基于Wnt/ β -catenin信号通路探讨复方丹参滴丸联合坎地沙坦酯对糖尿病肾病的治疗作用

基于Wnt/ β -catenin信号通路探讨复方丹参滴丸联合坎地沙坦酯对糖尿病肾病的治疗作用
Exploring the therapeutic effect of Compound Danshen Dripping Pills in conjunction with candesartan on diabetic nephropathy based on the Wnt/ β -catenin signaling pathway
目的 基于Wnt/β-连环蛋白(Wnt/β-catenin)信号通路探讨复方丹参滴丸联合坎地沙坦酯治疗糖尿病肾病(diabetic kidney disease,DKD)的效果。 方法 选取DKD患者126例,采用随机抽签法分为对照组(63例)和联合组(63例),对照组给予坎地沙坦酯治疗,联合组给予复方丹参滴丸联合坎地沙坦酯治疗,2组均治疗8周。比较2组的疗效、糖代谢指标[糖化血红蛋白(hemoglobin A1c,HbA1c)、空腹血糖(fasting plasma glucose,FPG)、胰岛素抵抗指数(homeostasis model assessment of insulin resistance,HOMA-IR)]、肾损伤指标[尿骨桥蛋白(osteopontin,OPN)、肾损伤分子-1(kidney injury molecule-1,KIM-1)、血尿素氮(blood urea nitrogen,BUN)、血肌酐(serum creatinine,Scr)]、免疫炎症因子[细胞间黏附分子-1(intercellular adhesion molecule-1,ICMA-1)、单核细胞趋化蛋白-1(monocyte chemoattractant protein-1,MCP-1)、白细胞介素-6(interleukin-6,IL-6)]、Wnt/β-catenin信号通路及安全性。 结果 联合组的临床治疗总有效率(95.24%)高于对照组(82.54%),P<0.05;治疗4、8周后,联合组的FPG、HbA1c、HOMA-IR低于对照组(P<0.05),联合组的OPN、KIM-1、BUN、Scr低于对照组(P<0.05),联合组的MCP-1、IL-6、ICMA-1低于对照组(P<0.05),联合组的Wnt、β-catenin的mRNA和蛋白表达低于对照组(P<0.05);联合组的不良反应总发生率(9.52%)与对照组(6.35%)比较,差异无统计学意义(P>0.05)。 结论 复方丹参滴丸联合坎地沙坦酯治疗DKD的疗效显著,能改善患者的血糖水平,减轻肾损伤程度,抑制免疫炎症反应,下调Wnt/β-catenin信号通路表达,且安全性较高。
Objective To explore the effect of compound Danshen Dripping Pills combined with candesartan cilexetil in the treatment of diabetic kidney disease (DKD) based on the Wnt/β-catenin signaling pathway. Methods A total of 126 patients with DKD were selected and randomly divided into the control group (63 cases) and the combination group (63 cases) using a random lottery method. The control group was treated with candesartan cilexetil, while the combination group was treated with compound Danshen Dripping Pills in combination with candesartan cilexetil. Both groups were treated for 8 weeks. The control group was given candesartan ester treatment, while the combined group was given Compound Danshen Dripping Pills combined with candesartan ester treatment, and both groups were treated for 8 weeks. The therapeutic effects, glucose metabolism indices [hemoglobin A1c (HbA1c), fasting blood glucose (FPG), and insulin resistance index (HOMA-IR)], kidney injury markers [osteopontin (OPN), kidney injury molecule-1 (KIM-1), blood urea nitrogen (BUN), and serum creatinine (Scr)], immunoinflammatory factors [intercellular adhesion molecule-1 (ICAM-1), monocyte chemoattractant protein-1 (MCP-1), and interleukin-6 (IL-6)], Wnt/β-catenin signaling pathway and their safety were compared between the 2 groups. Results The total effective rate of the combined group was 95.24% higher than that of control group 82.54% (P<0.05). After 4 and 8 weeks of treatment, the FPG,HbA1c and HOMA-IR in combination group were lower than those in control group (P<0.05), the OPN, KIM-1, BUN and Scr in the combination group were lower than those in the control group (P<0.05), the MCP-1, IL-6 and ICMA-1 in the combination group were lower than those in the control group (P<0.05),the mRNA and protein expressions of Wnt and β-catenin in the combination group were lower than those in the control group (P<0.05). There was no significant difference in the overall incidence of adverse reactions between the combination group (9.52%) and the control group (6.35%) (P>0.05). Conclusion Compound Danshen Dripping Pills combined with candesartan ester are effective in the treatment of DKD, which can improve blood glucose levels, reduce the severity of kidney injury,inhibit the immunoinflammatory response, and down-regulate the expression of the Wnt/β-catenin signaling pathway, demonstrating a high safety profile.
