摘要

婴幼儿胃食管反流病(gastroesophageal reflux disease, GERD)是婴幼儿期常见的消化道疾病,其临床表现多样,常与生理性反流和行为表现重叠,致使诊断存在挑战。该文通过系统检索PubMed、Web of Science、中国知网和万方数据等中英文数据库,系统综述了婴幼儿GERD的流行病学特征、临床表现及其对患儿和家庭的影响,重点评价了4种主要评估工具(I⁃GERQ、GSQ⁃I/YC、I⁃GERQ⁃R和GIGER量表)的开发背景、结构特点、信效度及临床应用价值,旨在为婴幼儿GERD的早期识别、标准化评估及个体化管理提供理论依据与实践参考。该文发现I⁃GERQ⁃R具有较高的特异性、良好的信效度和跨文化适用性,是目前推荐用于婴幼儿GERD症状评估与疗效监测的有效工具,GIGER量表在全面筛查方面展现出潜力;未来研究应致力于汉化及开发中文版本婴幼儿GERD问卷并建立多维度评估体系。

Abstract

Gastroesophageal reflux disease (GERD) in infants and young children is a common digestive disorder with diverse clinical manifestations that often overlap with physiological reflux and behavioral expressions, creating diagnostic challenges. Comprehensive searches were conducted in English- and Chinese-language databases, including PubMed, Web of Science, China National Knowledge Infrastructure, and Wanfang Data, to review the epidemiological characteristics, clinical presentations, and impacts of GERD on children and their families. Particular emphasis is placed on four principal assessment tools [Infant Gastroesophageal Reflux Questionnaire (I-GERQ), GERD Symptom Questionnaire for Infants/Young Children (GSQ-I/YC), I-GERQ Revised (I-GERQ-R), and Gastrointestinal and Gastroesophageal Reflux Scale for Infants and Toddlers (GIGER)], appraising their development background, structural characteristics, reliability, validity, and clinical application value. The aim is to support early identification, standardized assessment, and individualized management of GERD in infants and young children. Evidence indicates that the I-GERQ-R shows high specificity, good reliability and validity, and cross-cultural applicability, and is currently recommended for symptom assessment and treatment-response monitoring in GERD among infants and young children, while the GIGER scale shows potential for comprehensive screening. Future research should prioritize translation and development of Chinese versions of GERD questionnaires for infants and young children, as well as the establishment of a multidimensional assessment system.

婴幼儿胃食管反流病(gastroesophageal reflux disease, GERD)是婴幼儿期常见的消化道功能障碍性疾病。北美儿科胃肠病学、肝病和营养学会与欧洲儿科胃肠病学、肝病和营养学会联合制定的儿科胃食管反流(gastroesophageal reflux, GER)实践指南中将GER定义为胃内容物进入食管,伴有或不伴有反流和/或呕吐;当反流导致令人烦恼的症状和/或并发症,如食管炎或食管狭窄时,GER被视为病理性的,称为GERD1。尽管大多数婴幼儿的GERD症状在1岁后可自行缓解,但仍有部分患儿因病情迁延而发展为慢性疾病,如食管炎2、反复呼吸道感染3甚至生长发育迟缓4。另有研究显示,GERD与牛奶蛋白过敏(cow's milk protein allergy, CMPA)的临床表现高度重叠,约46%的GERD患儿合并CMPA5-6,这增加了临床诊断与治疗的复杂性。目前,国内对婴幼儿GERD的诊断主要依赖临床症状与客观检查,但这一模式常导致诊断过度或不足,且疗效评估缺乏统一标准。因此,系统回顾并评估现有评估工具的特点与应用价值,对推进该疾病的规范化管理至关重要。

1 文献检索策略

通过计算机检索以下数据库:中国知网、万方、SinoMed、PubMed、Embase及Web of Science。检索时限为自建库起至2025年8月。同时辅以手工检索,以主题词、自由词和布尔逻辑运算符结合为检索策略。中文检索词及检索策略:以知网为例,检索式为SU=(胃食管反流+胃食管反流病+GERD+GER)AND SU=(婴幼儿+小儿+儿童+早产儿+婴儿)AND SU=(评估+工具+问卷+量表)。

文献纳入标准:涉及婴幼儿GER或GERD的文章,包括指南、随机对照研究、类实验研究、综述、Meta分析与系统评价。排除标准:信息不全或重复发表的文献;案例报告、会议摘要、评论、计划书;非中、英文文献;无法获取全文的文献。

由2名经过循证医学培训的研究人员根据纳入与排除标准独立筛选文献并提取数据,交叉核对结果,若有异议,由第3名具备丰富经验的人员裁决。使用Endnote X9软件导入文献,2名研究人员根据题目和摘要进行初筛,阅读全文再次筛选,最终确定纳入文献。

2 婴幼儿GERD的发病机制

2.1 生理性与病理性反流

尽管指南对GER与GERD作出了明确界定,但在临床实践中,二者的区分仍是一大挑战。生理性GER通常表现为婴儿期的自限性过程,症状多在8周龄前出现且随月龄增长逐渐缓解,一般不影响生长发育14。然而,当反流频率增多和强度增加,引发喂养困难、烦躁不安等症状或黏膜损伤等并发症时,即发展为病理性GERD。因生理性与病理性反流在临床表现上呈连续变化,缺乏绝对界限,因此精准识别GERD需依赖综合评估。

2.2 婴幼儿的生理解剖结构

研究显示,约50%的婴儿每天至少发生1次GER,而GER的高发可能与婴幼儿胃肠道发育未成熟的特点密切相关7,主要包括以下机制:(1)下食道括约肌发育不成熟,压力较低,抗反流屏障功能较弱;(2)胃部呈水平位,幽门括约肌相对发达,奶液易通过松弛的贲门反流;(3)消化酶分泌不足、活性较低,胃排空延迟,进一步增加反流风险。此外,0~6月龄小婴儿胃容量较小、His角较钝、腹段食管较短、吊索纤维发育不成熟8,加之饮食以流质为主且多处于平卧位,这些因素共同导致婴儿期GERD的发生率显著升高。

2.3 与其他疾病的关联

GERD更常见于患有某些合并症的婴儿,包括早产史、神经系统损伤、膈疝修补术和呼吸系统疾病等9-11,这些因素均可能增加反流风险。付旭尧12的研究显示,支气管肺发育不良的婴儿GERD患病率增加。牛奶蛋白过敏的某些症状也与GERD的症状相似13,尤其是在有特应性症状、体征或家族史的婴儿中表现更为突出14-15

上述发病机制及共病风险凸显了婴幼儿GERD的复杂性,这也对其临床表现的多样性和诊断评估的挑战性产生了直接影响。

3 婴幼儿GERD现状

3.1 患病率与流行病学特征

生理性GER在健康婴儿中较为普遍,但发展为需要临床干预的GERD相对少见,其患病率因人群、研究设计及诊断标准的不同而异,并呈现出随年龄变化的特征及年代差异。

在年龄变化方面,多项研究证实,婴幼儿GERD的患病率随月龄增长呈显著下降趋势。新加坡一项针对亚洲婴儿的前瞻性研究发现,6周龄时GERD的患病率达26.5%,3月龄时降至7.7%,6月龄时为2.6%,12月龄时进一步降至1.1%16。Curien⁃Chotard和Jantchou17在法国进行的研究也报告了类似规律,符合诊断的婴幼儿GERD患病率在出生后第一个月达到峰值(19%),在3月龄时降至9%,6月龄时为5%,到10~12月龄时稳定在2%左右。Singendonk等18对全球数据分析的结果也呈现了这一随年龄变化的趋势,指出0~18个月的婴儿中超过25%每日出现GERD症状,随着年龄增长,GERD症状的发生率稳步降低。以上数据表明,绝大多数的婴幼儿GERD病例集中于婴儿早期,但因未进行大数据的统计,单纯的数据比较无法看出婴幼儿GERD患病率的地区差异。

此外,GERD的诊断率随着时间的推移而演变。近20年来,随着多通道腔内阻抗联合pH值监测等诊断技术的普及以及对婴儿哭闹、喂养问题等的认知深化,GERD的诊断趋于精准19。早期研究可能因诊断标准不一而高估或低估患病率,如一项美国多中心的调查显示,早产儿的GERD患病率在2.4%到29.9%的范围内20,存在较大异质性。近年来,临床指南更加强调基于典型症状和问卷的初步评估,并警示抑酸药物的过度使用17,这反映了诊疗观念的转变。因此,在解读不同时期的流行病学数据时,需充分考虑其诊断标准与时代背景的差异。

3.2 临床表现与影响

在婴幼儿中,GERD最常见的症状是进食后频繁呕吐或反流,其他症状缺乏特异性,且常与正常的婴儿行为重叠,如过度哭闹21,因此GERD的诊断与管理相对困难。婴幼儿GERD可能引起多种并发症,包括吸入性肺炎22、食管炎23、吞咽困难24等。此外,约12%的新生儿心肺事件(包括呼吸暂停、氧饱和度下降、心动过缓、明显危及生命的事件)被发现与早产儿的GER发作存在关联25-26

婴幼儿GERD不仅影响患儿自身健康,也对父母的心理状态造成显著负面影响,尤其是增加焦虑和抑郁风险。婴幼儿GERD易导致患儿呈现持续哭闹的状态,Petzoldt27的研究表明,婴幼儿的持续哭闹或肠绞痛与母亲产后抑郁的发生有关。一项针对GERD婴幼儿父母的调查研究显示,66.0%的父母患有广泛性焦虑症,63.4%患有抑郁症28。此外,多数研究表明,GERD不仅影响患儿健康,也给父母带来了沉重的心理负担29-30

鉴于GERD对患儿生长发育及家庭心理健康的双重不良影响,开发并应用可靠、有效的评估工具,以实现婴幼儿GERD的早期识别和干预,具有重要的临床价值与现实意义。

4 婴幼儿GERD的评估工具

4.1 婴儿GER问卷

婴儿胃食管反流问卷(Infant GER Questionnaire, I⁃GERQ)由Orenstein等31于1993年开发,由父母或监护人填写,用于评估婴儿GERD症状的流行率和严重性。问卷包含138个条目,涵盖了人口学信息、症状学(如反流、呼吸问题、烦躁等)及喂养习惯等多个维度,其内部一致性通过29对冗余问题进行评估,回答一致性的中位数为0.94(范围:0.80~1.00),重测信度中位数为0.88。该问卷通过其中的11个关键条目构成1个总分为25分的GERD评分,用于区分正常婴儿与GERD患儿,在截断值>7分时,阳性预测值=1.00,阴性预测值=0.98。该量表是可靠且有效的病史采集工具,能提高问诊的完整性,用于辅助临床诊断,但因其条目较多,目前在临床中较少使用。

4.2 婴儿GER问卷修订版

2006年,Kleinman等32对I⁃GERQ进行了修订,形成了I⁃GERQ修订版(I⁃GERQ Revised, I⁃GERQ⁃R)。I⁃GERQ⁃R共12个条目,包括反流频率、拒食、哭闹、打嗝、弓背(背部拱起)及呼吸暂停等症状,采用1周回忆期,总分范围为0~42分。该问卷由主要照顾者填写(通常是父母),可用于评估GERD症状的变化及治疗效果。研究验证其信效度良好,Cronbach's α为0.86~0.87,以总分≥16分为诊断临界值时,特异度高达100%,灵敏度为65%。

I⁃GERQ⁃R已在全球多个国家被广泛应用于婴幼儿GERD的症状评估与疗效评估,其显著优势在于兼具高度的特异性、良好的信度与效度、优秀的跨文化适用性以及较强的临床实用性,已获得大量研究验证,且广泛应用于临床研究的效果验证1633-40。但近年有研究显示,I⁃GERQ⁃R与儿科饮食评估工具(Pedi⁃EAT⁃10)得分存在高度相关,提示I⁃GERQ⁃R的结果可能不仅反映GERD,也可能与口咽性吞咽障碍有关41。因此,临床应用I⁃GERQ⁃R时,需注意其评分可能受到并存的口咽吞咽困难的影响,建议在存在喂养困难体征时,联合使用特异性吞咽功能评估工具进行鉴别。

4.3 年龄特异性GERD症状问卷

年龄特异性GERD症状问卷由Deal等42于2005年开发,用于评估GERD症状频率与严重程度,包括适用于1~11个月婴儿年龄特异性GERD症状问卷(GERD Symptom Questionnaire for Infants, GSQ⁃I)和1~4岁幼儿的年龄特异性GERD症状问卷(GERD Symptom Questionnaire for Young Children, GSQ⁃YC)量表,均为6个条目,由父母或监护人填写,能够区分GERD的婴幼儿和健康的婴幼儿。

GSQ⁃I评估弓背(背部拱起)、窒息/干呕、打嗝、易激惹、拒食和呕吐/反流6个方面;GSQ⁃YC则评估腹痛、打嗝/嗳气、进食窒息、吞咽困难、拒食和呕吐/反流6个方面。GSQ⁃I和GSQ⁃YC旨在评估7 d内反流症状的频率和严重程度,计算每个症状的单项评分和综合评分。综合评分>8分对诊断为GERD婴幼儿的灵敏度为85%,特异度为81.5%。该量表在临床研究中被用作评估干预措施效果的关键工具,如在一项关于后舌系带切开术对婴儿喂养及反流症状影响的随机对照试验中,研究者采用GSQ⁃I作为核心的患儿报告结局指标,结果显示,接受舌系带切开术的婴儿的多种反流症状的频率和严重程度均显著改善。这展现了GSQ⁃I在捕捉临床相关症状变化方面的良好反应度和实用性43

4.4 婴幼儿胃肠道与GER量表

婴幼儿胃肠道与GER量表(Gastrointestinal and Gastroesophageal Reflux Scale for Infants and Toddlers, GIGER)由Pados等44于2021年开发,用于评估2岁以下婴幼儿的胃肠道及GER相关症状。量表包含3个维度36个条目,各条目均采用6点Likert评分法,总分越高提示症状越严重。该工具使用父母易于理解的语言编写,Cronbach's α为0.78~0.94,并与GSQ⁃I和I⁃GERQ⁃R等现有工具相关,收敛效度良好,能够有效区分是否被诊断为GER或便秘的婴幼儿,具备已知组效度。

GIGER不能用于直接诊断GERD,但该量表更侧重于筛查广泛的胃肠道症状,其中包含与GERD高度相关的条目,为临床干预决策提供辅助参考。一项采用GIGER量表进行的前瞻性评估表明,舌系带切开术能显著缓解婴幼儿的胃肠道及GER症状,且这种改善在月龄更小或舌系带问题更严重的个体中尤为明显45。但目前该量表的诊断灵敏度与特异度尚未明确,不建议作为独立诊断工具使用。4种婴幼儿GERD评估工具比较见表1

综上所述,在4种GERD评估问卷中,I⁃GERQ⁃R是目前临床试验中最常用于疗效评估的诊断工具。该问卷通过比较干预前与干预后(如药物、饮食调整或舌系带切开术等)的得分变化,客观反映症状减轻程度,从而评估治疗效果。Kleinman等32提出I⁃GERQ⁃R可以检测到随时间的推移的有临床意义的变化。近年来有临床研究也证实,若患儿I⁃GERQ⁃R总分较干预前降低≥6分,则认为当前治疗策略有效,其为后续降阶梯治疗或计划性停药提供了依据,有助于避免长期不必要的用药;若评分降低未达6分,则提示当前治疗(尤其是抑酸治疗)可能不适用,需重新评估诊断37-38。另外,3分的差异可能代表最小重要差异,也具有一定的临床参考价值。

需强调的是,I⁃GERQ⁃R并非独立的诊断工具,而是一种症状量化与疗效监测手段,高分结果应视为启动进一步评估的信号,而非直接使用抑酸药的依据。其临床价值在于将家长主观、模糊的症状描述转化为客观量化的数据,结合病史、体格检查及其他客观检查(如多通道腔内阻抗联合pH值监测),共同指导诊断与治疗决策。

5 小结与展望

在当前婴幼儿GERD的评估与干预体系中,I⁃GERQ⁃R已在国际范围内显示出良好的症状评估与疗效监测适用性。该量表目前仍缺乏中文版本,未来可推进I⁃GERQ⁃R的汉化与文化调适工作,进一步验证其在我国婴幼儿中的信效度及诊断临界值。在此基础上,可进一步整合多模态数据源,如结合临床医生报告、器械检查结果等综合评估,构建融合多维度信息的评估模型,以提升GERD诊断与鉴别诊断的准确性。从更长远来看,应致力于建立覆盖不同发育阶段的婴幼儿GERD标准化评估体系,推动早期识别、个体化干预和全程管理,最终降低疾病整体负担。
