改良TROPIS术对老年高位肛瘘患者远期肛门功能的影响及相关因素分析

摘要

观察改良TROPIS术治疗老年高位肛瘘的疗效，分析影响患者远期肛门功能的相关因素。选择2022年2月—2024年6月重庆市开州区人民医院收治的老年高位肛瘘患者124例，随机分为观察组和对照组，每组各62例，分别接受改良TROPIS术和传统切开挂线术。比较两组手术时间、住院时间、创面愈合时间、治愈率和复发率；比较两组术前，术后1、3、6、12个月Wexner失禁评分。根据术后12个月Wexner失禁评分，将观察组分为肛门功能下降组和无肛门功能下降组，比较两组性别、年龄、BMI、病程、肛瘘性质、合并症、手术时间等临床资料，分析影响老年高位肛瘘患者改良TROPIS术远期肛门功能的相关因素。观察组手术时间、创面愈合时间、复发率均低于对照组（P<0.05）；术后1、3、6个月观察组Wexner失禁评分均低于对照组（P<0.05），术后12个月两组Wexner失禁评分差异无统计学意义（P>0.05）；肛门功能下降组年龄、BMI、合并糖尿病占比、复杂性肛瘘占比、牵涉外括约肌占比均高于无肛门功能下降组（P<0.05）。多因素Logistic回归分析显示，BMI、合并糖尿病、复杂性肛瘘、病变牵涉外括约肌为影响老年高位肛瘘患者改良TROPIS术后肛门远期功能的危险因素（P<0.05）。改良TROPIS术治疗老年高位肛瘘效果显著，但对肛门远期功能仍有一定影响，BMI、合并糖尿病、复杂性肛瘘、病变牵涉外括约肌为影响术后肛门远期功能的危险因素。
