赣州市某高校一起结核病聚集性疫情调查分析

赣州市某高校一起结核病聚集性疫情调查分析
Investigation and analysis of a tuberculosis cluster epidemic in a university in Ganzhou City
目的 对2023年4—5月发生在赣州市某高校一起结核病聚集性疫情调查处置情况进行分析,为今后处置类似疫情提供依据。 方法 对疫情发生发展经过和处置措施、病例流行病学调查情况、密切接触者和一般接触者的筛查结果等进行描述和统计分析。 结果 共发现3例学校肺结核病例,分布在同一班级同一宿舍;密切接触者中,病例所在班重组结核杆菌融合蛋白皮肤试验阳性/强阳性率22.86%(8/35),其他班1.79%(1/56),两者比较差异有统计学意义(χ2=8.497,P=0.004);密切接触者中重组结核杆菌融合蛋白皮肤试验阳性/强阳性率9.78%(9/92),一般接触者中重组结核杆菌融合蛋白皮肤试验阳性/强阳性率1.72%(1/58),密切接触者是一般接触者的5.69倍,但差异无统计学意义(χ2=2.530,P=0.112)。 结论 指示病例长时间带病在学校学习和生活,是聚集性疫情发生的主要原因。要加强对师生的健康教育,提高师生对结核病的认知水平和防控意识;学校要落实因病缺勤病因追查工作,及早发现肺结核可疑病例。
Objective : To analyze the investigation and handling of a tuberculosis cluster outbreak in a university in Ganzhou from April to May 2023, so as to provide a basis for dealing with similar epidemics in the future. Methods We described and statistically analyzed the occurrence and development of the epidemic and the measures to deal with it, the epidemiological investigation of the case, the screening results of close contacts and general contacts. Results : A total of 3 cases of tuberculosis were detected. They were distributed in the same class and dormitory. In the screening of close contacts, the EC positive/strong positive rate of the case in the class was 22.86% (8/35) and that of other classes was 1.79% (1/56), and the difference between the two was statistically significant (χ2=8.497,P=0.004). The EC positive/strong positive rate among close contacts was 9.78% (9/92), and the EC positive/strong positive rate among general contacts was 1.72% (1/58), which was 5.69 times that of general contacts, but the difference was not statistically significant (χ2=2.530,P=0.112). Conclusion The first case was studying and living in school for a long time, which was the main reason for the occurrence of cluster outbreaks. It is necessary to strengthen the health education of teachers and students, and improve the awareness of tuberculosis prevention and control among teachers and students; the university should implement the investigation of the causes of absence due to illness and detect suspected cases of tuberculosis as early as possible.
