多囊卵巢综合征患者血清SerpinB1、microRNA-137检测的临床意义

多囊卵巢综合征患者血清SerpinB1、microRNA-137检测的临床意义
Clinical significance of serum SerpinB1 and microRNA-137 detection in patients with polycystic ovary syndrome
目的 探究多囊卵巢综合征(PCOS)患者血清丝氨酸蛋白酶抑制剂B1(SerpinB1)、microRNA-137(miR-137)检测的临床意义。 方法 选取2023年9月—2024年12月徐州医科大学附属宿迁医院150例PCOS患者作为PCOS组,另选取同期该院100例健康体检的女性为对照组。比较两组一般资料及血清SerpinB1水平、miR-137表达,采用多因素一般Logistic回归模型分析PCOS的影响因素并构建列线图,绘制受试者工作特征(ROC)曲线分析血清SerpinB1、miR-137对PCOS的预测价值。 结果 PCOS组与对照组年龄、体质量指数、促卵泡生成素(FSH)比较,差异均无统计学意义(P >0.05)。PCOS组催乳素(PRL)、促黄体生成素(LH)、总胆固醇(TC)、甘油三酯(TG)均高于对照组(P < 0.05)。PCOS组SerpinB1水平高于对照组,miR-137基因相对表达量低于对照组(P < 0.05)。多因素一般Logistic回归分析结果表明,PRL水平高[O^R =11.900(95% CI:1.441,98.252)]、LH水平高[O^R =23.509(95% CI:1.908,289.627)]、TC水平高[O^R =5.889(95% CI:1.069,32.448)]、TG水平高[O^R =8.785(95% CI:1.267,60.921)]、SerpinB1水平高[O^R =11.480(95% CI:1.627,80.987)]和miR-137水平低[O^R =0.164(95% CI:0.041,0.650)]是PCOS的危险因素(P <0.05)。ROC曲线分析结果表明,SerpinB1、miR-137联合预测的曲线下面积为0.987(95% CI:0.970,1.000),敏感性为98.0%(95% CI:0.943,0.996),特异性为98.0%(95% CI:0.930,0.998)。 结论 血清SerpinB1、miR-137对PCOS有良好的预测价值。
Objective To investigate the clinical significance of detecting serum serine protease inhibitor B1 (SerpinB1) and microRNA-137 (miR-137) in patients with polycystic ovary syndrome (PCOS). Methods A total of 150 PCOS patients admitted to Suqian Hospital Affiliated to Xuzhou Medical University from September 2023 to December 2024 were enrolled as the PCOS group, and 100 healthy women undergoing physical examinations during the same period were selected as the control group. General clinical data and serum levels of SerpinB1 and miR-137 were compared between the two groups. Logistic regression analysis was performed to identify the influencing factors of PCOS, and a nomogram was constructed. Receiver operating characteristic (ROC) curves were generated to assess the predictive value of serum SerpinB1 and miR-137 for PCOS. Results There were no statistically significant differences between the PCOS and control groups in terms of age, BMI, and follicle-stimulating hormone (FSH) levels (P > 0.05). Serum levels of prolactin (PRL), luteinizing hormone (LH), total cholesterol (TC), and triglycerides (TG) were significantly higher in the PCOS group compared with the control group (P < 0.05). Serum SerpinB1 levels were significantly elevated, while miR-137 levels were significantly decreased in the PCOS group compared to the control group (P < 0.05). Logistic analysis indicated that high PRL levels [O^R = 11.900 (95% CI: 1.441, 98.252) ], high LH levels [O^R = 23.509 (95% CI: 1.908, 289.627) ], high TC levels [O^R = 5.889 (95% CI: 1.069, 32.448) ], high TG levels [O^R = 8.785 (95% CI: 1.267, 60.921) ], high SerpinB1 levels [O^R = 11.480 (95% CI: 1.627, 80.987) ], and low miR-137 levels [O^R = 0.164 (95% CI: 0.041, 0.650) ] were risk factors for PCOS (P < 0.05). ROC results demonstrated that the combined prediction of SerpinB1 and miR-137 achieved an AUC value of 0.987(95% CI:0.970, 1.000), with a sensitivity of 98.0% (95% CI: 0.943, 0.996) and a specificity of 98.0% (95% CI: 0.930, 0.998). Conclusion Serum SerpinB1 and miR-137 have good predictive value for PCOS.
