摘要

目的 探讨术前规范化药物治疗对鼻内镜下慢性鼻窦炎(CRS)伴鼻息肉(CRSwNP)患者术后鼻黏膜重塑的影响。 方法 选取2022年1月-2023年6月该院收治的CRSwNP患者80例,根据随机数表法将患者分为观察组(n = 40)和对照组(n = 40),对照组给予功能性鼻内镜手术治疗,观察组在对照组的基础上给予术前规范化药物综合治疗。比较两组患者鼻腔黏液纤毛清除功能、鼻功能和鼻黏膜重塑变化;采用广义估计方程(GEE)模型,分析其改善情况;采用多元线性回归模型,分析其与各评分之间的关系;采用双重差分模型,分析治疗效果。 结果 两组患者组内不同时点(治疗前、治疗后3个月和治疗后6个月)的糖精清除时间、鼻黏液纤毛清除率、黏液纤毛清除速率、鼻腔容积(NCV)、鼻腔最小横截面积(NMCA)、最小横截面积距前鼻孔距离(DCAN)、嗜酸性粒细胞(EOS)、转化生长因子β1(TGF-β1)和血管内皮细胞生长因子(VEGF)水平比较,差异均有统计学意义(F = 124.36,P = 0.000;F = 113.58,P = 0.000;F = 98.67,P = 0.000;F = 87.37,P = 0.000;F = 107.26,P = 0.000;F = 77.45,P = 0.000;F = 3.80,P = 0.008;F = 2.56,P = 0.012;F = 2.13,P = 0.015);观察组治疗后3和6个月糖精清除时间较对照组短,鼻黏液纤毛清除率较对照组高,黏液纤毛清除速率较对照组快,NCV和NMCA较对照组大,DCAN较对照组短,EOS、TGF-β1和VEGF水平较对照组低,差异均有统计学意义(F = 44.36,P = 0.000;F = 38.23,P = 0.000;F = 30.44,P = 0.000;F = 27.33,P = 0.000;F = 36.11,P = 0.000;F = 22.00,P = 0.000;F = 2.44,P = 0.014;F = 1.89,P = 0.017;F = 1.75,P = 0.021);两组患者糖精清除时间、鼻黏液纤毛清除率、黏液纤毛清除速率、NCV、NMCA、DCAN、EOS、TGF-β1和VEGF水平变化趋势比较,差异均有统计学意义(F = 18.33,P = 0.000;F = 15.78,P = 0.000;F = 13.00,P = 0.000;F = 8.17,P = 0.002;F = 10.23,P = 0.000;F = 4.34,P = 0.004;F = 2.89,P = 0.013;F = 1.67,P = 0.022;F = 1.43,P = 0.031)。GEE分析结果显示,观察组鼻黏液纤毛清除率、黏液纤毛清除速率、NMCA、NCV、DCAN和糖精清除时间的改善情况明显优于对照组,差异均有统计学意义(P < 0.05);治疗后6个月,两组患者Lund-Kennedy评分、Lund-Mackay评分和鼻腔鼻窦结局测试-20(SNOT-20)评分明显低于术前,且观察组明显低于对照组,差异均有统计学意义(P < 0.05);多元线性回归模型分析结果显示,糖精清除时间、DCAN、EOS、TGF-β1、VEGF水平、鼻黏液纤毛清除率、黏液纤毛清除速率、NCV和NMCA与Lund-Kennedy评分、Lund-Mackay评分和SNOT-20量表评分密切相关(P < 0.05)。双重差分模型结果显示,观察组Lund-Kennedy评分、Lund-Mackay评分和SNOT-20量表评分的改善情况明显优于对照组。观察组总有效率为92.50%,明显高于对照组的75.00%,差异均有统计学意义(P < 0.05)。 结论 在鼻内镜术前采用规范化药物治疗,可以有效地促进CRSwNP患者鼻黏膜修复,抑制术后鼻腔重塑,且疗效较好,值得临床推广应用。

Abstract

Objective To explore the impact of preoperative standardized drug therapy on postoperative nasal mucosal remodeling in chronic rhinosinusitis (CRS) with nasal polyps (CRSwNP) patients under nasal endoscopy. Methods 80 patients with CRSwNP from January 2022 to June 2023 were selected as the research subjects. They were randomly divided into an observation group (n = 40) and a control group (n = 40) according to the random number table method. The control group received functional endoscopic surgery, while the observation group received preoperative standardized drug comprehensive treatment on the basis of the control group. The changes in mucociliary clearance function, nasal function, and nasal mucosal remodeling between the two groups of patients were compared. The Generalized Estimation Equation (GEE) model was used to analyze its improvement situation; The multivariate linear regression analysis was used to analyze its correlation with various scores; The Difference-in-difference model was used to analyze the treatment effect of patients. Results The saccharin clearance time, nasal mucociliary clearance rate, mucociliary clearance rate, nasal cavity volume (NCV), nasal minimum cross-sectional area (NMCA), distance from the minimum cross-sectional area to the anterior nostril (DCAN), eosinophil (EOS), transforming growth factor-β1 (TGF-β1) and vascular endothelial growth factor (VEGF) level at different time points within the two groups of patients (before treatment, 3 months after treatment, and 6 months after treatment), the analysis of variance using repeated measurements showed that there were statistically significant differences (F = 124.36, P = 0.000; F = 113.58, P = 0.000; F = 98.67, P = 0.000; F = 87.37, P = 0.000; F = 107.26, P = 0.000; F = 77.45, P = 0.000; F = 3.80, P = 0.008; F = 2.56, P = 0.012; F = 2.13, P = 0.015); After 3 and 6 months of treatment, the observation group had a shorter saccharin clearance time, a higher nasal mucociliary clearance rate, a faster mucociliary clearance rate, larger NCV and NMCA, shorter DCAN, and lower levels of EOS, TGF -β1, and VEGF compared to the control group. The differences were statistically significant (F = 44.36, P = 0.000; F = 38.23, P = 0.000; F = 30.44, P = 0.000; F = 27.33, P = 0.000; F = 36.11, P = 0.000; F = 22.00, P = 0.000; F = 2.44, P = 0.014; F = 1.89, P = 0.017; F = 1.75, P = 0.021). There were statistically significant differences in the trends of saccharin clearance time, nasal mucociliary clearance rate, mucociliary clearance rate, NCV, NMCA, DCAN, EOS, TGF-β1, and VEGF level between the two groups (F = 18.33, P = 0.000; F = 15.78, P = 0.000; F = 13.00, P = 0.000; F = 8.17, P = 0.002; F = 10.23, P = 0.000; F = 4.34, P = 0.004; F = 2.89, P = 0.013; F = 1.67, P = 0.022; F = 1.43, P = 0.031). The GEE analysis results showed that the nasal mucociliary clearance rate, NMCA, mucociliary clearance rate, NCV, DCAN, saccharin clearance time were better than those of the control group (P < 0.05); After 6 months treatment, the Lund-Kennedy score, Lund-Mackay score, and sino-nasal outcome test-20 (SNOT-20) score of both groups decreased compared with before treatment, and the observation group was lower than that of the control group (P < 0.05); The results of multivariate linear regression analysis showed that the saccharin clearance time, DCAN, EOS, TGF-β1, VEGF level, and the nasal mucociliary clearance rate, mucociliary clearance rate, NCV, and NMCA were closely related to Lund-Kennedy score, Lund-Mackay score, and SNOT-20 score, respectively (P < 0.05); The results of the Difference-in-difference model showed that the improvement of Lund-Kennedy score, Lund-Mackay score, and SNOT-20 score in the observation group were better than those in the control group. After treatment, the total effective rate of the observation group (92.50%) was higher than that of the control group (75.00%), the difference was statistically significant (P < 0.05). Conclusion Standardized drug therapy before nasal endoscopy can effectively promote nasal mucosal repair in CRSwNP patients, inhibit postoperative nasal remodeling, and has good therapeutic effects, which is worthy of clinical application.

慢性鼻窦炎(chronic rhinosinusitis,CRS)是一种常见且复杂的鼻腔和鼻窦慢性化脓性炎症。主要表现为:黄脓涕、头痛、鼻塞和嗅觉减退等症状。CRS病因多样,主要与病原微生物感染、鼻内解剖结构异常、免疫缺陷和黏液纤毛有关,但发病机制尚未完全明确[1-2]。CRS根据是否合并鼻息肉,可分为CRS伴鼻息肉(chronic rhinosinusitis with nasal polyps,CRSwNP)和CRS不伴鼻息肉(chronic rhinosinusitis without nasal polyps,CRSsNP)两种类型。其中,CRSwNP的临床表现更为严重,治疗难度更大,且发病率逐年上升[3-4]。目前,CRS常采用功能性鼻内镜鼻窦手术进行治疗,其能够解除阻塞,清除鼻腔和鼻窦的不可逆病变,恢复鼻窦引流、鼻腔和鼻窦黏膜的正常功能[5-6]。但相关研究[7-8]指出,单纯的手术治疗难以根治CRSwNP,术后清除效果和预后不理想,且复发率较高。近年来,术前规范化药物干预(如:糖皮质激素和白三烯受体拮抗剂等),通过抑制炎症级联反应和调节免疫微环境,对黏膜修复的调控作用逐渐受到关注[9]。但临床关于术前规范化药物干预调控重塑进程的系统性研究较少见。因此,如何准确地评估CRSwNP患者的手术疗效,并指导围手术期治疗,以提高整体疗效,降低复发风险,成为临床研究的重点。本研究旨在分析术前规范化药物治疗对鼻内镜下CRSwNP患者术后鼻黏膜重塑的影响,以期为CRSwNP的治疗提供理论依据。

1 资料与方法

1.1 一般资料

选取2022年1月-2023年6月本院收治的CRSwNP患者80例。采用随机数表法,将患者分为观察组和对照组,各40例。对照组给予功能性鼻内镜手术治疗,观察组在对照组的基础上给予术前规范化药物综合治疗。两组患者一般资料比较,差异无统计学意义(P > 0.05),具有可比性。见表1

纳入标准:内镜检查提示窦口鼻道复合体、筛区、嗅裂区黏膜肿胀病变或出现黏脓性分泌物;符合CRSwNP的诊断标准;有手术适应证;依从性好,临床资料完整。排除标准:存在精神疾病或凝血功能障碍者;免疫功能低下或免疫缺陷者;合并良恶性肿瘤或真菌性鼻窦炎者。本研究患者及家属知情同意,并经本院医学伦理委员会审核批准,伦理批件号:2021-146-01。

1.2 方法

1.2.1 对照组

实施功能性鼻内镜手术。全身麻醉后,进行Messerklinger式手术治疗,切除息肉组织,并根据患者病情切除钩突,选择适宜的开放窦腔方法。术后辅以常规抗生素静脉滴注,以及3~5 d的激素治疗。鼻腔填塞物移除后,用盐水冲洗鼻腔,每天2至3次,持续3~6个月,部分患者需持续冲洗。出院后,嘱患者每天清晨口服20~30 mg泼尼松,每5天递减,并口服大环内酯类抗生素和黏液促排剂3个月。同时,常规使用丙酸氟替卡松鼻喷剂3~6个月。

1.2.2 观察组

在对照组治疗基础上增加术前规范化药物综合治疗。具体措施包括:口服罗红霉素分散片150 mg,每天2次,连用7 d;晨起使用丙酸氟替卡松鼻喷剂于每侧鼻腔喷2次,连用7 d;晨起顿服泼尼松片0.5 mg/kg,连用7 d;服用桉柠蒎肠溶软胶囊1粒和孟鲁司特钠咀嚼片1片,每天3次,持续7 d。

1.3 观察指标

1.3.1 鼻功能

于治疗前、治疗后3个月和治疗后6个月,采用前鼻压测量器和鼻声反射仪,检测鼻腔容积(nasal cavity volume,NCV)、最小横截面至前鼻孔距离(distance from the minimum cross-sectional area to the anterior nostril,DCAN)和鼻腔最小横截面积(nasal minimum cross-sectional area,NMCA);采用糖精实验,检测糖精清除时间、鼻黏液纤毛清除率和黏液纤毛清除速率。所有操作均严格按照说明书进行。

1.3.2 鼻黏膜重塑情况

于治疗前、治疗后3个月和治疗后6个月,采用酶联免疫吸附试验,检测血清转化生长因子-β1(transforming growth factor-β1,TGF-β1)、嗜酸性粒细胞(eosinophil,EOS)和血管内皮生长因子(vascular endothelial growth factor,VEGF)水平。所有操作均严格按照说明书进行。

1.3.3 症状严重程度

于治疗前和治疗后6个月,采用Lund-Kennedy评分评估鼻黏膜形态,分数越高,说明症状越严重。采用Lund-Mackay评分评估症状严重程度,此评分共设置6项,每项最高计分12分,双侧判定为24分,分数越高,代表症状越严重。

1.3.4 生活质量

于治疗前和治疗后6个月,采用鼻腔鼻窦结局测试-20(sino-nasal outcome test-20,SNOT-20)量表评分评估患者生活质量。此量表共设置20个条目,总分为0~5分,分数越高,表明生活质量越差。

1.3.5 临床疗效

显效:临床症状均消失;有效:症状明显改善,存在部分改善;无效:患者治疗后病情未出现好转。总有效率(%) = (显效+有效)/总例数×100.00%。

1.4 统计学方法

采用SPSS 22.0统计学软件分析数据。计数资料以例或百分率(%)表示,比较采用χ2检验;计量资料以均数±标准差(x¯±s)表示,组间比较,采用独立样本t检验,重复测量数据比较,采用重复测量设计方差分析。采用多元线性回归模型和双重差分模型分析影响因素。P < 0.05为差异有统计学意义。

2 结果

2.1 两组患者临床疗效比较

治疗后,观察组20例显效,17例有效,3例无效,总有效率为92.50%,对照组17例显效,13例有效,10例无效,总有效率为75.00%,观察组总有效率明显高于对照组,差异有统计学意义(χ2 = 4.50,P = 0.034)。见表2

2.2 两组患者治疗前后鼻功能比较

2.3 两组患者鼻黏膜重塑情况比较

两组患者组内不同时点(治疗前、治疗后3个月和治疗后6个月)的EOS、TGF-β1和VEGF水平比较,差异均有统计学意义(F = 3.80,P = 0.008;F = 2.56,P = 0.012;F = 2.13,P = 0.015);观察组治疗后3和6个月EOS、TGF-β1和VEGF水平明显低于对照组,差异均有统计学意义(F = 2.44,P = 0.014;F = 1.89,P = 0.017;F = 1.75,P = 0.021);两组患者EOS、TGF-β1和VEGF水平的变化趋势比较,差异均有统计学意义(F = 2.89,P = 0.013;F = 1.67,P = 0.022;F = 1.43,P = 0.031)。见图2表4

2.4 鼻黏液纤毛清除情况和鼻功能改善情况

采用广义估计方程(generalized estimation equation,GEE),评价两组患者治疗前后鼻功能情况。在校准两组患者年龄、体重指数、病程和饮酒等一般资料的基础上,将患者治疗前后不同时点的鼻黏液纤毛清除情况和鼻功能指标纳入到GEE模型。结果显示:观察组糖精清除时间、黏液纤毛清除速率、NCV、NMCA、鼻黏液纤毛清除率和DCAN的改善情况明显优于对照组,差异均有统计学意义(P < 0.05)。见表5

2.5 两组患者治疗前后症状评分和生活质量评分比较

两组患者治疗后6个月Lund-Kennedy评分、Lund-Mackay评分和SNOT-20量表评分明显低于治疗前,差异均有统计学意义(P < 0.05);观察组治疗后6个月Lund-Kennedy评分、Lund-Mackay评分和SNOT-20量表评分明显低于对照组,差异均有统计学意义(P < 0.05)。见表6

2.6 双重差分模型分析

以Lund-Kennedy评分、Lund-Mackay评分和SNOT-20量表评分为因变量,以组别与治疗前后,以及组别与治疗前后的交互项为自变量,将性别、年龄和病程作为协变量,纳入双重差分模型进行回归分析。结果显示:Lund-Kennedy评分、Lund-Mackay评分和SNOT-20量表评分治疗前后的系数为-2.221、-0.553和-5.442(P < 0.01),治疗后比治疗前的各评分分别下降2.221、0.553和5.442分;组别×治疗前后的系数对Lund-Kennedy评分、Lund-Mackay评分和SNOT-20量表评分均产生明显影响(B分别为-1.503、-0.312和-6.439,P < 0.01),观察组的Lund-Kennedy评分、Lund-Mackay评分和SNOT-20量表评分在治疗后下降的幅度比对照组分别多1.503、0.312和6.439分。见表7

2.7 鼻黏液纤毛清除和鼻功能与各评分的关联性

将鼻黏液纤毛清除和鼻功能纳入多元线性回归模型进行分析,逐步排除存在共线性的混杂因素,最终校正年龄和性别等协变量后,结果显示:糖精清除时间、NCV、黏液纤毛清除速率、NMCA、鼻黏液纤毛清除率、DCAN、EOS、TGF-β1和VEGF水平与Lund-Kennedy评分、Lund-Mackay评分和SNOT-20量表评分之间密切相关(P < 0.05)。见表8

2.8 典型病例

患者 男,32岁。规范化药物治疗前,CT提示双侧筛窦、上颌窦黏膜明显增厚( > 5 mm),窦腔内可见不均匀软组织密度影,鼻道内见息肉样肿块填充,鼻窦自然开口狭窄。治疗后6个月,CT提示双侧筛窦、上颌窦黏膜增厚较前明显减轻( < 3 mm),窦腔内软组织密度影缩小(鼻息肉退缩),鼻窦自然开口扩大。见图3

3 讨论

3.1 术前规范化药物治疗对鼻内镜下CRSwNP患者鼻功能的影响

CRSwNP是一种发生于鼻窦黏膜的慢性炎症疾病。大多是由于急性鼻窦炎未得到及时或彻底治疗所导致[10-11]。鼻内镜手术虽然可以减轻鼻窦炎症状,但是术后并发症相对较多,而且容易复发,往往需要给予药物治疗[12-13]。目前,CRSwNP的术前规范化药物治疗方案主要包括:糖皮质激素(布地奈德和糠酸莫米松)、白三烯受体拮抗剂和抗组胺药等。本研究中,采用的术前综合用药方案以糖皮质激素为主。其中,丙酸氟替卡松鼻喷剂和泼尼松片属于糖皮质激素类药物,能够抑制Th2型炎症反应,减少EOS浸润和细胞因子分泌,可直接缓解黏膜水肿和息肉形成,具有强效抗炎和抗过敏作用。罗红霉素分散片属于大环内酯类抗生素,通过干扰细菌蛋白质合成,可抑制敏感菌生长,从而抑制TGF-β1诱导的纤维化进程,并改善黏液纤毛清除功能。孟鲁司特钠咀嚼片属于白三烯受体拮抗剂,其通过竞争性降低白三烯与受体结合,减少血管通透性和EOS趋化。桉柠蒎肠溶软胶囊是黏液溶解性祛痰药,用于稀释痰液,促进排痰。本研究通过术前规范化药物,旨在多靶点阻断Th2型炎症通路,协同减少EOS趋化和炎性介质释放,为手术创造更优的解剖条件(减小息肉体积,改善窦口引流)。炎症反应与鼻腔黏液纤毛清除功能下降密切相关,鼻内镜手术能够控制鼻腔炎症反应,有效地促进黏膜纤毛功能改善。因此,通过对80例CRSwNP患者进行研究,结果显示:治疗后6个月,两组患者鼻腔黏液纤毛清除功能、鼻功能和鼻黏膜重塑水平均得到改善,且观察组改善幅度大于对照组。这提示:对于CRSwNP患者,在鼻内镜术前使用规范化药物治疗,可修复鼻腔黏膜,恢复鼻黏膜功能和黏膜纤毛功能,减少分泌物,促进鼻腔通气。

3.2 CRSwNP患者鼻功能的改善与Lund-Kennedy评分、Lund-Mackay评分和SNOT-20量表评分的关系

TGF-β1、VEGF和EOS是评估鼻黏膜重塑的重要指标。TGF-β1能够抑制炎症细胞的增殖,但同时,对纤维化和血管生成具有促进作用,从而引发结构重塑和细胞生长;VEGF对鼻黏膜上皮细胞生长具有积极作用;EOS通过促进Th2细胞因子的释放和迁移,促进鼻腔黏膜炎症反应,导致鼻窦或窦口鼻道复合体阻塞[14-16]。以往有研究[17-18]表明,在CRSwNP鼻内镜手术患者中,糠酸莫米松对鼻黏膜修复效果较好,糖皮质激素支架置入联合鼻内镜手术,可缓解患者鼻部症状。另有研究[19]证实,根治性鼻内镜手术可降低鼻部VAS评分和Lund-Kennedy评分。本研究也显示,两组患者治疗后3和6个月EOS、TGF-β1和VEGF水平均有下降,且时间、组间和交互效应均存在差异,这提示:术前规范化药物治疗,能抑制术后鼻黏膜重塑,使得术后鼻息肉复发风险降低。考虑原因为:术前规范化药物治疗,通过增强抗炎效果,降低了EOS水平,提高了上皮细胞对凋亡刺激的抵抗力,减少了继发性纤维化,从而有效地抑制了鼻黏膜重塑过程。进一步通过多元线性回归分析结果表明:糖精清除时间、DCAN、EOS、TGF-β1、VEGF水平、鼻黏液纤毛清除率、黏液纤毛清除速率、NCV和NMCA均与Lund-Kennedy评分、Lund-Mackay评分和SNOT-20量表评分密切相关。

3.3 GEE分析和双重差分模型进一步分析术前规范化药物治疗的优势

本研究中,GEE分析结果进一步表明,观察组糖精清除时间、NCV、鼻黏液纤毛清除率、NMCA、黏液纤毛清除速率和DCAN的改善情况均优于对照组。这说明:在鼻内镜术前采用规范化药物治疗CRSwNP,能更好地改善患者的鼻功能,提高生活质量。Lund-Kennedy评分、Lund-Mackay评分和SNOT-20量表评分均是评估CRSwNP患者治疗效果的重要指标。本研究结果表明,治疗后6个月,两组患者Lund-Kennedy评分、Lund-Mackay评分和SNOT-20量表评分均降低,且观察组低于对照组。随后采用双重差分模型也证实了这一点。且治疗后观察组总有效率较高,这也进一步证明了其临床应用的可行性和有效性。该方案突破了传统术后干预的局限性,通过术前优化黏膜微环境,降低了手术难度,减少了术中出血,通过抑制纤维化和血管生成,改善黏液纤毛清除功能,降低了炎症负荷和复发风险,为CRSwNP的精准治疗提供了新的选择。

3.4 本研究的具局限性

本研究纳入的样本量较少,使得研究结果有一定程度的偏倚;未将EOS型与非EOS型进行亚组分析,可能掩盖个体化治疗差异;术后随访仅6个月,可能低估了长期复发风险。下一步需扩大样本量,对患者进行亚组分析,并延长随访时间,进行更深入的探究。

综上所述,在鼻内镜术前采用规范化药物治疗CRSwNP,临床疗效好,在促进患者鼻功能恢复的同时,还加速了鼻黏膜损伤修复,且安全性较高,患者预后好。
