胰十二指肠切除术(PD)后胰瘘(POPF)是影响患者预后的主要并发症,其发生与胰肠吻合口愈合密切相关。本文系统梳理近年来国内外相关研究,从术前评估、术中技术及术后管理三个方面,阐述影响PD术后胰肠吻合口愈合的关键因素及其防治策略。术前方面,胰腺质地柔软、主胰管细小、肥胖及炎症-营养指标异常(如C-反应蛋白/白蛋白比值)是重要风险因素;术中方面,吻合方式的选择应基于胰腺条件个体化决策,新型缝合材料、机器人辅助手术及生物材料(如水凝胶)在降低POPF风险方面展现出潜在优势;术后方面,引流液细菌定植控制、早期引流管移除及血糖管理是影响愈合的重要环节。未来应进一步探索多因素协同机制,构建动态预测模型,并通过高质量临床研究验证新技术与新材料的有效性,以实现POPF的精准化与个体化防治。

Abstract

Postoperative pancreatic fistula (POPF) remains a major complication affecting prognosis after pancreaticoduodenectomy (PD), closely associated with the healing of pancreaticojejunostomy. This review systematically summarizes recent advances in the key factors influencing anastomotic healing after PD from three aspects: preoperative assessment, intraoperative techniques, and postoperative management. Preoperatively, soft pancreatic texture, small main pancreatic duct, obesity, and inflammatory-nutritional markers (such as the C-reactive protein-to-albumin ratio) are identified as major risk factors. Intraoperatively, the choice of anastomotic technique should be individualized based on pancreatic characteristics, while novel suture materials, robotic assistance, and emerging biomaterials (e.g., hydrogels) show promising potential in reducing POPF risk. Postoperatively, control of bacterial colonization in drainage fluid, early drain removal, and strict glycemic management play critical roles in anastomotic healing. Future studies should focus on elucidating multifactorial synergistic mechanisms, developing dynamic predictive models, and validating new techniques and biomaterials through high-quality clinical trials to achieve precision and individualized management of POPF.

胰十二指肠切除术(pancreaticoduodenectomy,PD)是治疗胰头癌及壶腹周围肿瘤的标准手术方式,然而该手术范围广、创伤大、技术复杂,术后并发症发生率高。尽管随着手术技术和围手术期管理的进步,高容量中心PD手术死亡率已降至3%以下[1]。但术后胰瘘(postoperative pancreatic fistula,POPF)经常发生,发病率超过10%[1-3]。因此无论术式如何,POPF与许多其他术后并发症有关,包括腹腔内积液、腹腔内感染(intra-abdominal infection,IAI)、胃排空延迟(delayed gastric emptying,DGE)、胰腺切除术后出血(post-pancreatectomy hemorrhage,PPH)及再次手术[4]。这些并发症可导致住院时间延长,最终影响患者的预后和生活质量[4],甚至可能危及生命[5]。因此,系统整合影响胰肠吻合口愈合的关键因素,对于优化围手术期管理策略、降低POPF发生率具有重要意义。
根据国际胰瘘研究小组2016年修订的标准,POPF被定义为术后第3天或以后引流液中淀粉酶浓度超过正常血清淀粉酶上限3倍,并根据其对临床进程的影响分为三级:A级(生化漏,无需特殊处理)、B级(需临床干预)和C级(危及生命,需重大干预)。B级和C级胰瘘合称为临床相关胰瘘(clinically relevant POPF,CR-POPF),是临床管理的重点与难点[6]
近年来,POPF防治策略日益多维度化,因此,许多研究都集中在降低POPF和POPF相关并发症的发生风险[7],包括胰腺解剖与生理特征,手术技术的改变、术后细菌污染、代谢管理与药物支持(生长抑素或奥曲肽的应用)治疗等因素[8-9],这对于早期识别高危患者、优化手术决策、实施精准干预至关重要。本文系统检索近年来国内外相关文献,从术前评估、术中技术、术后管理三个维度,全面归纳影响PD术后胰肠吻合愈合的关键因素,旨在为临床防治POPF提供理论依据,并为未来研究指明方向。

1 术前评估的进展

术前患者的生理状态与胰腺局部解剖特征是预测POPF风险的基础。近期研究进一步验证了这些因素的作用,同时致力于开发整合性预测工具,实现更早期精准风险分层。

1.1 胰腺解剖与生理特征

胰腺的质地柔软与胰管直径<3 mm是PD术后CR-POPF被广泛接受的风险因素之一[10]。有研究[10-12]对行PD手术的患者进行回顾性队列分析,认为质地柔软的胰腺外分泌功能较为旺盛,富含蛋白水解酶的胰液分泌增加、组织脆性增加、缝合线保持能力较差从而导致吻合口出血缺血或坏死,最终导致愈合不良。且主胰管(main pancreatic duct,MPD)直径细小(通常以<3 mm为临界值)使得黏膜对黏膜吻合技术难度增大,吻合口通畅性难以保障。外科医生触诊是判断质地的常用方法,随着影像学技术的发展,CT测量及硬度测量等客观方法也是常用的评估方式[10]

1.2 肥胖与营养状态

1.2.1 肥胖

据研究结果表明,高体质量指数(bady mass index,BMI)(≥25 kg/m2)或内脏脂肪面积过大(≥92 cm2)是POPF的明确危险因素[13-16]。肥胖可能通过促进胰腺脂肪浸润(增加腺体柔软度)以及导致术后高腹压,损伤压迫吻合口血管,影响吻合口血供与愈合,从而导致延迟愈合[2,17]。尽管高BMI被多项研究明确为POPF危险因素[13-16],但也有研究指出BMI作为单一指标存在局限性,体脂分布(如内脏脂肪面积、腹膜-门静脉距离)可能更具预测价值[15]。未来需进一步探讨不同肥胖表型对术后结局的影响[16]

1.2.2 低白蛋白血症(<2.5 g/dL)

术前白蛋白水平<2.5~3.0 g/dL提示营养状况不佳,可能影响组织修复能力与胶原蛋白的合成,从而延迟胰肠吻合愈合,增加POPF风险[2]。Panayi等[18]通过查询ACS-NSQIP数据库(2008—2021年)中980万例手术患者的数据表明营养状态和白蛋白的联合评估是手术预后最强的风险预测因子,因此对于接受手术的患者,建议同时考虑白蛋白水平和营养状态情况,优化围手术期管理策略。

1.2.3 C-反应蛋白/白蛋白比值(C-reactive protein-to-albumin ratio,CAR)

白蛋白降低是POPF常见的风险因素,许多研究表明,CAR与包括胰腺癌在内的多种癌症患者的预后相关,Funamizu等[19]通过比较POPF组与非POPF组的CAR差异,确定临界值(≥0.05),通过确立的CAR临界值进行二分类曲线分析,表明术前CAR≥0.05可能成为POPF的危险因素。Qu等[20]表明,CAR综合反映了全身炎症反应与营养状况,可作为一项有效的术前预测指标,在术前将其调整到安全范围来降低POPF风险。

2 术中不同吻合术和缝合材料选择的研究进展及现状

2.1 胰肠吻合技术

自1898年Codivilla首次实施了PD,至今已经有120多年的历史,随着外科手术技术、手术设备和缝合材料的更新,以及围手术期管理模式的改进,PD已变得日趋成熟,成为国内外胰腺外科中心的常规手术之一[21]。迄今为止,目前常见的胰肠吻合方式包括:导管对黏膜胰肠吻合、套入式胰肠吻合、捆绑式胰肠吻合,以及多种改良的胰肠吻合术式[22],如Blumgart胰肠吻合[23]、“陈氏”贯穿纵向“U”形胰肠吻合[24]等。尽管存在多种胰肠吻合技术,但多项Meta分析显示,不同吻合方式在CR-POPF发生率上无显著差异[22,25],因此术式的选择需综合考虑患者胰腺条件、术者经验及术中情况,未来需要更多高质量随机对照试验(RCT)研究以明确最优吻合方式。

2.1.1 套入式吻合术

⑴ 标准套入式吻合术:该术式适用于MPD直径<3 mm且胰腺质地柔软的患者,通过将胰腺残端内陷入空肠实现吻合[26]。2015年,陈孝平院士创建了“陈氏”贯穿纵向“U”形胰肠吻合,其对胰腺残端进行鱼口状封闭来保证吻合处的密封性,但目前有关“陈氏”吻合的公开研究报道较少,临床异质性明显[25-26];1996年,彭淑牖教授提出捆绑式胰肠吻合,该术式始终保持浆肌层完整,同时用石炭酸或者电凝的方式破坏相连的空肠黏膜,从而避免了两层之间的液体积聚,从根本上解决了发生POPF的风险,但该吻合方式中胰腺残端暴露于肠腔内,可能增加胰液对吻合口的腐蚀及残端出血风险[25]。⑵ 改良的套入式吻合术:在标准术式的基础上进行改良,如Li等[27]研究采用双荷包线连续闭锁缝合的内陷式胰肠吻合术,其核心在于将胰腺残端完全内陷进入空肠腔,该术式认为吻合口的愈合主要包括黏膜愈合及空肠浆膜外层与胰腺残存物之间的纤维愈合,且MPD长度的保留对维持血供至关重要。该术式对MPD直径没有限制,可利用空肠黏膜的快速再生能力实现胰管沿线的永久性生物愈合。

2.1.2 导管对黏膜吻合术

⑴ 标准导管对黏膜吻合术:该术式由Cattell和Warren于1956年首次提出,至今仍在临床广泛应用[26]。其优点在于确保胰管和空肠黏膜的连续性和通畅性,符合生理要求。但该术式技术难度大,当患者胰腺质地较软,胰管直径较细时,吻合过程中,易发生胰管及胰腺实质撕裂,影响吻合口质量[25]。⑵ 改良导管对黏膜吻合术:在标准术式的基础上改进,例如“2+3”胰肠吻合术,包括连接胰腺与空肠的2条缝线,以及连接MPD与空肠黏膜的3条断裂缝线,可更好地缝合MPD的后壁,从而完成高质量缝合并保持更好的缝合强度和张力,从而降低吻合狭窄的发生率[28]

2.2 术中辅助技术与材料使用

2.2.1 缝合技术与材料的创新

2021年专家共识[29]表明,目前可使用的缝合材料有人工合成的不可吸收缝线如丝线、聚丙烯缝线(如Prolene)等,及可吸收缝线如聚糖乳酸(PGLA)910缝线(如Vicryl)、含三氯生抗菌剂的聚对二氧环己酮缝线等。在胰液、胆汁等复杂的碱性环境中,人工合成可吸收的单股聚对二氧环己酮缝线相较于多股编织缝线其衰减程度明显较低,且打结顺滑,便于腹腔镜下操作;人工合成不可吸收的缝线在碱性环境下可长时间保持较高的抗张强度,但需打结数多于丝线或编织性缝线。天然材料中可吸收缝线,容易导致缝线周围产生炎性反应,目前少有应用[29]。且Liu等[30]回顾性队列研究表明,带倒刺缝合线在微创PD中显著降低CR-POPF发生率(7.5% vs. 22.4%,P=0.015)。

2.2.2 胰腺残端处理

Adamenko等[31]研究了一种创新的灌溉和被动引流技术,其基本原理在于稀释或去除从胰管未密封的分支渗漏的胰液,避免胰液的积聚,来降低淀粉酶和脂肪酶自动激活的风险,从而预防CR-POPF。在纳入的研究中,手术区域的连续冲洗可稀释胰腺分泌物浓度和细菌载量,并消除其随后导致的其他并发症的炎症过程[32]。且不同术式可对残端进行处理,例如Li等[27]所研究的双荷包线缝合的新型内陷手术,使用4-0 Prolene线穿过胰腺实质,连续荷包缝合锁紧,可将主胰导管周围残余胰组织仔细缝合,有效封闭了胰腺残余末端。

2.2.3 水凝胶片的应用

研究[33]报道使用间充质干细胞片预防POPF,但其存在促进残留肿瘤细胞增殖的风险,同时称使用水凝胶密封剂可预防POPF,但该密封剂使用起来困难,尚无关于用PVA水凝胶片预防POPF的报告。2025年Zhou等[34]研究开发了具有高效抗菌、抗炎和促进伤口愈合特性的多功能水凝胶。该水凝胶具有优异的可塑性、易于临床应用、良好的生物相容性、中等机械强度以及低膨胀和强附着力,此发现为开发下一代生物材料以应对POPF提供了新的选择。

2.2.4 促纤维化材料

多项Meta分析报告称软胰腺缝线保持能力较差,常因技术难题导致实质撕裂,同时纤维化是自然伤口愈合过程中不可或缺的成分,它通过积累胶原蛋白和其他间质物质,使组织变得更坚硬[35]。此外,Lee等[35]通过研究表明胰内注射青霉素G等化合物可通过激活胰腺星状细胞、诱导转化生长因子-β1通路,在POPF高发期(术后2~3 d)实现可逆性的胰腺纤维化,为预防POPF提供了新颖的机制性策略。

2.2.5 机器人辅助手术

机器人胰十二指肠切除术(robotic pancreaticoduodenectomy,RPD)与腹腔镜胰十二指肠切除术(laparoscopic pancreatico-duodenectomy,LPD)同属微创胰十二指肠切除术(minimally invasive pancreaticoduodenectomy,MIS-PD),随着技术的发展机器人手术系统提供高清3D视野和高度灵活的机械臂,对于MPD细小等高难度吻合,能提高操作精度,减少组织创伤,其次,MIS-PD的腹腔暴露时间比开放性PD更短,这可能会减少手术过程中的潜在感染,从而减少化脓性POPF的发生,尤其显著降低B级胰瘘的发生率[36]。但RPD存在明显局限性:学习曲线长、设备成本高、手术费用昂贵,且现有研究可能存在选择偏倚,因此RPD的推广应用需综合考虑医疗资源、患者经济条件和术者经验[37]

3 术后管理

3.1 并发症预防

3.1.1 肠道菌群失调

近年来患者微生物群已成为癌症研究的焦点,Biesel等[38]对178例PD患者的胰腺组织样本进行研究,表明肠道菌群多样性降低或致病菌富集可能影响宿主对肿瘤细胞的免疫反应,进而影响胰腺肿瘤患者的预后。其次,细菌污染被认为是POPF发生的关键因素,细菌分泌蛋白酶可直接激活胰蛋白酶原转化为胰蛋白酶,导致胰腺自身消化、胶原蛋白降解、降低组织机械强度[39]。同时产生的炎症级联反应(白细胞浸润、IL-6和CRP等细胞因子释放),将导致组织水肿和微循环障碍,延迟吻合口愈合甚至诱发破裂。未来需进一步研究抗生素干预能否阻断这一机制链,降低POPF发生率[39-40]

3.1.2 引流管的早期移除

PD术后预防性引流的重要性仍存在争议,没有报告讨论引流管的引流期间与术后并发症的关联[41],但有研究表明延迟引流管移除会增加细菌逆行迁移的风险,早期引流管移除似乎是合适的,并且可以减轻特定患者放置引流管的潜在并发症[42]。Kawai 等[41]表明,PD术后,术后第4天拔除腹腔引流管时,腹部引流液中细菌培养阳性率为3.7%,术后第8天拔除引流管时,腹部引流液细菌培养阳性率上升至30.8%。Li等[43]研究表明无论引流液淀粉酶(drain fluid amylase,DFA)水平如何,早期拔除引流管均可使患者获益,亚组分析显示,术后第1天DFA水平为5 000 IU/L且早期引流管拔除的患者,POPF发生率显著降低,术后第3天DFA水平可能大幅降低,此时拔除引流管安全且不增加并发症发生风险[44]

3.2 代谢管理与支持治疗​

3.2.1 术后血糖

PD需切除十二指肠及部分胰腺组织,其中十二指肠是葡萄糖依赖性促胰岛素多肽(glucose-dependent insulinotropic polypeptide,GIP)释放的肠道区域之一,有助于维持葡萄糖稳态,因此上述组织的切除可能会导致术后高血糖症,其次术后早期高血糖(≥140 mg/dL)与术后并发症的发展显著相关[45]。尤其在非糖尿病患者中(如术后第3天空腹血糖≥7.685 mmol/L)与POPF、腹腔感染、Clavien-Dindo并发症分级Ⅲ~Ⅴ级并发症的发生率相关。依据术后早期血糖水平可预测POPF、腹腔感染等并发症的发生,对甄别高危患者并采取积极干预措施具有重要的临床意义[45-46]

3.2.2 药物支持治疗

2014年,一项发表广泛发表的单中心RCT[47]发现,一种新型生长抑素类似物帕西雷肽,其半衰期更长且受体亲和力更高,可用于降低胰液分泌量,对POPF产生影响,但其与其他生长抑素类似物目前的应用价值仍有争议[48-49]。此外,糖皮质激素确实能降低炎症和应激反应,而胰腺切除术后并发症可能是诱导因素;红霉素可能对幽门舒张有影响,用于治疗DGE;质子泵抑制剂(proton pump inhibitors,PPI)抑制胃酸生成可能会降低吻合口出血率[50]。另外,PD、远端胰腺切除术或全胰腺切除术与内分泌和外分泌功能不全有关,口服替代脂肪酶和胰淀粉酶用于改善饮食功能,而胰岛素治疗可用于治疗内分泌不足[51]。PPI可以抑制胰腺残余物的萎缩,因此可能对获得外分泌和内分泌功能产生积极作用,从而预防胰瘘[50]

4 总结与展望

PD术后胰肠吻合口的预后是由贯穿围手术期全过程以及多维度风险因素来共同决定的复杂临床结局。本文系统总结了PD术后胰肠吻合口愈合的影响因素,构建了从术前评估、术中优化到术后管理的综合防治框架:术前,胰腺质地柔软、MPD<3 mm、高BMI及低白蛋白血症等是POPF的独立危险因素[11-17],CAR可作为有效的术前预测指标[19-20]。术中,吻合技术与缝合材料的选择直接影响吻合口质量[22-29],机器人辅助手术、新型水凝胶片及促纤维化材料等技术创新为高风险患者提供了新的预防策略[34-38]。术后,引流液微生物定植是POPF发生的重要风险因素[39-42],早期引流管移除,严格血糖管理及合理应用生长抑素类似物对减少并发症具有积极作用[41-51]表1)。

未来需进一步探索多风险因素的协同作用机制,特别是细菌污染与胰酶激活的相互作用;整合术前影像组学、生物学标志物及术中参数,构建基于多组学数据的动态POPF预测模型;开展新型生物材料(如水凝胶片、促纤维化材料)的多中心随机对照试验,验证其在高危人群中的作用;建立基于引流液微生物监测的靶向抗生素干预策略。通过上述多维度研究,构建更为完善的个体化防治体系,降低POPF发生率,改善患者预后。
