慢性病患者社会融合的概念演进、评估工具与干预策略研究进展

Research progress on concept evolution,assessment tools and intervention strategies of social integration in patients with chronic diseases

摘要

随着慢性病发病率的持续攀升及患者生存期的显著延长,慢性病患者的社会融合问题日益成为公共卫生与临床相关领域关注的重点。本文对慢性病患者社会融合的概念、评估工具、影响因素及干预措施进行综述,旨在为开发本土化慢性病患者社会融合评估工具、制定多学科协作干预方案以及提升慢性病患者融合水平和生活质量提供参考。

Abstract

With the continuous increase in the incidence of chronic diseases and the significant extension of patients′ survival periods,social integration among patients with chronic diseases increasingly become a focus of attention in public health and clinical-related fields. This article reviews the concept,assessment tools,influencing factors,and intervention strategies of social integration for patients with chronic diseases,aiming to provide references for developing localized social integration assessment tools for patients with chronic diseases,formulating interdisciplinary collaborative intervention plans,and improving the integration level and quality of life of patients with chronic diseases.
