矛头蝮蛇血凝酶联合奥美拉唑治疗上消化道出血患者的疗效

Efficacy of haemocoagulase from Agkistrodon acutus combined with omeprazole in patients with upper digestive tract hemorrhage

摘要

目的 探讨矛头蝮蛇血凝酶联合奥美拉唑治疗上消化道出血患者的疗效及对炎症和血液流变学的影响。 方法 选取收治的106例上消化道出血患者作为研究对象，用随机数字表法分为观察组和对照组，每组53例。对照组予以奥美拉唑治疗，观察组在对照组治疗的基础上予以矛头蝮蛇血凝酶治疗。比较2组患者的临床疗效、炎症因子水平、血液流变学以及临床指标。 结果 观察组的临床有效率为94.34%，高于对照组（81.13%），P0.05；治疗后，与对照组比较，观察组的超敏C反应蛋白（high sensitive C-reactive protein，hs-CRP）、肿瘤坏死因子-α（tumour necrosis factor-α，TNF-α）和白细胞介素-6（interleukin-6，IL-6）水平显著下降（P0.05）；红细胞沉降率显著降低（P0.05），红细胞压积、全血高切黏度和全血低切黏度明显升高（P0.05），止血时间、住院时间明显缩短（P0.05），复发率明显降低（P0.05）。 结论 矛头蝮蛇血凝酶联合奥美拉唑治疗上消化道出血能提高临床疗效，改善患者的临床指标和血液流变学，缓解炎症反应。

Abstract

Objective To investigate the efficacy of haemocoagulase from Agkistrondon acutus combined with omeprazole in patients with upper digestive tract hemorrhage and the effects on inflammation and hemorheology. Methods A total of 106 patients with upper digestive tract hemorhage admitted to our hospital were randomly divided into an observation group （n=53） and a control group （n=53）. The control group was treated with omeprazole， while the observation group was treated with haemocoagulase from Agkistrodon acutus in addition. The clinical efficacy， the levels of inflammatory factors， hemorheology and clinical indicators were compared between the 2 groups before and after treatment. Results The clinical effective rate of the observation group （94.34%） was higher than that of the control group （81.13%）， P0.05. After treatment， the levels of high sensitive C-reactive protein （hs-CRP）， tumour necrosis factor-‍α （TNF-‍α） and interleukin-6 （IL-6） in the observation group decreased more significantly than those in the control group （P0.05）. The erythrocyte sedimentation rate in the observation group was lower than that in the control group （P0.05）， and the hematocrit， whole blood high shear viscosity and whole blood low shear viscosity were higher than those in the control group （P0.05）. The hemostasis time and hospitalization time in the observation group were shorter than those in the control group （P0.05）， and the recurrence rate was lower than that in the control group （P0.05）. Conclusion The hemocoagulase therapy is beneficial to patients with upper gastrointestinal bleeding， which can improve the clinical efficacy， clinical indicators and hemorheology， and relieve inflammation.

上消化道出血患者的主要表现为十二指肠、胃等部位出血^［1］。其病因复杂、发病迅速，主要原因包括消化性溃疡、肝硬化等^［2-3］。因此及时、有效的止血对于患者的生命安全至关重要^［4］。从矛头腹蛇中提取的生物活性物质蛇毒类血凝酶具有止血作用，已经应用于多种原因引起的消化系统出血的患者^［5］。本研究拟探讨矛头蝮蛇血凝酶联合奥美拉唑治疗上消化道出血患者的疗效及对炎症及血液流变学的影响。

1 资料

1.1 一般资料

选取本院收治的106例上消化道出血患者作为研究对象，用随机数字表法分为观察组和对照组，每组53例。观察组：男36例，女17例；平均年龄为（47.28±7.16） 岁；十二指肠溃疡患者28例，胃溃疡患者17例，糜烂性出血胃炎患者8例。对照组：男36例，女17例；平均年龄为（47.28±7.16） 岁；其中十二指肠溃疡患者25例，胃溃疡患者16例，糜烂性出血胃炎患者12例。2组患者一般资料比较差异无统计学意义（P0.05），具有可比性。

1.2 纳入标准与排除标准

纳入标准：①符合消化道出血临床诊断标准^［6］者；②经内镜检查确诊者；③此前未接受相关治疗者；④对本研究所用药物无过敏史者；⑤患者和家属知情同意，并签署知情同意书者。

排除标准：①存在严重肝、肾功能异常者；②消化系统存在肿瘤者；③凝血功能异常者；④认知功能障碍者；⑤中途退出研究者。

2 方法

2.1 治疗方法

对照组给予抗炎和抗休克治疗方案，同时维持血容量，将40 mg奥美拉唑（山东新时代药业有限公司）溶于100 mL生理盐水中进行静脉滴注，每次滴注30 min，每日3次，连续治疗5 d。

观察组在对照组治疗的基础上，将2 U矛头腹蛇血凝酶（蓬莱诺康药业有限公司）溶于20 mL生理盐水中进行静脉滴注，每日2次，连续治疗5 d。

2.2 观察指标

2.2.1 临床疗效

在1~2 d内没有发生活动性出血为显效；49~72 h之内完成止血，同时未发生再次出血为有效；72 h内出血现象没有得到缓解，内镜检查显示仍然伴有持续性出血，需要更换治疗方法为无效^［7］。

2.2.2 炎症因子

治疗前后分别抽取患者5 mL外周静脉血，以3 000 r·min^-1离心10 min，取上清，采用免疫比浊法检测超敏C反应蛋白（hypersensitive C-reactive protein，hs-CRP），应用酶联免疫吸附试验检测肿瘤坏死因子-α（tumour necrosis factor-α，TNF-α）和白细胞介素-6（interleukin-6，IL-6）。

2.2.3 血液流变学

治疗前后分别抽取患者5 mL空腹静脉血，利用BC-6000型全自动血液分析仪（深圳迈瑞生物医疗电子股份有限公司）检测红细胞沉降率、红细胞压积水平、全血高切黏度水平及全血低切黏度的水平。

2.2.4 临床指标

观察患者止血时间、住院时间以及复发率的情况。

2.3 统计学方法

用SPSS 18.0软件对数据进行分析处理。计数资料用“例（%）”表示，采用χ² 检验；计量资料用（\overline{x}±s）表示，采用t检验。P0.05为差异有统计学意义。

3 结果

3.1 2组临床疗效的比较

观察组的临床总有效率为94.34%，显著高于对照组的81.13%（P0.05）。见表1。

3.2 治疗前后2组患者炎症因子的比较

治疗前，2组的炎症因子水平比较差异无统计学意义（P0.05）；治疗后，2组的hs-CRP、TNF-α和IL-6水平与治疗前比较显著降低（P0.05），且观察组的hs-CRP、TNF-α和IL-6水平与对照组相比下降幅度更明显（P0.05）。见表2。

3.3 治疗前后2组患者血液流变学的比较

治疗前，2组的血液流变学比较差异无统计学意义（P0.05）；治疗后，2组的红细胞沉降率较治疗前明显降低（P0.05），红细胞压积、全血高切黏度和全血低切黏度较治疗前明显升高（P0.05），且观察组的红细胞沉降率明显低于对照组（P0.05），红细胞压积、全血高切黏度和全血低切黏度均明显高于对照组（P0.05）。见表3。

3.4 2组患者临床指标的比较

观察组的止血时间、住院时间较对照组明显缩短（P0.05），复发率较对照组显著降低（P0.05）。见表4。

4 结论

当前临床应用质子泵抑制剂治疗上消化道出血，有一定的止血作用^［8-9］。但相关研究结果表明，长期服用质子泵抑制剂可增加肠道感染的发生率，因此不宜长期服用^［10］。奥美拉唑是一种新型质子泵抑制剂，其能够抑制胃酸分泌，在短时间内改善并维持胃液的pH值^［11］。同时，奥美拉唑还可以促进凝血，从而发挥止血、抑酸的作用^［12］。蛇毒类血凝酶是临床上应用较多的止血药物，其对基质的特异性很高，通过将纤维蛋白A分解为短肽，从而使纤维蛋白单体在非共价交联中聚合成可溶性纤维蛋白多聚体，起到止血的作用^［13］。蛇毒类血凝酶因其具有良好的止血作用，故被用于治疗多种消化道出血^［14］。

本研究结果显示，观察组患者的临床总有效率为94.34%，明显高于对照组的81.13%（P0.05），表明矛头蝮蛇血凝酶联合奥美拉唑治疗上消化道出血患者有利于提高临床疗效，这与张锐^［15］的研究结果相似，表明2种药物相互作用能发挥更好的功效。红细胞沉降率可以间接反映红细胞的体积和大小^［16］。全血黏度可以反映血液流速，作为诊断血栓性疾病的参考指标^［17］。本研究结果显示，治疗后观察组的红细胞沉降率较对照组明显降低，红细胞压积、全血高切黏度和全血低切黏度较对照组明显降低（P0.05），表明矛头蝮蛇血凝酶有利于改善上消化道出血患者的血液流变学，这与彭婕^［18］的研究结果相似。原因可能是蛇毒类血凝酶中含有凝血因子X活化因子和类凝酶，其具有较好的止血效果。

出血和应激都会影响人体的免疫系统，从而引起细胞炎症因子的变化^［9］。相关研究发现，IL-6、TNF-α参与上消化道出血的发生、发展过程，通过降低机体的免疫功能和诱导炎症的级联反应，从而促进炎症的扩散，加重出血部位的损伤，进一步加重出血^［19］。本研究结果显示，观察组患者的hs-CRP、TNF-α、IL-6水平较对照组显著下降，这与梅婷等^［20］的研究结果相似，原因可能是经蛇毒类血凝酶治疗后，出血现象得到缓解，炎症反应减轻。此外，治疗后观察组的止血时间、住院时间较对照组显著缩短，复发率较对照组显著降低，原因可能是矛头蝮蛇血凝酶中的类凝血激酶使凝血酶原转化为凝血酶，使血浆不含钙时仍能生成不稳定的纤维蛋白，从而增强血液的凝聚能力，最终达到止血效果。

综上所述，矛头蝮蛇血凝酶联合奥美拉唑对上消化道出血患者治疗效果较好，止血作用显著，可有效改善患者的临床指标和血液流变学，同时缓解炎症反应。
