椎间盘退变与年龄、BMI及椎旁肌退变的关系
The relationship between intervertebral disc degeneration and age, BMI, and paraspinal muscle degeneration
目的 分析椎间盘退变与年龄、身体质量指数(body mass index,BMI)及椎旁肌(腰大肌、多裂肌、竖脊肌)退变的关系。 方法 随机选取2019年6月至2024年12月期间319名腰腿疼痛患者的影像学资料,采用Image J软件测量L4~S1平面椎旁肌横截面积(cross-sectional area, CSA)、椎间盘CSA及椎旁肌脂肪浸润程度,通过计算椎旁肌功能性相对横截面积(functional relative cross-sectional area, FrCSA)来评价椎旁肌退变程度,椎间盘退变程度采用Pfirrmann分级进行分级。各椎间盘等级间年龄、BMI及椎旁肌FrCSA的差异采用单因素方差分析比较,其中不满足正态分布或方差齐性的数据采用Kruskal-Wallis秩和检验进行比较;椎间盘等级与年龄、BMI及椎旁肌FrCSA的相关性采用Spearman 相关性分析进行评价;椎间盘退变的影响因素采用多因素线性回归进行分析。 结果 L5/S1、L4/L5平面上,各椎间盘等级间年龄、BMI及椎旁肌FrCSA均存在统计学差异(P均<0.05)。椎间盘等级与年龄、BMI存在正相关(P均<0.05),与椎旁肌FrCSA存在负相关(P均<0.05)。多因素回归显示L5/S1平面上,年龄、BMI、腰大肌FrCSA、多裂肌FrCSA是椎间盘退变的独立影响因素;L4/L5平面上,年龄、BMI、腰大肌FrCSA、竖脊肌FrCSA是椎间盘退变的独立影响因素。 结论 年龄、BMI及椎旁肌退变程度与椎间盘退变程度存在相关性,且年龄、BMI、椎旁肌退变是椎间盘退变的独立影响因素。
Objective This paper is written to analyze the relationship between intervertebral disc degeneration and age, body mass index (BMI), and paraspinal muscle degeneration. Methods Imaging data from 319 patients with low back and leg pain were randomly selected between June 2019 and December 2024. Image J software was used to measure the cross-sectional area (CSA) of paraspinal muscle, CSA of intervertebral disc, and degree of fat infiltration in the paraspinal muscles at the L4~S1 level. The severity of paraspinal muscle degeneration was evaluated by calculating the functional relative cross-sectional area (FrCSA) of paraspinal muscles, while the degree of intervertebral disc degeneration was graded according to the Pfirrmann classification system. The differences of age, BMI and FrCSA of paravertebral muscle among different intervertebral disc grades were compared by one-way ANOVA, and the data that did not meet the normal distribution or homogeneity of variance were compared by Kruskal Wallis rank sum test. The correlation between intervertebral disc grade and age, BMI and FrCSA of paraspinal muscles was evaluated by Spearman correlation analysis. The influencing factors of intervertebral disc degeneration were analyzed by multivariate linear regression. Results At the L5/S1 and L4/L5 levels, statistically significant differences in age, BMI, and FrCSA of the paraspinal muscles were observed among different disc degeneration grades (all P<0.05). Disc degeneration grades showed a positive correlation with age and BMI (all P<0.05), and negatively correlated with FrCSA of paravertebral muscle (all P<0.05). Age, BMI, and FrCSA of paravertebral muscle were identified as independent influencing factors for intervertebral disc degeneration. Conclusion Age, BMI, and the degree of paraspinal muscle degeneration were significantly correlated with the severity of intervertebral disc degeneration. Furthermore, these three factors (age, BMI, and paraspinal muscle degeneration) were independent influencing factors of disc degeneration.
