1992-2021年中国睾丸癌疾病负担年龄-时期-队列模型及预测分析

摘要

目的 分析1992-2021年中国睾丸癌疾病负担变化趋势,并预测2022-2031年疾病负担趋势。方法 基于2021年全球疾病负担(GBD 2021)数据库获得1992-2021年中国和全球人群睾丸癌的发病数、死亡数、伤残调整寿命年(DALYs)及其年龄标准化率。采用年估计变化百分比(EAPC)评估年龄标准化率(ASR)的变化,使用Joinpoint回归分析对中国睾丸癌的发病率和DALYs率进行趋势分析,用年龄-时期-队列模型分析年龄、时期和出生队列对中国睾丸癌的发病率和 DALYs率的影响,用自回归积分移动平均(ARIMA)模型对2022-2031年睾丸癌发病率和 DALYs率的趋势进行预测。结果 1992-2021年中国和全球睾丸癌标准化发病率呈上升趋势,标准化死亡率和标准化DALY 率呈下降趋势,中国年估计变化百分比(EAPC)分别为2.77、-1.99、-1.95,全球EAPC分别为1.43、-0.49、-0.23。Joinpoint回归分析显示,从1992-2021年中国睾丸癌发病率整体呈上升趋势[平均年度变化百分比(AAPC):3.09],DALYs率整体呈下降趋势并趋于稳定(AAPC:-1.28)。年龄-时期-队列模型结果显示,中国睾丸癌发病率和 DALYs率随着年龄的变化出现先升高后下降趋势;发病风险随着时期变化呈上升趋势,DALYs风险随时期的推移呈先下降后上升趋势;发病风险的出生队列效应随时间变化呈先上升后下降趋势,而出生越晚的队列 DALYs风险越低。ARIMA 模型预测结果显示,2022-2031年中国睾丸癌发病率呈上升趋势,DALYs率保持平稳变化。结论 中国睾丸癌发病率呈上升趋势,DALYs率下降并趋于稳定,未来疾病负担将相应加重,因此需加强监测、筛查与早诊早治,以改善患者预后、减轻负担。

Abstract

Objective To analyze the disease burden trend of testicular cancer in China from 1992 to 2021 and to predict the trend from 2022 to 2031. Methods Based on the 2021 Global Burden of Disease(GBD2021)database,the incidence,mortality,disability-adjusted life years(DALYs),and age-standardized rates of testicular cancer among the Chinese and global populations from 1992 to 2021 were obtained.The annual estimated percentage change(EAPC)was used to assess the changes in age-standardized rates(ASR).Joinpoint regression analysis was employed to conduct trend analysis on the incidence rate and DALYs rate.The age-period-cohort(APC)model was utilized to analyze the effects of age,period,and birth cohort on the incidence rate and DALYs rate.The autoregressive integrated moving average(ARIMA)model was used to predict the trend of incidence rate and DALYs rate from 2022 to 2031. Results From 1992 to 2021,the standardized incidence rate of testicular cancer in China and the world showed an upward trend,while the standardized mortality rate and DALY rate showed a downward trend.EAPC in China was 2.77,-1.99,and -1.95,respectively,and the global EAPC was 1.43,-0.49,and -0.23,respectively.The Joinpoint regression analysis showed that from 1992 to 2021,the incidence rate in China showed an upward trend(AAPC=3.09),while the DALYs rate showed a downward trend and stabilized(AAPC= -1.28).The APC model results showed a firstly increasing trend of the incidence rate and DALYs rate in China,which then decreased with age.The risk of disease incidence increased with the change of period,while the risk of DALYs gradually decreased over time.The birth cohort effect of the onset of disease showed an increasing trend first,which then decreased over time,and the cohort with an earlier birth date had a lower risk of DALYs.The ARIMA model prediction showed an increasing incidence rate from 2022 to 2031,while the DALYs rate remained stable. Conclusion The incidence of testicular cancer in China is on the rise,while the DALYs rate has decreased and stabilized.In the future,the disease burden will increase accordingly.Therefore,it is necessary to strengthen the monitoring,screening,and early diagnosis and treatment to improve prognosis and reduce the disease burden.
