易精神状态检查评分也有明显提高,提示对总体认知功能也具有积极效果[51]。在另一项针对LLD合并认知功能障碍的研究中发现,tDCS对于抑郁症状的改善效果明显,对于视觉空间记忆功能也有一定程度的改善。但注意力和执行功能未见明显改善[52]。上述结果说明tDCS在老年人群的情绪改善方面较为稳定,而认知功能的效应存在领域差异性,需要更大样本与更标准化的认知测量加以验证。

与rTMS相比,tDCS的并行可操作性更强,便于与认知任务或情绪加工训练同步实施,大量研究已尝试把tDCS与认知训练或心理治疗并行,以期协同改善抑郁与认知功能[53]。在LLD的研究中,已报道的1项随机研究将tDCS刺激左侧DLPFC与团体认知训练结合,进行了5周干预:2组(真/假刺激)抑郁评分随时间均下降,组间无明显差异;但仅真刺激组在视觉空间记忆上明显提升,并伴随相关脑区功能连接改变,提示对认知功能可能具有增益而非额外“抗抑郁”效应[52]。另有1项开放标签研究显示,居家认知训练结合 tDCS在高龄轻度认知障碍(mild cognitive impairment,MCI)或缓解期MDD人群中显示可行、安全且对总体认知有改善,为LLD居家干预提供可借鉴的实施路径[54]

国内在tDCS治疗LLD领域的研究起步较晚,但发展迅速,主要聚焦于认知增强与联合干预。国内的1项RCT研究发现,将tDCS与rTMS联合应用治疗抑郁症比单独使用更有效。该研究中,联合治疗组在rTMS治疗前30~60 min进行20 min的tDCS刺激,结果显示这种“预刺激”策略能明显降低rTMS的起效阈值,促进皮层兴奋性和大脑可塑性发生更深层次、更持久的改变,实现诱导LTP效应[55]

疗效评价方面也正逐渐从单一量表向神经生理标志物转变。国内1项纳入46例LLD患者的研究发现,与健康老人相比,LLD的事件相关电位(event-related potential,ERP)N400潜伏期普遍延长且波幅有所减小,显示LLD患者存在和语言有关的认知功能障碍[56]。提示ERP可作为tDCS疗效的生物标记物。1项在认知任务态下的功能性磁共振成像(functional magnetic resonance imaging,fMRI)研究中,发现tDCS干预后LLD患者在执行N-back任务时,前额叶皮层的激活效率明显提升(即以更少的脑区激活完成同等难度的任务),这揭示了tDCS在改善执行功能方面的独特优势[57]。同时,有研究发现,高精度经颅直流电刺激(high-definition transcranial direct current stimulation,HD-tDCS)可在MDD中诱导靶向性灰质增加,并引起默认模式网络的功能连接增强[58]。这些证据表明,tDCS不仅能改善情绪主观体验,更引起了脑网络层面的神经可塑性改变。

在治疗安全性方面,tDCS的不良反应主要为瘙痒、头痛、刺痛等,其症状较轻且持续时间较短[59],总体耐受性良好。其设备便携、成本较低,便于开展维持治疗以及远程督导的居家治疗,尤其适合合并多种躯体疾病及多重用药的LLD患者。但目前针对 LLD的临床研究数量仍有限,与认知改善之间的因果链与作用机制尚未明晰;未来需在样本量、参数标准化、长期随访与客观认知结局等方面进一步加强研究。

3 tACS

tACS以低强度、特定频率的交流电通过头皮电极耦合脑内振荡,主要通过神经夹带与网络耦合(如跨区相位一致性、跨频耦合)影响皮层—边缘系统环路可塑性[60],从而调节情绪与认知相关网络的活动[61]。与tDCS只能通过电极极性调节兴奋性不同,tACS可以实现频率选择与网络节律对齐。

近年来的研究发现,tACS治疗抑郁障碍具有一定效果,但最佳参数尚无共识[62-64]。多项小样本RCT以10 Hz tACS 靶向刺激额叶,报告了目标节律(α波)被有效调控,表现为左侧额叶α功率与功能连接改变,并与抑郁减轻相关[65-66]。而国内学者进行了1项较大样本(纳入干预组和安慰剂组各50人)的双盲RCT,针对首发、未用药MDD患者,使用77.5 Hz、15 mA刺激前额叶,发现tACS组在抑郁严重度评分上明显优于假刺激组;随访期间缓解率与应答率也明显更高,疗效持续至第8周[67]。过往tACS都采用低强度(2 mA)参数,而该研究证实了通过头皮向深部脑区(如脑干、小脑)传递足够电流强度的可行性。同时也为LLD治疗提供了新思路:老年人由于脑萎缩和颅骨增厚,可能更需要此类高强度刺激方案。该团队也通过进一步神经影像研究证实77.5 Hz/15 mA可增强大脑皮层、皮层下区域以及小脑、脑干的大范围激活,为临床效应提供机制线索[68]

但是截至目前,直接针对LLD的tACS干预仍非常匮乏。1项Meta分析表明,tACS可能增强认知功能(涵盖所有认知领域,包括视觉注意力、工作记忆、执行控制等),并对老年人的认知衰退有一定的缓解效果[69]。有研究在痴呆人群使用40 Hz tACS联合认知训练观察到认知与情绪(含抑郁评分)改善[70],提示tACS对老年抑郁的治疗潜力。但认知障碍人群与LLD并不等同,该结果不能直接外推到LLD,未来还需要开展以LLD为对象的RCT研究。

在治疗安全性方面,因tACS是通过调整电流强度和电极节律从而使内源性神经振荡逐渐与外源刺激同步,故其不良反应少、强度更小[71],一般为头痛、短暂耳鸣等,LLD患者总体耐受性良好,并无皮肤烧伤或认知恶化等情况出现[72],并且tACS还能兼容心脏起搏器,对于共病心血管疾病的LLD患者经评估后可能适用。在治疗费用方面,tACS与tDCS用同一设备实现,费用都远低于rTMS设备,且多为家用便携设备,可以避免LLD患者频繁就医以及大幅度降低长期治疗的经济负担。

4 新兴探索性技术及其在LLD应用中的挑战

常见的NIBS还有tRNS和tFUS等技术。其中,tRNS是通过在大脑头皮施加随机频率的交流电流,以调控大脑皮层神经元活动的一种经颅电刺激技术。根据刺激频率的不同,可将tRNS分为全频(0.1~640 Hz)、低频(0.1~100 Hz)和高频(101~640 Hz)[73]。其中,高频tRNS更可能诱发神经元去极化与兴奋性增加,而低频tRNS则抑制神经元兴奋性[74]。其作用机制与rTMS、tDCS和tACS相似,均可通过改变皮质兴奋性、促进突触可塑性和调节神经递质功能来实现抗抑郁效应。

现有研究提示tRNS可能具有一定抗抑郁潜力[75]。在1项RCT研究中,将66例MDD或双相情感障碍患者随机分配到tRNS组和安慰剂组。tRNS组接受100~640 Hz、直流电2 mA、叠加偏置电流2 mA的刺激方案,疗程20次/4周。结果显示,2组的抑郁症状均有明显改善,但组间没有明显差异,提示其独立疗效尚需进一步验证[76]。另一项研究发现,tRNS干预并未明显提升正性情绪,但在老年人组(≥60岁)中特质消极情绪水平越高,干预后情绪改善越明显[77]。这提示tRNS可能通过调节高消极特质相关的异常神经活动,成为LLD情绪症状干预的一个潜在方向。然而,目前国内尚未有关于tRNS治疗LLD的报道。老年脑萎缩导致的皮层距离增加,高频噪声信号在老化大脑中的衰减模型尚不明确。现有证据多为小样本病例报告,缺乏针对老年脑网络特征的参数优化方案,因此尚不能作为常规临床推荐。

除tRNS外,tFUS作为新兴NIBS技术,利用低强度聚焦超声波穿透颅骨,能够实现更高的空间分辨率和刺激深度,作用范围可达皮层下结构。研究证实,tFUS在动物实验与健康人中的初步结果显示可调控情绪与认知相关环路[78-79]。1项案例报告(男,46岁,患有MDD)结果表明,经tFUS刺激后HAMD评分下降[80],同时有研究发现,tFUS组经过3周治疗后抑郁症状缓解率达35%,反刍性消极思维明显减少(开放研究,需RCT证实)[81]。但现有针对MDD或LLD的临床证据仍极其有限,多数停留在安全性与可行性验证阶段[82]

综上,虽然tRNS和tFUS在机制上具有创新性,但应用于LLD治疗主要存在以下缺口:①样本量不足,缺乏统计效力足够的双盲RCT研究;②参数未标准化,频率、强度及靶点选择缺乏统一标准;③安全性数据缺失,缺乏针对LLD的长期安全性追踪。因此,目前这2项技术更多应定位为科研探索,而非临床常规治疗。

5 小结与展望

综上所述,NIBS目前已在治疗抑郁症领域有了广泛的应用,同时也为降低 LLD的复发率和改善认知功能提供了新的治疗思路。不同的NIBS技术对于治疗LLD在参数、疗效及局限性方面存在不同(表1)。rTMS、tDCS、tACS等技术在短期疗效和安全性方面已有较一致的结论,具有较广阔的应用前景,但仍存在一定不足。①缺少长期随访。现有研究多集中在短期疗程结束后的症状缓解,而对LLD患者的长期疗效追踪与复发风险预防仍缺乏系统数据。考虑到老年患者复发率高、病程迁延的特点,未来应加强多时间点、长随访的纵向研究。②治疗参数未标准化。目前除tDCS使用一个较为固定的参数进行治疗外,其余技术的刺激参数(频率、强度、脉冲数、疗程等)差异较大,不利于结果对比与推广应用。未来需在大样本RCT基础上探索并建立标准化参数体系,并结合LLD人群的异质性(如首发与复发、轻度与重度)形成个体化治疗方案。③疗效评估手段有限。目前多依赖量表判断抑郁症状及认知功能是否改善,但考虑到老年人文化程度、理解能力等差异较大,可能导致结果偏倚。仅有少数研究采用了客观神经影像和神经电生理指标进行疗效评价,与量表相比,这些神经指标能更敏锐地捕捉到大脑节律的细微变化,且不受老年人语言表达能力和自知力缺失的影响,具有更客观的诊断与评价价值。但因神经影像设备成本较高,且存在一定限制条件(如体内不能植入金属,老年人大多有种植假牙等情况),故存在一定局限性。因此,未来研究应考虑在更多引入客观生理指标的同时,结合量表判断以实现更精准、可重复的疗效评估。④联合干预模式不足。现有证据多为NIBS与药物治疗的结合,但在药物耐受性差、依从性低的老年群体中,仍需探索NIBS与心理治疗、认知训练或其他康复措施的联合应用,以期实现多靶点协同的治疗效果。

此外,尽管NIBS技术前景广泛,但在我国临床推广仍存在以下现实阻碍。①设备成本高、医保覆盖不足。目前所采用的进口设备价格高昂,且仅部分省市将rTMS纳入医保报销范畴,tDCS等多为自费项目,加重了老年患者的经济负担。未来可鼓励产学研合作,加速国产设备的研发与临床验证,降低治疗成本,同时为NIBS治疗纳入医保提供保障。②基层培训缺失。社区及基层医院缺乏具备操作资质的专业人员,导致出现“有设备、无人用”等问题。考虑开展基层医生实操培训,构建“专家提供技术支持—社区医院实施治疗—家庭远程监控”的闭环,解决基层技术力量薄弱的问题。③本土化参数缺失。目前临床多直接沿用欧美国家治疗参数,并未充分考虑中国老年人实际头围结构和异质性。同时,未来应研究建立多中心本土化参数数据库,并探索NIBS与中医针灸等传统康复手段的联合应用,已有研究提示rTMS联合头皮针治疗可能产生协同增效作用[83]。这种具有中国特色的综合治疗方案也更容易被老年患者接受。

值得注意的是,随着网络神经科学和人工智能的发展,NIBS的应用正逐步走向精准化与个体化。通过连接组学方法与机器学习建模,可以基于脑网络特征预测患者对不同刺激参数的反应,从而实现靶点的个体化定位与疗效优化。同时,跨疾病的证据也为LLD干预提供了参考,如NIBS在阿尔茨海默病、帕金森病等老年神经疾病中的应用已显示出改善认知与运动功能的潜力,提示其在合并症干预中的拓展空间。此外,从公共卫生角度来看,LLD患者群体庞大且复发率高,若能推动便携化、可居家化的NIBS技术落地,将有助于缓解老龄化社会中的抑郁负担,并提升整体心理健康水平。

总结而言,未来研究应聚焦于长期随访、参数标准化、客观疗效指标及多模态联合应用,并积极引入跨学科技术手段,推动NIBS在LLD领域的规范化、精准化与可推广性发展,以期真正提升老年抑郁患者的治疗水平和生活质量。
