湖北省随县一起人感染布鲁氏菌病畜间流行病学调查与处置
Epidemiological investigation and treatment of a case of brucellosis infection in Suixian County, Hubei Province
目的 2025年5月,湖北省随县某休闲山庄养殖场发生1起饲养人员确诊感染布鲁氏菌病的病例,为尽快查明发病原因与感染风险因素,切断传播链条,控制扩散风险。 方法 对涉疫养殖场开展现场调查,同时对周边10 km范围内牛羊养殖场户进行应急排查,采集牛、羊血清开展布病抗体检测,对相关人员进行布病筛查,开展畜间流行病学分析与应急处置。 结果 除本次事件关联场点外,其他养殖场未发现其他异常情况或畜群流产报告。患者羊场19只羊进行全面采血检测,确诊2只为布病阳性。排查监测牛羊血清534份,布病抗体检测结果均为阴性。相关接触人员及采样人员布病筛查结果均为阴性。经分析,本次主要感染因外省风险地区贩羊车辆未经清洗消毒进入养殖区域和购入饲草,且养殖人员在无防护条件下接触羊的流产物导致。对阳性羊进行扑杀和无害化处理,并对周围环境进行彻底清洗和消毒。3个月后患病羊场存栏羊只再次进行全群检测,结果均为阴性。 结论 应强化输入风险管控,加大监测力度,筑牢养殖场生物安全屏障,加强从业人员防护培训,全面提升布病综合防控工作。
Objectives The cause and risk factors of a confirmed case of brucellosis infection among a livestock farmer at a recreational resort farm in Suixian County, Hubei Province in May 2025 were promptly investigated to interrupt the transmission chain and control the risk of spread. Methods On-site investigations were conducted at the patient's farm, along with emergent screenings of cattle and sheep farmers within a radius of 10 km. Samples of serum from cattle and sheep were collected for detecting antibody against Brucella, and screening of Brucella was conducted among relevant personnel. Epidemiological analysis of animal populations and measures of emergency treatment were implemented. Results No other abnormalities or reports of livestock abortion was found at other farms except for the site associated with this incident. 19 sheep from the patient's farm were subjected to comprehensive sampling and testing of blood, and 2 were confirmed to be positive for brucellosis. 534 samples of serum from cattle and sheep were screened and monitored, with results of detecting antibody against Brucella being negative. The results for screening brucellosis were negative for all contacts and sampling personnel. The results of analyzing etiology showed that vehicles of transporting sheep from high-risk areas in other provinces entered the farming zone and purchased forage without proper cleaning and disinfection, and personnel came into contact with the aborted products of sheep without protective conditions, resulting in human infection. Positive sheep were culled and subjected to harmless disposal, with thorough cleaning and disinfection of the surrounding environment. The flock of infected sheep in the farm underwent another whole-flock testing 3 months later, and the results were all negative. Conclusions Comprehensive prevention and control measures for Brucellosis including controlling input risk, ensuring the intensity of monitoring, building a strong biological safety barrier for farms, and conducting publicity and training for practitioners should be strengthened.
