Clinical value of automated breast ultrasound in measuring inter‐rectus distance: A preliminary study

摘要

目的:探讨自动乳腺容积超声(ABUS)跨领域应用于腹直肌间距(IRD)测量的技术可行性、性能优势及临床价值,并与传统手持超声(HHUS)对比分析。方法:选取2024年1—3月于四川省妇女儿童医院接受IRD测量的无孕产史健康女性志愿者40例,由3名不同职称医师分别采用ABUS和HHUS,在静息状态及卷腹状态下测量脐上5 cm、3 cm、2 cm、脐部、脐下2 cm、3 cm共6个点位的IRD。采用独立样本t检验比较两种技术的操作时间和图像质量,采用Mann-Whitney U检验分析ABUS与HHUS两种方法IRD测量值有无差异,采用组内相关系数(ICC)评价医师间测量一致性。结果:ABUS操作时间为(3.76±0.46)min,显著短于HHUS的(6.13±0.55)min(t=17.18,P<0.001);ABUS图像质量评分(4.55±0.49),显著高于HHUS(4.12±0.62)(t=-3.07,P=0.005)。两种方法在6个测量点位的IRD值比较,差异均无统计学意义(均P>0.05)。ABUS组医师间一致性(静息状态ICC: 0.907~0.976,卷腹状态ICC: 0.838~0.955)优于HHUS组(静息状态ICC: 0.725~0.924,卷腹状态ICC: 0.796~0.911)。结论:本研究首次将ABUS跨领域应用于IRD测量,其准确性与HHUS等效,且医师间一致性更优,兼具操作高效、图像质量优异的优势,是一种适用于腹直肌分离的大规模筛查、精准临床评估及长期随访的新方法。

Abstract

Objective: To explore the technical feasibility, performance advantages, and clinical value of the cross-application of automated breast ultrasound (ABUS) in measuring the inter-rectus abdominis distance (IRD), and to compare it with traditional hand-held ultrasound (HHUS). Methods: Forty nulliparous healthy female volunteers who participated in IRD measurement from January to March 2024 were enrolled. Three physicians with different professional titles measured IRD at 6 points (5 cm, 3 cm, and 2 cm above the umbilicus; the umbilicus; and 2 cm and 3 cm below the umbilicus) in the resting and curl-up states using both ABUS and HHUS. The independent samplest-test was used to compare the operation time and image quality between the two techniques; the Mann-Whitney U test was applied to analyze the differences in IRD measurements between ABUS and HHUS; and the intraclass correlation coefficient (ICC) was used to evaluate inter-observer measurement consistency among physicians. Results: The operation time of ABUS was (3.76±0.46) min, which was significantly shorter than that of HHUS (6.13±0.55) min (t=17.18, P<0.001). The image quality score of ABUS was (4.55±0.49), which was significantly higher than that of HHUS (4.12±0.62) (t=-3.07, P=0.005). There were no statistically significant differences in the IRD values between the two methods at the 6 measurement points (all P>0.05). The inter-observer consistency in the ABUS group (ICC: 0.907-0.976 in the resting state; ICC: 0.838-0.955 in the curl-up state) was superior than that in the HHUS group (ICC: 0.725-0.924 in the resting state, ICC: 0.796-0.911 in the curl-up state). Conclusion: This study is the first to apply ABUS for IRD measurement. ABUS demonstrates measurement accuracy equivalent to that of HHUS, along with superior inter-observer reliability, standardized scanning protocols, operational efficiency, and enhanced image quality. This technique is suitable for large-scale screening, accurate clinical assessment, and long-term follow-up of diastasis recti abdominis.
