三维影像联合手术录像编辑在提升住院医师外科技能中的应用研究

三维影像联合手术录像编辑在提升住院医师外科技能中的应用研究
Research on the application of three-dimensional image combined with surgical video editing in promoting surgical skills of residents
目的 探讨三维可视化影像联合手术录像编辑教学模式在提升住院医师外科技能中的应用效果。 方法 选取2021年7月— 2022年6月在中山大学附属第六医院肝胆胰脾外科进行规范化培训的36名住院医师为研究对象,随机分为对照组(n=18)和试验组(n=18)。对照组采用临床阅片结合临床教师手术回放讲解的传统教学方法,试验组采用三维可视化影像联合住院医师自主进行手术录像编辑和讲解的教学方法。两组均采用腹腔镜训练器考核和问卷调查进行评价。 结果 腹腔镜训练器考核结果显示,试验组腹腔镜下单结缝合打结平均耗时为(37.2±3.4) s,对照组为(48.4±5.8) s,两组差异具有统计学意义(t=7.084,P<0.001)。问卷调查结果显示,试验组认为:新教学模式对调动学员学习积极性有帮助的比例高于对照组(88.9% vs. 50.0%,P=0.027);对激发工作兴趣有帮助的比例高于对照组(88.9% vs. 44.4%,P=0.012);对提高解剖认识有帮助的比例高于对照组(100.0 % vs. 38.9%,P=0.001);对加深手术理解有帮助的比例高于对照组(100.0 % vs. 33.3%,P=0.001);对提升腔镜技能有帮助的比例高于对照组(94.4% vs. 33.3%,P=0.001);对教学模式满意的比例也高于对照组(100.0% vs. 55.6 %,P=0.003)。 结论 三维可视化影像联合手术录像编辑教学模式能够有效调动住院医师的学习积极性,激发其对肝胆胰脾外科的从业兴趣,提高其对于肝胆胰脾外科相关解剖的认识,加深对手术的理解和提升腹腔镜下的操作技能,值得临床上推广应用。
Objective To explore the application effect of three-dimensional visual image combined with surgical video editing teaching mode in promoting surgical skills of residents. Methods A total of 36 residents who participated in the standardized training in the Department of Hepatobiliary, Pancreatic and Splenic Surgery of the Sixth Affiliated Hospital, Sun Yat-sen University from July 2021 to June 2022 were selected as the research subjects and randomly divided into the control group and the experimental group, with 18 members in each. The traditional teaching method of clinical image reading combined with teachers’ interpretation of clinical surgical playback was used in the teaching of the control group, while the teaching method of three-dimensional visual image combined with residents’ autonomous surgical video editing and interpretation was adopted in the teaching of the experimental group. Both groups were evaluated by laparoscopic trainer assessment and questionnaire survey. Results Result of the evaluation of laparoscopic trainer showed that the average time of laparoscopic single knot suture and knot tying of the experimental group was (37.2±3.4) s, and that of the residents of the control group was (48.4±5.8) s, and the difference was statistically significant (t=7.084, P<0.001). The questionnaire result showed that the proportion of residents in the experimental group who considered the new teaching mode was conducive to mobilizing residents’ learning enthusiasm was higher than that in the control group (88.9% vs. 50.0%, P=0.027). The proportion of residents in the experimental group who thought the new teaching mode was helpful to stimulate work interest was higher than that in the control group (88.9% vs. 44.4%, P=0.012). The proportion of residents in the experimental group who considered it helped enhance their anatomical understanding was higher than that of the control group (100.0% vs. 38.9%, P=0.001). The proportion of residents in the experimental group who thought it was helpful to deepen their understanding of surgery was higher than that of the control group (100.0% vs. 33.3%, P=0.001). The proportion of residents in the experimental group who thought it was helpful to improve their endoscopic skills was higher than that of the control group (94.4% vs. 33.3%, P=0.001). In addition, the proportion of residents in the experimental group satisfied with the teaching mode was also higher than that of the control group (100.0% vs. 55.6%, P=0.003). Conclusion The teaching mode of three-dimensional visual images combined with surgical video editing can effectively engage residents’ learning initiative, stimulate their interest in hepatobiliary, pancreatic and splenic Surgery, improve their comprehension of related anatomy of hepatobiliary, pancreatic and splenic surgery, deepen their understanding of surgery and promote their laparoscopic skills. It is worth popularizing clinically.
