摘要

目的 探讨个体化的3D可视化及混合现实(MR)技术(以下简称MR技术)在西藏地区藏族患者泌尿系统结石微创手术术前谈话中应用的可行性和有效性。方法 选取2024-2025年在西藏自治区人民医院泌尿外科拟行微创手术的藏族结石患者,随机分为实验组(n=41)与对照组(n=40)。实验组接受基于MR技术的术前谈话,对照组接受传统模式谈话。谈话语言为藏语,固定1名藏族医生进行谈话或作为翻译(固定另外1名汉族医生)。比较两组患者手术相关知识掌握度、主观满意度及医院焦虑抑郁量表评分变化。结果 两组基线资料无统计学差异(P>0.05)。实验组对手术相关知识掌握度提升幅度显著高于对照组[(29.4±6.1)分 对比 (14.5±5.0)分,P<0.001)],术后知识得分[(65.1±5.5)分 对比(49.3±4.9)分]与主观满意度评分[(4.8±0.5)分 对比(4.1±1.0)分]均显著优于对照组(P<0.001)。焦虑评分分析显示显著的时间·组别交互效应(P<0.001):实验组谈话后焦虑即显著下降并持续至出院前,而对照组谈话后出现焦虑升高。结论 在西藏地区藏族泌尿系结石患者中,应用MR技术进行术前谈话可显著提升患者对手术的认知水平与满意度,并能有效缓解传统谈话方式所致的短期焦虑,具有良好的临床应用价值。

Abstract

Objective To evaluate the feasibility and effectiveness of individualized three-dimensional (3D) visualization and mixed reality (MR) technology in preoperative conversation for Tibetan patients with urolithiasis undergoing minimally invasive surgery in Tibet. Methods Tibetan patients with urolithiasis scheduled to undergo minimally invasive surgery at the Department of Urology, Tibet Autonomous Region People's Hospital, during 2024 and 2025 were randomly assigned to an experimental group (n=41) or a control group (n=40). The experimental group received preoperative conversations based on MR technology, while the control group received traditional conversations. All conversations were conducted in Tibetan. One Tibetan physician conducted the conversations or served as an interpreter (with one Han Chinese physician also present). The two groups were compared regarding their acquisition of surgery-related knowledge, subjective satisfaction, and changes in Hospital Anxiety and Depression Scale score. Results No statistically significant differences were found in baseline data between the two groups (P>0.05). The improvement in knowledge acquisition was significantly greater in the experimental group than in the control group [(29.4±6.1) vs. (14.5±5.0), P<0.001]. Postoperative knowledge score [(65.1±5.5) vs. (49.3±4.9)] and subjective satisfaction score [(4.8±0.5) vs. (4.1±1.0)] were also significantly higher in the experimental group (P<0.001). Analysis of anxiety score revealed a significant time·group interaction effect (P<0.001): anxiety in the experimental group decreased significantly after the conversation and remained low until discharge, whereas anxiety in the control group increased after the conversation. Conclusion For Tibetan patients with urolithiasis in Tibet, the use of MR technology in preoperative conversations significantly improves patients' understanding of the surgery and their satisfaction, while effectively alleviating short-term anxiety induced by traditional conversation methods, demonstrating good clinical application value.
