摘要

目的 探讨以左膈下静脉为解剖标志,优先解剖肾上腺中央静脉,在后腹腔镜左肾上腺切除术(RLLA)中的可行性。 方法 回顾性分析2021年1月-2023年12月于该院行RLLA的116例患者的临床资料,根据手术方式的不同,将患者分为实验组(以左膈下静脉为解剖标志,并优先解剖肾上腺中央静脉的RLLA)和对照组[传统“三层面法”解剖性后腹腔镜肾上腺切除术(RLA)],各58例。比较两组患者术中及术后24 h临床指标、应激指标、免疫功能和并发症。 结果 两组患者顺利完成RLLA,且无1例中转开放手术。实验组手术时间、找到中央静脉时间、引流管留置时间和术后住院时间明显短于对照组,术中出血量和术后引流量明显少于对照组,差异均有统计学意义(P < 0.05);两组患者术后胃肠功能恢复时间比较,差异无统计学意义(P > 0.05)。两组患者术前炎症因子、应激指标和免疫功能指标比较,差异均无统计学意义(P > 0.05);两组患者术后24 h的白细胞(WBC)、C反应蛋白(CRP)和白细胞介素-6(IL-6)明显高于术前,且实验组明显低于对照组,差异均有统计学意义(P < 0.05)。两组患者术后24 h的丙二醛(MDA)明显高于术前,超氧化物歧化酶(SOD)和谷胱甘肽过氧化物酶(GSH-Px)明显低于术前,且实验组MDA明显低于对照组,SOD和GSH-Px明显高于对照组(P < 0.05)。实验组术后24 h的CD4+和CD4+/CD8+明显低于术前,且实验组术后24 h的CD3+、CD4+和CD4+/CD8+明显高于对照组,差异均有统计学意义(P < 0.05);两组患者术后24 h的CD8+比较,差异无统计学意义(P > 0.05)。实验组发生术中血压波动率明显低于对照组,差异有统计学意义(P < 0.05);两组患者均未发生腹膜损伤、肾蒂血管损伤和腹膜后血肿。 结论 RLLA中以左膈下静脉作为解剖标志,优先解剖肾上腺中央静脉是可行的,可以改善手术指标,减轻应激反应和免疫应答,具有临床推广应用价值。

Abstract

Objective To explore the feasibility of using the left inferior phrenic vein as an anatomical landmark and prioritizing the dissection of the central adrenal vein in retroperitoneal laparoscopic left adrenalectomy (RLLA). Methods 116 patients who had RLLA carried out in our hospital between January 2021 and December 2023 were chosen. They were separated into the experimental group (RLLA with the left inferior phrenic vein as an anatomical landmark and prioritizing the dissection of the central adrenal vein) and the control group [conventional three-layer method anatomic retroperitoneal laparoscopic adrenalectomy (RLA)], with 58 cases in each group. The clinical data of the two groups were compared, and the intraoperative and postoperative 24 h clinical indicators, stress indicators, immune function, and complications were evaluated. Results RLLA was successfully completed in two groups, and no cases were converted to open surgery. The operation time, time for searching the central vein, retention time of the drainage tube and postoperative hospital stay in the experimental group were significantly shorter than those in the control group. The intraoperative blood loss and postoperative drainage volume were significantly less than those in the control group, the differences were all statistically significant (P < 0.05); There was no statistically significant difference in the recovery time of gastrointestinal function between the two groups of patients after surgery (P > 0.05). There were no statistically significant differences in preoperative inflammatory factors, stress indicators and immune function indicators between the two groups of patients (P > 0.05). The white blood cell (WBC), C-reaction protein (CRP) and interleukin-6 (IL-6) of the two groups of patients 24 hours after the operation were significantly higher than those before the operation, but the experimental group was significantly lower than the control group, the differences were statistically significant (P < 0.05). The malondialdehyde (MDA) of the two groups of patients 24 hours after the operation was significantly higher than that before the operation, while the superoxide dismutase (SOD) and glutathione peroxidase (GSH-Px) were significantly lower than those before the operation, and the MDA in the experimental group was significantly lower than that in the control group, and the SOD and GSH-Px were significantly higher than those in the control group (P < 0.05). The levels of CD4+ and CD4+/CD8+ in experimental groups of patients 24 hours after the operation were significantly decreased than those before the operation, and the levels of CD3+, CD4+ and CD4+/CD8+ in the experimental group were significantly higher than those in the control group. The differences were statistically significant (P < 0.05). There was no statistically significant difference in CD8+ between the two groups of patients 24 hours after surgery (P > 0.05). The number of cases with intraoperative blood pressure fluctuations in the experimental group was significantly lower than that in the control group, the difference was statistically significant (P < 0.05). Neither peritoneal injury, renal pedicle vascular injury or retroperitoneal hematoma occurred during or after the operation in the two groups of patients. Conclusion In RLLA, using the left inferior phrenic vein as an anatomical landmark and prioritizing the dissection of the central adrenal vein is feasible, which can improve surgical indicators, reduce stress response and immune response, and have clinical application value.

肾上腺是人体重要的内分泌器官之一。当其发生病变,会出现高血压和低血钾等一系列临床症状。肾上腺切除术是治疗肾上腺异常的最终方法。上世纪80年代前,以开放式肾上腺切除术为主。但是,开腹对患者的创伤较大,且术后并发症发生率高[1-2]。随着微创技术的发展,腹腔镜下肾上腺切除术(laparoscopic adrenalectomy,LA)得到了广泛的应用[3]。LA包括经腹腔入路和经腹膜后入路。其中,以“三层面法”解剖性后腹腔镜肾上腺切除术(retroperitoneal laparoscopic adrenalectomy,RLA)最为常用[4-5]。在RLA操作中,术者需沿肾上腺周边三个潜在的低血管分布区域(腹面、背面和肾面)进行精细游离。由于肾上腺中央静脉的解剖位置特殊,在术中处理时,极易引发血压波动和严重出血等并发症。因此,肾上腺中央静脉的妥善处理,是手术成败的核心环节[6]。本研究探讨了以左膈下静脉为解剖标志,寻找中央静脉在后腹腔镜左肾上腺切除术(retroperitoneal laparoscopic left adrenalectomy,RLLA)中的可行性,以期为临床提供参考依据。

1 资料与方法

1.1 一般资料

回顾性分析2021年1月-2023年12月于该院行RLLA的116例患者的临床资料,根据手术方式的不同,将患者分为对照组(传统“三层面法”解剖性RLA)和实验组(以左膈下静脉为解剖标志,并优先解剖肾上腺中央静脉的RLLA),各58例。两组患者性别、年龄、体重指数(body mass index,BMI)、肿瘤直径和肿瘤类型等一般资料比较,差异均无统计学意义(P > 0.05),具有可比性。见表1

纳入标准:确诊为左侧肾上腺肿瘤者;于本院行RLLA,可耐受手术者;自愿接受手术,并签署知情同意书。排除标准:恶性肾上腺肿瘤者;有心、肺功能疾病或凝血功能障碍者;有精神疾病或认知功能障碍者。

1.2 方法

1.2.1 术前准备

术前,通过影像学检查确定肿瘤位置,行血常规、肝功能和内分泌检查等。根据肿瘤性质,进行围手术期准备:皮质醇腺瘤和醛固酮腺瘤患者,服用螺内酯和氯化钾,以纠正电解质紊乱;嗜铬细胞瘤患者使用酚苄明降压2周,待血压和心率接近正常水平后,可安排手术,术前3 d,给予静脉扩容治疗;髓样脂肪瘤患者对症处理(止痛和降压)。

1.2.2 实验组

采用以左膈下静脉为解剖标志,并优先解剖肾上腺中央静脉的RLLA。行气管插管麻醉,患者取右侧卧位,抬高腰桥(图1A),于腋后线12肋下的A点,使用尖刀纵行切开皮肤约2 cm。接着,用弯钳钝性分开腰背筋膜,将食指探入,分离腹膜后间隙。然后,将自制气囊(图1B)置入腹膜后间隙,充气500~800 mL,维持扩张状态1~3 min后,退出气囊,检查气囊完整性。在食指引导下,于腋中线髂棘上2 cm处置入10 mm Trocar,于B点处置入12 mm Trocar,并开通气腹,注意避免腹侧穿刺器进入腹腔。再用吸引器紧贴前腹壁,纵行分离腹膜外脂肪,找到前腹膜反折(图1C)。于腋前线肋缘下的C点,在腔镜直视下,置入5 mm Trocar,观察并进入后腹腔,清理腹膜外脂肪,显露侧锥筋膜。然后,纵行切开肾筋膜,沿腰大肌前缘钝性+锐性相结合,分离显露弓状韧带(图1D),内侧弓状韧带向中线延伸处为肾动脉,用超声刀分离出肾动脉(图1E),再沿膈肌表面向肾后上方分离,直达膈下水平。在肾动脉后上方与膈肌前缘之间寻及膈下静脉后,沿膈肌表面向近心端游离膈下静脉,距肾动脉上缘约2 cm处,可遇到分叉血管,斜向外上方朝肾上腺方向,即为肾上腺中央静脉(图1F)。分离后,使用Hem-o-lok夹,于近心端双重钳夹后,离断,再依次游离肾上腺内外侧面和下极,对于较粗的肾上腺动脉,使用Hem-o-lok夹,夹闭后,离断,对于较细的肾上腺动脉,使用超声刀慢挡离断。彻底解放肾上腺上极后,利用游离肾上腺展现出的自然悬吊作用,将游离好的标本放入特制的标本袋中,并安全取出体外。最后,在腹腔镜下观察穿刺孔无出血后,依次退出Trocar,腹膜后留置引流管1根,并缝合切口。

1.2.3 对照组

采用传统“三层面法”解剖性RLA。手术步骤参照谢程等[7]进行。

1.3 观察指标

1.3.1 手术相关情况

记录两组患者手术时间、找到中央静脉时间、术中出血量、术后引流量、引流管留置时间、术后住院时间和术后胃肠功能恢复时间。

1.3.2 炎症因子

于术前和术后24 h,采集患者静脉血,检测血白细胞(white blood cell,WBC)、C反应蛋白(C-reactive protein,CRP)和白细胞介素-6(interleukin-6,IL-6)表达水平[8]

1.3.3 应激指标

于术前和术后24 h,采集患者静脉血,检测丙二醛(malondialdehyde,MDA)、超氧化物歧化酶(superoxide dismutase,SOD)和谷胱甘肽过氧化物酶(glutathione peroxidase,GSH-Px)表达水平[9]

1.3.4 免疫功能指标

于术前和术后24 h,采用流式细胞术,检测CD3+、CD4+和CD8+水平[10]

1.3.5 并发症

统计两组患者术中和术后并发症发生情况,包括:术中血压波动、术中及术后腹膜损伤、肾蒂血管损伤和腹膜后血肿。

1.4 统计学方法

采用SPSS 26.0统计学软件分析数据。计数资料以例(%)表示,组间比较采用χ2检验;符合正态分布的计量资料以均数±标准差(x¯±s)表示,组间比较采用独立样本t检验,组内比较行配对t检验。P < 0.05为差异有统计学意义。

2 结果

2.1 两组患者手术相关情况比较

两组患者均顺利完成RLLA,且无1例中转开放手术。实验组手术时间、找到中央静脉时间、引流管留置时间和术后住院时间明显短于对照组,术中出血量和术后引流量明显少于对照组,差异均有统计学意义(P < 0.05);两组患者术后胃肠功能恢复时间比较,差异无统计学意义(P > 0.05)。见表2

2.2 两组患者炎症因子比较

两组患者术前炎症因子比较,差异无统计学意义(P > 0.05);两组患者术后24 h的WBC、CRP和IL-6明显高于术前,且实验组明显低于对照组,差异均有统计学意义(P < 0.05)。见表3

2.3 两组患者应激指标比较

两组患者术前应激指标比较,差异无统计学意义(P > 0.05);两组患者术后24 h的MDA明显高于术前,SOD和GSH-Px明显低于术前,且实验组MDA明显低于对照组,SOD和GSH-Px明显高于对照组,差异均有统计学意义(P < 0.05)。见表4

2.4 两组患者免疫功能指标比较

两组患者术前CD3+、CD4+和CD4+/CD8+比较,差异均无统计学意义(P > 0.05);实验组术后24 h的CD4+和CD4+/CD8+明显低于术前,且实验组术后24 h的CD3+、CD4+和CD4+/CD8+明显高于对照组,差异均有统计学意义(P < 0.05);两组患者术后24 h的CD8+比较,差异无统计学意义(P > 0.05)。见表5

2.5 两组患者并发症比较

实验组术中血压波动发生率为10.34%(6/58),明显低于对照组的25.86%(15/58),差异有统计学意义(χ2 = 2.20,P = 0.030);两组患者均未发生腹膜损伤、肾蒂血管损伤和腹膜后血肿。

3 讨论

3.1 以左膈下静脉为解剖标志优先解剖肾上腺中央静脉在RLLA中的优势

肾上腺是腹膜后器官,通过释放多种激素调节生理功能,当肾上腺功能紊乱时,可引发多种疾病[11]。LA是治疗肾上腺疾病的一种标准方法,包括:经腹腔和经腹膜后两种手术入路[12-13]。在切除小型肿瘤时,与经腹腔途径相比,RLA在手术时间、失血量控制、术后疼痛管理,以及恢复速度上,有明显优势,尤其对于有腹部手术史的患者而言,效果更为明显[14-15]。GROZDEV等[16]研究发现,对于肿瘤直径≤5 cm且行RLA的患者而言,其手术时间和住院时间明显缩短,术后早期并发症发生率和中转开放手术率均明显降低,这表明:RLA在该类患者中可行、有效,且安全。传统的“三层面法”解剖性RLA游离范围较大,增加了邻近脏器和血管的损伤风险[17]。为了优化这一术式,临床探索多种创新方法,如:以左膈下静脉为解剖标志,优先解剖肾上腺中央静脉,再游离肾上腺[18]。王磊等[6]研究表明,与传统的“三层面法”解剖性RLA相比,优先解剖肾上腺中央静脉并结扎,可减少在组织分离过程中的出血量,保证视野清晰,最大限度地减少因不当操作对周围正常组织的损伤,从而加快患者术后恢复速度。蔡兰辉[19]研究显示,优先精确解剖肾上腺中央静脉,可以缩短手术时间,血管精确解剖,可有效地控制出血量,降低术中血压波动率,同时,促使引流管更早拔除,加快患者术后康复进程,明显缩短住院时间。本研究结果显示,实验组手术时间、找到中央静脉时间、引流管留置时间和术后住院时间明显缩短,术中出血量和术后引流量明显减少,术中血压波动率明显降低,与以往研究[617-19]结果一致。这表明:以膈下静脉为解剖标志,优先精确解剖肾上腺中央静脉,有助于手术顺利进行,可减少术中机械性创伤,且患者术后恢复更快。此外,术后创伤区域通常会释放各种炎症因子,WBC、CRP和IL-6是评估体内炎症反应的重要指标[20]。CRP是反映手术创伤后急性期反应的独立指标,IL-6能敏感且较早地反映组织损伤的程度,IL-6和CRP均与手术创伤程度成正比。本研究结果显示,实验组术后24 h的炎症因子水平明显低于对照组。手术创伤还会引发术后的氧化应激反应,导致氧化损伤。本研究结果显示,实验组术后24 h MDA明显低于对照组,SOD和GSH-Px明显高于对照组。这表明:以左膈下静脉为解剖标志,并优先解剖肾上腺中央静脉,可以帮助患者的抗氧化系统,在术后早期就能够大量地消耗术中产生的氧自由基。外科手术会对患者机体造成一定的损伤,影响患者康复。T淋巴细胞亚群在人体免疫系统中占据着关键地位,特别是CD4+和CD8+水平,能够精确地反映机体的免疫功能状态,是预后评估的重要依据[21]。本研究结果显示,实验组术后24 h的CD3+、CD4+和CD4+/CD8+明显高于对照组,这表明:以左膈下静脉为解剖标志,并优先解剖肾上腺中央静脉,更有利于保护患者的免疫功能。

3.2 笔者寻找肾上腺中央静脉的经验

肾上腺中央静脉直径约3~5 mm,其周围血管网络密集,并被脂肪组织紧紧包裹。因此,肾上腺中央静脉的定位、暴露和分离,是手术的一大难点。手术医生不仅要具备精湛的技巧,还需拥有丰富的血管游离经验[22]。笔者寻找肾上腺中央静脉的经验如下:沿腰大肌前缘分离显露弓状韧带,以内侧弓状韧带为标志,找到左肾动脉,无需分离打开肾动脉鞘,在其上方紧贴膈肌前缘寻及左膈下静脉,然后,向肾静脉方向游离,即可在血管分叉处,顺利找到中央静脉;同时,要避免遗漏和损伤变异的肾上腺静脉,这有助于预测和避免出血[23-24]。在本研究中,所有病例的左膈下静脉与左肾上腺中央静脉之间,均交汇后汇入左肾静脉,在找到膈下静脉向肾静脉游离的过程中,需明确有无肾上腺中央静脉的变异。通过遵循这些经验,可有效地提高手术成功率和安全性。

3.3 本研究的局限性

本研究为回顾性研究,且纳入样本量较小,结果可能存在偏倚,下一步将扩大样本量,采用随机对照试验,进一步佐证本研究结论。

综上所述,在RLLA中,选择以左膈下静脉为标志,优先解剖肾上腺中央静脉,可以改善手术指标,减轻应激反应和免疫应答,具有临床推广应用价值。
