肺表面活性物质联合枸橼酸咖啡因治疗新生儿呼吸窘迫综合征的效果
Efficacy of pulmonary surfactant combined with caffeine citrate on neonatal respiratory distress syndrome
目的 探究肺表面活性物质(pulmonary surfactant,PS)联合枸橼酸咖啡因治疗新生儿呼吸窘迫综合征(neonatal respiratory distress syndrome,NRDS)的临床疗效。 方法 选取收治的NRDS患儿92例,用随机双色球法分为观察组和对照组,每组46例。对照组给予枸橼酸咖啡因治疗,观察组在对照组治疗的基础上联合PS治疗。2组均连续治疗7 d。记录2组患儿的临床指标(机械通气时间、给氧时间和住院时间);比较2组患儿治疗7 d后的临床治疗效果;比较2组治疗前后的血气指标[动脉氧分压(partial pressure of oxygen,PaO2)、动脉二氧化碳分压(partial pressure of carbon dioxide,PaCO2)和血氧饱和度(blood oxygen saturation,SpO2)]及血清生化因子[血清铁蛋白(serum ferritin,SF)、C反应蛋白(C-reactive protein,CRP)和降钙素原(procalcitonin,PCT)];记录2组患儿治疗期间并发症的发生情况。 结果 治疗7 d后,观察组的给氧时间、机械通气时间和住院时间均明显短于对照组(P<0.05);观察组的临床治疗总有效率(93.5%)明显高于对照组(78.3%),P<0.05;2组PaO2、SpO2水平均较治疗前明显升高,且观察组明显高于对照组(P<0.05);2组PaCO2、SF、CRP和PCT水平均较治疗前明显降低,且观察组明显低于对照组(P<0.05);观察组患儿治疗期间并发症的发生率(13.0%)明显低于对照组(30.4%),P<0.05。 结论 PS联合枸橼酸咖啡因可有效提高NRDS的临床治疗效果,改善患儿的血气指标,促进患儿肺功能恢复,且患儿治疗期间并发症的发生风险较低,具有较好的安全性。
Objective To explore the clinical efficacy of pulmonary surfactant (PS) combined with caffeine citrate in the treatment of children with neonatal respiratory distress syndrome (NRDS). Methods 92 children with NRDS were enrolled and divided into observation group (n=46) and control group (n=46) by using the random two-color ball method. The control group was treated with caffeine citrate, and the observation group was combined with PS on the basis of the control group. The both groups were continuously treated for 7 days. The clinical indicators (mechanical ventilation time, oxygen supply time, and hospital stay) in the 2 groups were recorded, and the clinical treatment effect after 7 days of treatment and blood gas indexes [arterial partial pressure of oxygen (PaO2), arterial partial pressure of carbon dioxide (PaCO2), blood oxygen saturation (SpO2)] and serum biochemical factors [serum ferritin (SF), C-reactive protein (CRP), procalcitonin (PCT)] before and after treatment were compared between the 2 groups. The complications were recorded in the 2 groups during treatment. Results After 7 days of treatment, the observation group had significantly shorter oxygen supply time, mechanical ventilation time and hospital stay than the control group (P<0.05). The total effective rate of clinical treatment in observation group (93.5%) was significantly higher than that in control group (78.3%) , P<0.05. The PaO2 and SpO2 in the 2 groups were significantly enhanced compared with those before treatment, and the above indicators in observation group were significantly higher than those in control group (P<0.05). The PaCO2, SF, CRP and PCT in the 2 groups were significantly reduced compared with before treatment, and the indicators were significantly lower in observation group than those in control group (P<0.05). The total incidence rate of complications in observation group (13.0%) was significantly lower compared with that in control group (30.4%) (P<0.05). Conclusion PS combined with caffeine citrate can effectively enhance the clinical treatment effect of NRDS, improve the blood gas indexes,and promote the recovery of pulmonary function, and it has low risk of complications during treatment and good safety.
