代谢相关脂肪性肝病发病的影响因素分析
Analysis of influencing factors on the onset of in metabolic dysfunction-associated steatotic liver disease
目的 探究代谢相关脂肪性肝病(Metabolic dysfunction-associated fatty liver disease,MAFLD)发病的影响因素。 方法 选取2020年1月至2023年6月在我院体检的上饶市居民200例,使用自行设计的流行病学调查表调查包括生活习惯、慢性疾病情况。收集研究组实验室检查结果、超声检查结果及幽门螺杆菌(Helicobacter pylori,Hp)感染检测结果等临床资料。根据腹部B超检查结果分为MAFLD组(n=75)和健康体检组(n=125),比较2组的一般资料,并以是否患有MAFLD为因变量,将单因素分析中有统计学差异的变量作为自变量,进行多因素Logistic回归分析。 结果 ⑴MAFLD组合并高血压、合并糖尿病、Hp感染的患者均多于健康体检组,年龄、平均血压(Mean blood pressure,MBP)、体重、体质量指数(Body mass index,BMI)、腰围、空腹血糖(Fasting blood glucose,FBG)、总胆固醇(Total cholesterol,TC)、甘油三酯(Triglyceride,TG)、高密度脂蛋白胆固醇(High-density lipoprotein cholesterol,HDL-C)、天门冬氨酸氨基转移酶(Aspartate transferase,AST)、丙氨酸氨基转移酶(Alanine amino-transferase,ALT)和谷氨酰转肽酶(Gamma-glutamyl transferase,GGT)均高于健康体检组(P<0.05)。MAFLD组低密度脂蛋白胆固醇(Low-density lipoprotein cholesterol,LDL-C)水平低于健康体检组(P<0.05)。多因素Logistic回归分析,结果显示,BMI、FBG、TC、TG、HDL-C、LDL-C、Hp感染为MAFLD的危险因素(P<0.05)。 结论 BMI、FBG、TC、TG、HDL-C、LDL-C、Hp感染为MAFLD的危险因素。
Objective To explore the influencing factors of the onset of metabolic dysfunction-associated fatty liver disease (MAFLD). Methods Two hundred residents of Shangrao City who underwent physical examinations in our hospital from January 2020 to June 2023 were selected. A self-designed epidemiological questionnaire was used to investigate the conditions including living habits and chronic diseases. The clinical data such as the laboratory test results, ultrasound examination results and Helicobacter pylori (Hp) infection detection results of the research group were collected. According to the results of abdominal B-ultrasound examination, the patients were divided into the MAFLD group (n=75) and the healthy physical examination group (n=125). The general data of the two groups were compared. Taking whether MAFLD was present as the dependent variable and the variables with statistical differences in the univariate analysis as the independent variables, multivariate Logistic regression analysis was conducted. Results ⑴ The number of patients in the MAFLD group with hypertension, diabetes, and Hp infection was all higher than that in the healthy physical examination group. Age, mean blood pressure (MBP), body weight, and body mass index BMI, waist circumference, fasting blood glucose (FBG), total cholesterol (TC), triglyceride (TG), high-density lipoprotein cholesterol (HDL-C), aspartate transferase (AST), alanine amino-transferase (ALT) and gamma-glutamyl transferase (GGT) were higher than those in the healthy physical examination group (P<0.05). The level of low-density lipoprotein cholesterol (LDL-C) in the MAFLD group was lower than that in the healthy physical examination group (P<0.05). Multivariate Logistic regression analysis was made and the results showed that BMI, FBG, TC, TG, HDL-C, LDL-C and Hp infection were risk factors for MAFLD (P<0.05). Conclusion BMI, FBG, TC, TG, HDL-C, LDL-C and Hp infection are risk factors for MAFLD.
