摘要

网络游戏成瘾(IGD)深刻影响青少年身心发展,当代技术革新为干预提供了新范式,数字化干预作为一种新兴治疗方式受到广泛关注,但在网络游戏成瘾上的应用有待进一步研究.基于此,梳理网络游戏成瘾的发展过程、传统及数字干预方法,并基于个体游戏行为的过程演化规律,提出青少年网络游戏成瘾的全过程数字化干预策略,并对未来IGD研究进行展望,以期为青少年IGD的干预提供更具生态效度的建议与参考.

Abstract

Internet gaming disorder (IGD) profoundly affects the psychological and physical development of adolescents. As an emerging treatment approach, digital interventions have attracted increasing attention. Although recent technological innovations have provided new paradigms for intervention, their application to IGD remains insufficiently explored. To address this gap, the present study reviews the developmental trajectory of IGD, both traditional and digital intervention approaches. Based on the processual characteristics of adolescents’ gaming behavior, we propose a full-process digital intervention framework for IGD and outline future research directions. This review aims to provide ecologically valid guidance and reference for improving intervention strategies targeting adolescent IGD.

0 引言

在全球范围内,网络游戏成瘾(internet gaming disorder,IGD)已引起各界关注1.2018年《国际疾病分类第11版》(international classification of diseases 11th revision,ICD-11)正式将游戏障碍纳入,将其归类为“因成瘾行为引起的障碍”,并置于“精神、行为或神经发育障碍”类别下2.IGD已成为严峻的公共卫生问题3.在一项涉及29个国家和地区的227 665名参与者的元分析研究中,IGD男性患病率为8.5%,女性为3.5%4.青少年心理发展尚未成熟,自我控制能力、情绪调节和奖励系统等较弱5-6,容易受到游戏的强烈吸引和负面影响,是IGD的高危易感人群7.IGD不仅增加青少年个体干眼症8、肥胖症9等生理疾病的发生风险,同时引发学业压力10、孤独和抑郁11等多重心理问题.与此同时,同伴压力和社会环境等因素12会干扰个体的执行控制功能,进一步导致成瘾性网络游戏的恶性循环13.在这样一个移动互联网和智能设备高度普及、网络游戏设计花样繁多的数字时代,网络游戏的可达性和吸引力不断增强,进一步加剧了这一问题.在此背景下,探索有效的干预与治疗策略变得尤为重要.

虽然传统的IGD干预和治疗已在一定程度上取得了一些积极成果13,但是在实践过程中逐渐暴露出显著的局限性:一方面,线下干预模式难以满足青少年作为数字原住民对匿名性、即时性的需求,青少年患者及其监护人往往因此拒绝持续参与;另一方面,现有研究多将技术视为中立工具,忽视了游戏算法设计、社交反馈机制等技术特性本身对成瘾行为的诱发作用,导致干预效果不佳.尽管传统治疗仍是当前IGD干预体系的核心组成部分,但传统治疗聚焦于成瘾治疗,缺乏从预防到康复的全过程思维,且在可及性、实时性和情境适配性方面受到线下情境限制,难以覆盖青少年在日常生活中反复出现的高风险使用情境.在此意义上,数字化干预并非要取代传统治疗,而是在其基础上提供更具连续性、情境敏感性和可扩展性的补充路径.

在此背景下,本研究试图回答以下核心问题:青少年IGD受哪些关键因素影响并如何发展?传统干预模式的局限何在?数字化干预如何应用于预防、治疗与康复的全过程?据此,本文依次梳理IGD的发展过程、传统治疗的特点,提出全过程数字化干预策略,并对未来IGD研究进行展望,以期为青少年IGD的干预提供更具生态效度的建议与参考.

1 IGD的发展过程

IGD的发展是一个由功能性使用逐渐演化为失控性依赖的连续性过程.根据Brand等14-15提出的I-PACE模型(interaction of person-affect-cognition-execution model),该过程并非由单一因素驱动,而是由易感因素、情绪与认知反应、执行控制系统以及行为强化机制等多层因素在时间维度上相互作用而形成的动态系统.I-PACE模型指出,成瘾的发展体现为从早期以情绪调节与认知期待为核心的模式,逐步过渡到中期的强化学习与习惯化行为,最终在执行控制受损的背景下形成渴求主导、冲动强化与行为依赖的病理性状态,从而表现出典型的IGD发展过程.

在发展早期,IGD的形成受到个体已有易感性因素的影响,包括气质特征(如高冲动性、奖赏敏感性)16、情绪调节困难17、孤独、焦虑及抑郁等1118.易感性因素在青少年群体尤为突出:个体在这一阶段普遍存在执行控制系统尚未成熟、奖赏敏感性较高、情绪调节能力弱的特点19,同时又面临学业压力、人际不确定性与自我认同发展的挑战1020,使其更易通过外部情绪调节方式寻求稳定感.这些特质并不会直接导致IGD,但会使青少年在面对具有高奖赏特性的游戏情境时,产生更强烈的情绪激活和注意偏向,使其更有可能将游戏作为缓解压力、调节情绪或获得成就感的途径14-15.在此阶段,情绪反应与情境性认知加工开始在大脑奖赏系统中形成可强化的联结21.

在发展中期,青少年个体面临负性情绪时,更倾向于自动启动游戏相关的期待与行为反应21.随着游戏使用频率增加,青少年的情绪调节与认知加工系统逐步形成对游戏的偏向性依赖,个体的情绪和认知反应逐渐自动化并具有可预测性14-15.情绪反应系统表现为对游戏的需求显著增强,而现实生活中的负性情绪难以通过其他方式得到有效修复,游戏逐渐成为主要的应对方式22-23,渴求感成为推动游戏行为的重要内部动力24.与此同时,在认知加工系统中,注意偏向与线索反应性逐步增强,青少年发展出稳定的功能性偏差,对游戏相关线索的反应速度和敏感度显著提升24.在强化作用下(如短期的情绪缓解、胜利带来的情绪体验),这些反应通过情绪调节策略、游戏的相关认知偏差及应对方式的不断相互作用得到巩固,从而使行为更具自动化和习惯化特征14-15.

在发展后期,青少年个体的情绪调节能力、认知能力与执行控制能力均呈现高度偏离,表现出明显的病理化特征13-15.此时3个可变系统失衡并形成恶性循环:情绪反应系统高度依赖游戏维持情绪稳定,一旦减少或停止使用便出现焦虑、抑郁等情绪反应25;认知加工系统中产生悲观与无助感24,青少年常认为自己“无法控制自己”“离开游戏就毫无价值”;此外,青少年的执行控制系统的脆弱性尤为突出,由于前额叶功能尚未完全成熟,其冲动抑制与自我调节能力难以抵抗反复强化后的游戏线索的诱惑,导致复发成为常态19.此阶段持续的游戏行为不仅损害学业、人际与睡眠等关键发展领域,还可能干扰其核心发展任务的完成,如自我认同形成与自主性发展等,从而进一步放大负性影响,推动IGD恶性循环.

2 IGD的常规治疗

Young等26首次将IGD作为一种心理障碍进行干预和治疗,自此针对IGD的常规治疗开始初现成果.目前的临床实践以个体为中心展开治疗,主要采用心理咨询、物理治疗、药物治疗以及联合治疗.

2.1 心理咨询

青少年的认知发展可塑性强,认知行为疗法(cognitive behavioral therapy,CBT)强调重构个体认知策略,减少个体对消极事物的动机驱力,是治疗青少年IGD的主要方法之一27.CBT可以帮助患者提高抑制控制能力、识别不良的认知,并采取更多具有适应性的决策28.有研究发现,接受CBT治疗的小组报告的每周游戏时间和IGD症状明显少于无干预的小组29.也有研究表明了群体CBT治疗和支持性心理咨询2种治疗的IGD症状都明显减轻,且无组间差异30.即便是基于小组的CBT干预,其疗效在6个月的随访中也保持治疗增益31.此外,选择最佳的干预方法是CBT的另一大特点,CBT可以针对患者的特征进行个性化调整,通过对患者不同的成瘾原因及程度设计个性化治疗方案,结果取得显著成效32.此外,成瘾行为被认为是无念行为,患者通常寻求短期快乐,而减少对于长期结果的追求33;而正念干预(mindfulness intervention)通过加强自我调节、抑制控制和注意力集中等可减少成瘾中常见的习惯性思维34.有许多研究证明了正念显著提高青少年的即时和长期自我控制能力35,对IGD具有较好的治疗效果36.

此外,青少年行为问题往往反映着家庭问题,因此家庭治疗也是干预IGD的高效手段之一37.家庭干预的目标主要集中在家庭功能和凝聚力的恢复上,将治疗重心放在构建良性的亲子沟通38、鼓励家庭成员参与更多与游戏无关的新活动,改善成瘾现状39.有研究指出,家庭特征以及家庭失效沟通是导致个体IGD的家庭风险因素,建构家庭治疗效应网络,进一步验证了家庭治疗在改善IGD中的积极作用40-41.

2.2 物理疗法

目前针对IGD的物理疗法,主要集中在电针(electro-acupuncture,EA)、重复经颅磁刺激(repetitive transcranial magnetic stimulation,rTMS)、经颅直流电刺激(transcranial direct current stimulation,tDCS).这些方法通过调节大脑的神经活动或改善身体的生理状态,帮助患者减少游戏成瘾行为.EA因其副作用较小而广受欢迎,通过刺激网络成瘾患者的穴位,可以有效改善睡眠质量和冲动特质42;rTMS有着定位精确且疗效持久的优势,可以通过高频rTMS刺激患者左侧背外侧前额叶皮层,有效改善IGD患者执行控制能力43;tDCS作为非侵入性神经调节技术,近年来因其易操作且费用低的特点备受欢迎,tDCS能够直接作用于异常神经回路,因而开始被用于治疗成瘾行为44.有研究证实了tDCS在改善IGD患者抑制控制功能的积极作用,且双侧tDCS优于右侧背外侧前额叶皮层45-46.

2.3 药物治疗

目前,虽然针对IGD的药物治疗研究还处在初步阶段,但在处理一些共病问题时,药物治疗也表现出较好的效果.如通过药物改善IGD及相关合并症(如多动症、抑郁症)的临床表现.在一项元分析研究中发现,大多数的IGD患者都患有其他精神病,只有24%的样本仅患有IGD47.安非他酮是治疗IGD常用的药物,作为一种去甲肾上腺素和多巴胺再摄取抑制剂,安非他酮通过调节大脑多巴胺释放,从而减少欲望及戒断症状,也常用来治疗抑郁、焦虑和多动48.尽管也有研究探索了纳曲酮在行为成瘾治疗中的积极作用49,但是针对IGD的药物治疗仍有待进一步研究.

2.4 联合治疗

单一治疗方法存在明显弊端,通过将心理、药物和物理疗法结合在一起进行治疗,可以更大程度提高治疗效果,减少复发率,并改善患者的整体功能.如一项临床试验通过使用EA刺激相应穴位并结合CBT的方法证明,联合治疗相比于单一疗法疗效更好50.也有系统综述和元分析研究表明,联合疗法比药物治疗或心理治疗在减少IGD症状更有效51.而心理咨询的联合治疗主要强调认知重构的作用52,有研究发现CBT和正念的组合是最有效缓解IGD的干预措施53;还有研究表明现实疗法和正念结合可减少IGD患者的跨期决策冲动36.

综上,如表1所示,传统治疗方法虽在改善认知偏差、情绪调节和执行控制等方面具有一定疗效,但整体仍受限于线下场景、人力投入和干预即时性等问题.

无论是心理、物理还是药物手段,其多针对已经被诊断为IGD的患者,且多在固定场所进行干预,存在生态效度不足与可持续性有限等局限;联合治疗虽可提升疗效,却难以在实践中广泛落实.因此,传统方法难以构建覆盖IGD发展全过程的系统性干预框架,这为数字化技术介入提供了必要性与发展方向.

3 数字化干预方法

生态瞬时干预(ecological momentary intervention,EMI)是指针对生态瞬时评估(ecological momentary assessment,EMA)量身定制的干预措施,即基于移动端在自然生活环境中反复采集个体的情绪、行为和认知体验,提供即时且个性化的反馈和干预2454.近年来,EMI逐渐与数字化传感器采集(如心率变异性、步数、睡眠节律、面部表情识别等)结合,主动或被动收集数字行为特征,显著提高了个体自我健康管理的效果55.此外,目前也有研究通过自然语言处理和机器学习技术来分析社交媒体的文本信息预测个体的成瘾行为56.

数字游戏干预(gaming intervention)强调通过特殊设计的功能性游戏达到情绪调节、自我控制训练或现实参与强化目标,而非仅提供娱乐体验.例如,体感游戏通过运动参与提升身体活动水平,可以减少游戏成瘾并保护用户心理健康57.也有研究表明,在课堂环境中使用定制数字游戏能够有效提升儿童的专注力、执行和控制功能58.还有研究使用运动类游戏,改善情绪状况,治疗游戏过度沉浸或成瘾59.此外,游戏行业规范和游戏设计本身也能够有效改善青少年过度游戏行为,避免青少年IGD发展.例如,目前政策法规和大部分行业规范都强调对互联网平台的构建和未成年人网络游戏等环节进行监管,以减轻青少年IGD症状和控制上网时间,并已取得积极效果60.有些终端中提供处理IGD的工具和知识等以帮助青少年建立理性认识,有效预防和干预青少年IGD61.

虚拟现实治疗(virtual reality therapy,VRT)是构建高度可控的场景,模拟诱发情境,通过反复暴露、应对策略练习和认知重建减少冲动行为的治疗方法62.VRT通过放松、高风险情境模拟和声音辅助认知,改善了皮质-边缘系统回路的功能连接性,减轻IGD症状,效果与CBT相当63.增强现实(augmented reality,AR)指的是数字媒体在现实世界的呈现技术,将数字内容与现实内容整合,通过技术手段在现实世界增加信息层64.基于AR的干预通过即时提示、注意引导或风险预警帮助用户在真实生活场景中练习情绪与行为控制64.有研究基于AR构建了可持续环境,传递IGD相关的信息,如IGD冲突、戒断和复发的含义和特点,提高对电子游戏成瘾危害的认知65.此外,Pokemon Go(一款基于AR的网络游戏,需要在现实生活中行走以完成游戏任务)等AR混合身体活动的游戏也可以提升青少年现实社交与环境参与,有利于心理健康66.

数字自助干预(digital self-help interventions)是指无需治疗师实时参与的数字化自助干预,如在线接纳承诺疗法(electronic acceptance and commitment therapy,e-ACT)、正念自助干预以及智能移动健康应用.e-ACT可以有效减轻游戏障碍和降低每周游戏频率,同时增强心理灵活性67.正念自助干预在改善认知和情绪方面有着积极作用,改善成瘾患者的控制功能68.移动健康应用融合了多种数字方法,能够评估成瘾水平,追踪情绪和睡眠状况,提供IGD知识库,有助于个体管理情绪,改善游戏行为、提升数字素养和认知水平69.互动式自助网站或对话式人工智能的推广应用可以帮助个体改善网络成瘾或焦虑抑郁情绪70-71.

远程心理服务(telepsychology services)则利用视频问诊、结构化在线会谈与远程随访提供与传统治疗质量近似的服务72.对于因社交回避、情绪问题或家庭资源不足难以进入线下服务的IGD患者,这类手段提升了心理治疗可及性.例如,一项通过摄像头对IGD患者进行远程心理治疗的研究表明,预后患者成瘾症状显著减轻,自我效能感显著提升,表现出更健康的网络行为73.有针对的远程干预程序也可以减轻IGD症状并改善青少年心理健康状况74.

综上,数字化干预正通过多技术协同为青少年IGD治疗提供更高效的支持.EMI依托实时数据提升情境化调节效果,数字游戏干预以功能性设计促进情绪与行为管理,VR/AR为冲动控制与认知重建提供高生态效度的练习场景,数字自助干预与移动健康应用实现低门槛的持续支持,远程心理服务弥补了线下资源不足.整体而言,这些技术在可及性、实时性与个性化方面具有明显优势,为IGD的预防、治疗与康复提供了更灵活、可扩展的干预途径.

4 IGD的数字化干预策略

IGD干预面临着干预维度割裂和生态效度不足的双重困境.一方面,青少年IGD干预多以个体为中心,依赖传统治疗以矫正青少年个体的认知偏差或神经递质失衡.另一方面,青少年IGD的传统干预多在实验室或治疗室进行,生态效度略显不足.目前,基于数字技术的干预被认为是最具有潜力的IGD干预途径75.数字化手段在实时监测、多模态数据整合、情境化反馈以及远程干预方面展现出显著优势,为构建基于病程演进的全过程干预体系提供了可能.因此,本研究以IGD的发展过程为线索,提出由早期、中期和后期3个阶段组成的数字化全过程干预框架.

在早期阶段,干预对象主要是已表现出高频游戏使用或情绪调节困难、但尚未达到诊断标准的易感个体.此阶段的干预目标在于尽早识别风险、延缓成瘾进程,并纠正游戏认知.干预措施应立足日常生活,通过校园筛查、线上评估与EMA持续监测心理与行为特征.在策略上,可利用游戏化训练、健康管理应用和数字自助干预加强情绪调节与理性认知.此外,VR/AR可分别通过模拟诱发情境或叠加即时提示,帮助青少年在可控或真实场景中练习自我控制,促进技能向日常情境迁移.若干预得当,青少年的游戏依赖将得到抑制,现实功能与支持系统也随之改善.

在中期阶段,干预对象通常已出现IGD症状,或达到诊断标准,其情绪调节、认知加工与执行控制系统均出现不同程度的偏离.此阶段IGD患者多经过专业治疗并且病情得到一定控制,因此数字干预目标是削弱由易感因素驱动的自动化使用反应,并重建对游戏行为的自主控制.干预时机需尽可能贴近高风险行为本身,而非仅依赖固定频率的心理治疗.远程心理服务在此阶段具有关键作用,并与智能终端和数字自助干预协同运作:治疗师通过远程咨询提供连续的专业支持,同时制定个性化自助方案以在日常情境中强化情绪、认知与行为调节;智能终端则实时采集行为与情绪数据,为治疗师调整干预计划提供参考,使方案更具针对性与动态适配性.在此阶段,通过这种实时的、多方面的干预策略,青少年的IGD症状得以有效控制,情绪调节和执行控制功能得到改善.

在后期阶段,青少年虽已接受系统治疗并获得一定改善,但仍面临复发风险.此阶段的干预目标从症状控制转向巩固康复,确保系统治疗后的认知功能可以迁移到不同情境.融合EMI的移动健康管理应用与数字自助干预在此期间尤为重要.通过各类数字化自助服务,为个体提供随时可用的情绪调节与健康管理支持,促进其在学习、运动和现实社交中逐步恢复功能.此外,健康管理应用能够系统记录易感因素、调节方式与康复过程指标,从而强化自我效能,促进个体对复发的规律性与可管理性形成理性认知.在此阶段,青少年对高强度专业投入的依赖逐渐降低,最终可借助低强度、自动化的数字监测来维持健康的生活模式并降低复发可能性.

5 未来展望

青少年IGD干预随着网络硬件和游戏软件的迭代发展也在不断更新进步,理论指导和临床应用也在持续推进,但数字化干预现阶段仍存在以下不足,需要在未来进一步改善.

第一,IGD数字化干预技术创新亟待突破.虽然IGD数字化干预已经初见成效,但有效干预技术数量仍然相对较少76,有许多新技术的应用有待探索.例如,脑机接口技术可以解码个体的神经表征从而以闭环方式个性化刺激相应脑区77.但目前实证研究数量不足且样本规模有限,其便携性和算法实时性也制约着临床推广,未来可以考虑IGD背景下的应用研究.

第二,传统IGD心理咨询治疗的数字化急需重构.传统治疗方式,尤其是传统心理咨询,其疗效毋庸置疑.面对日益扩大的心理治疗资源缺口,传统咨询模式已难以完全满足当前的需求.COVID-19大流行进一步加速了数字技术在心理治疗方面的革新72,但是数字化治疗在突破时空限制的同时也面临双重挑战:技术层面存在数字鸿沟导致的依从性差异,伦理层面需建立符合标准的用户数据管理体系.

第三,需要减少个体中心的单维干预治疗.在数据维度,应整合游戏行为日志(如操作延迟时间)、生理指标(如心率)和情感计算(如微表情识别)等多模态数据,构建高度融合的数字监测体系;在服务维度,需打通个体、家校、医院等多维度数据资料壁垒,实现从教育、监测、治疗的多方数据共建共享.然而,这些融合需要建立计算机科学、临床心理学和公共卫生的协同创新机制,不仅需要考虑多源异构数据的标准化清洗方法、动态干预策略的强化学习优化,还需要考虑临床验证路径.
