冷冻疗法联合当归四逆汤对白蛋白结合型紫杉醇化疗诱导的周围神经病变的疗效观察

冷冻疗法联合当归四逆汤对白蛋白结合型紫杉醇化疗诱导的周围神经病变的疗效观察
Observation of therapeutic effect of cryotherapy combined with Danggui Sini decoction on peripheral nerve toxicity induced by albumin-bound paclitaxel
目的 观察冷冻疗法联合当归四逆汤对白蛋白结合型紫杉醇化疗诱导的周围神经病变的临床疗效及安全性。 方法 将赣南医科大学第一附属医院2022年9月—2023年12月期间收治的60例晚期非小细胞肺鳞癌患者,随机分为对照组和试验组,对照组进行白蛋白结合型紫杉醇联合化疗,试验组在对照组基础上采用冷冻疗法联合当归四逆汤治疗。统计分析2组患者化疗期间周围神经病变发生率及分级情况、欧洲癌症研究与治疗组织化疗致周围神经病变生活质量问卷(European organization of research and treatment of cancer QLQ-CIPN20,EORTC QLQ-CIPN20)评分、化学疗法诱导的周围神经病的临床评估量表(Clinical assessment scale for chemotherapy-induced peripheral neuropathy,CAS-CIPN)评分、Karnofsky功能状态(Karnofsky performance status,KPS)评分以及药物不良反应。 结果 试验组化疗2、4、6个周期后的周围神经病变发生率及分级低于对照组(P<0.05);试验组化疗2、4、6个周期后的EORTC QLQ-CIPN20评分均低于对照组(P<0.05);试验组化疗4、6个周期后的CAS-CIPN评分均低于对照组(P<0.05);试验组化疗2、4、6个周期后的KPS评分均高于对照组(P<0.05);2组患者骨髓抑制发生率比较差异无统计学意义(P>0.05);2组患者均无明显肝肾功能异常情况。 结论 冷冻疗法联合当归四逆汤能够有效降低白蛋白结合型紫杉醇化疗后的周围神经病变的发生率和严重程度,提高患者治疗依从性,改善生活质量,且安全可靠。
Objective : To observe the clinical effect and safety of cryotherapy combined with Danggui Sini decoction on the peripheral nerve toxicity caused by albumin-bound paclitaxel chemotherapy. Methods Sixty patients with advanced non-small cell lung squamous cell carcinoma from September 2022 to December 2023 were randomly divided into control group and experimental group. The control group received albumin-bound paclitaxel combined chemotherapy, and the experimental group received cryotherapy plus Danggui Sini decoction on the basis of chemotherapy. The incidence and grading of peripheral nerve toxicity, European organization of research and treatment of cancer QLQ-CIPN20 (EORTC QLQ-CIPN20), Clinical assessment scale for chemotherapy-induced peripheral neuropathy(CAS-CIPN), Karnofsky performance status(KPS), myelosuppression, liver and kidney function were observed and analyzed between the two groups during chemotherapy. Results The incidence and grade of peripheral nerve toxicity in the experimental group after 2, 4 and 6 cycles of chemotherapy were significantly lower than those in the control group (P<0.05). The EORTC QLQ-CIPN20 scores in the experimental group after 2, 4 and 6 cycles of chemotherapy were significantly lower than those in the control group (P<0.05). The CAS-CIPN scores of the experimental group were significantly lower than those of the control group after 2, 4 and 6 cycles of chemotherapy (P<0.05). The KPS scores of the experimental group aafter 2, 4 and 6 cycles of chemotherapy were higher than those of the control group (P<0.05). The incidence of myelosuppression was similar between the two groups, with no statistical significance (P>0.05). Both groups of patients had no significant abnormalities in liver and kidney function. Conclusion Cryotherapy combined with Danggui Sini decoction can effectively reduce the incidence and severity of peripheral nerve toxicity after albumin-bound paclitaxel chemotherapy, improve patients' quality of life, improve treatment compliance, and is safe and reliable.
