防治复杂性输尿管上段结石术中逃逸相关策略研究进展

防治复杂性输尿管上段结石术中逃逸相关策略研究进展

摘要

复杂性输尿管上段结石在输尿管镜钬激光碎石过程中易发生结石碎片入肾,导致一期清石率下降并增加二次手术与医疗成本。本文基于近年相关临床研究与文献证据,归纳并比较输尿管镜治疗中防治结石逃逸的策略。现有研究结果显示,器械协同与物理拦截或负压吸引等辅助手段在预防结石上移方面显示出潜在的临床价值,其中 NTrap网篮等装置的疗效表现较为突出;但相关研究多为单中心回顾性分析,仍需多中心随机对照试验进一步验证并指导个体化选择。本文综述结石逃逸防治研究进展,以供临床参考。

Abstract

During ureteroscopic holmium laser lithotripsy for complex upper ureteral calculi,stone fragments are prone to migrate into the kidney,leading to a decreased primary stone-free rate and increased need for secondary surgeries and healthcare costs.Based on recent clinical studies and literature evidence,this article summarizes and compares strategies for preventing stone escape during ureteroscopic treatment.Comprehensive analysis of existing research indicates that auxiliary measures,such as instrument coordination,physical interception,or negative pressure suction,demonstrate potential clinical value in preventing stone migration,with devices like the NTrap basket showing relatively prominent efficacy.However,most related studies are single-center retrospective analyses,and further validation through multicenter randomized controlled trials is needed to guide individualized treatment selection.This review outlines advances in the prevention and management of stone escape for clinical reference.
