摘要
目的 探讨肌少症、超重与夜尿症的相关性,以期为夜尿症的病因识别提供依据。方法 基于2011-2018年美国国家健康与营养调查(NHANES)数据库,纳入12 711名具有完整数据的参与者,其中肌少症患者1156例,夜尿症患者2911例。应用多因素logistic回归模型逐层校正混杂因素,分析肌少症与夜尿症的关联,并进行亚组及分层分析。结果 夜尿症组肌少症患病率高于无夜尿症组(13.0%对比7.9%,P<0.001),多因素校正后,肌少症是夜尿症的独立危险因素(OR=1.592,95%CI:1.277~1.986,P<0.001),超重人群合并肌少症时夜尿症风险进一步升高(OR=1.508,95%CI:1.178~1.930,P=0.002)。肌少症合并超重组夜尿症患病率高于有单纯超重或肌少症组(χ2=42.132,P<0.001)。亚组分析显示,肌少症与夜尿症的关联存在人群异质性,在不同性别、全年龄段、非西班牙裔、高中以上学历、各类吸烟人群、少量饮酒及无基础慢性病人群中更为显著(P <0.05)。结论 肌少症是夜尿症的独立危险因素,肌少症合并超重人群夜尿患病风险进一步升高。
Abstract
Objective To investigate the independent association between sarcopenia, overweight and nocturia, so as to provide evidence-based insights for the etiology of nocturia. Methods Data from the 2011-2018 National Health and Nutrition Examination Survey (NHANES) were analyzed. A total of 12 711 participants with complete information were involved, including 1156 individuals with sarcopenia and 2911 with nocturia. Multivariable logistic regression models were used to assess the association between sarcopenia and nocturia with stepwise adjustment for potential confounding factors. Subgroup and stratified analyses were further performed to evaluate the consistency of the association across different populations. Results The prevalence of sarcopenia was significantly higher in participants with nocturia than in those without(13.0% vs. 7.9%, P<0.001). After multivariable adjustment, sarcopenia remained independently associated with nocturia(OR=1.592, 95%CI:1.277-1.986, P<0.001). Further analysis showed that overweight individuals with sarcopenia had an increased risk of nocturia(OR=1.508, 95%CI:1.178-1.930, P=0.002). Chi-square test revealed that the prevalence of nocturia was higher in participants with complicated sarcopenia and overweight than in those with only overweight or sarcopenia(χ2 = 42.132, P<0.001). Subgroup analyses revealed population heterogeneity in the association between sarcopenia and nocturia, with a more pronounced association observed among different genders, age, non-Hispanic participants, individuals with high school education or above, various smoking participants, those with light alcohol consumption, and those without chronic diseases(P<0.05). Conclusion Sarcopenia is an independent risk factor for nocturia. The coexistence of sarcopenia and overweight is associated with an increased risk of nocturia.
