腹腔镜胰十二指肠切除术后胰瘘影响因素分析

摘要

探讨腹腔镜胰十二指肠切除术后胰瘘发生的相关因素。回顾性分析2017年3月—2021年4月山东第一医科大学附属省立医院肝胆外科收治的501例实施腹腔镜胰十二指肠切除术患者的临床资料，根据是否发生胰瘘分组，比较两组临床特征，利用Logistic回归分析胰瘘发生的危险因素。共有134例（26.7%）患者术后发生胰瘘。与非胰瘘组比较，胰瘘组术前行胆管引流（PTCD）病例少，术后出血和腹腔感染病例较多，住院时间较长，两组差异有统计学意义（P<0.05）。多因素Logistic回归分析发现术前行PTCD引流可降低胰瘘风险，而腹腔感染是胰瘘发生的独立危险因素（P<0.05）。胰瘘多继发于腹腔感染，术前行PTCD引流及术后控制腹腔感染对预防及治疗胰瘘具有重要作用。
