基于“因瘀致毒,瘀毒致变”探讨心力衰竭合并利尿剂抵抗的辨治

基于“因瘀致毒,瘀毒致变”探讨心力衰竭合并利尿剂抵抗的辨治
Treatment of heart failure complicated with diuretic resistance based on the theory of toxins caused by blood stasis and lesions induced by blood stasis and toxins
心力衰竭合并利尿剂抵抗的发病属于“瘀毒致病”的范畴,其核心病机演变为“因瘀致毒,瘀毒致变”。中医药可通过抗炎、抗氧化、调节血管内皮功能等途径,发挥改善心力衰竭合并利尿剂抵抗的作用。基于瘀毒理论,结合现代医学研究,系统探讨瘀毒与水湿、气滞、阳虚等因素的交互作用及在疾病进展中的动态机制。瘀血痹阻心脉,日久化热生毒,毒瘀搏结,壅塞三焦,阻碍气血津液输布,致水液气化不利、水肿顽固难消则加重心力衰竭,致利尿剂抵抗。解毒活血是本病的治本之法,针对兼证的不同,提出了兼顾益气温阳、疏肝理气、健脾利水及虫类药通络解毒等的治法。强调了“化瘀解毒、扶正固本”的辨治原则,旨在通过调和气血、疏通经络、解毒利水改善心功能并提高利尿剂敏感性。
Heart failure combined with diuretic resistance falls within the category of "diseases caused by blood stasis and toxins", and its core pathogenesis evolution features "toxins caused by blood stasis and lesions caused by blood stasis and toxins". Traditional Chinese medicine can play a role in improving heart failure complicated with diuretic resistance through anti-inflammation, antioxidant and vascular endothelial function regulation and other pathways. Based on the theory of blood stasis and toxins, combined with modern medical research, this paper systematically explores the interaction between blood stasis and toxins and factors such as water dampness, qi stagnation, and yang deficiency, as well as the dynamic pathogenesis in the progression of this disease. It states that blood stasis blocks the heart meridian, which over time transforms into heat and generates toxins. The toxins and blood stasis accumulate and block sanjiao, hindering the distribution of qi, blood and body fluids, which leads to a poor transformation of body fluids and persistent edema, thereby aggravating heart failure and causing a resistance to diuretics. While detoxification and promoting blood circulation count as the fundamental treatment methods for this disease, treatment methods are proposed in this paper that take into account benefiting qi and warming yang, dispersing the liver and regulating qi, strengthening the spleen and promoting diuresis, as well as using insect drugs to unblock meridians and detoxify based on different concurrent syndromes. This study underscores the therapeutic principle of "removing blood stasis and detoxifying, strengthening and consolidating the body's resistance", aiming to improve cardiac function and diuretic sensitivity by harmonizing qi and blood, unblocking meridians, detoxifying and promoting diuresis.
