粤港澳大湾区医疗资源配置现状与优化策略研究
Research on current situation and optimization strategy of medical resources allocation in Guangdong-Hong Kong-Macao Greater Bay Area
医疗卫生资源合理配置是实现医疗服务均质化的关键要素,是“健康湾区”建设的基本要求,研究分析了粤港澳大湾区内医疗资源配置现状及问题,以提出粤港澳大湾区医疗资源配置的优化策略.通过资料收集、数据统计等方式对粤港澳大湾区内各城市的物力资源配置、人力资源配置、财力资源配置进行对比分析;绘制各城市的卫生机构和医疗人力资源的洛伦兹曲线图并测算基尼系数. 结果显示:卫生院(站)(GI = 0.71)、急诊机构(GI = 0.54)、专科疾病防治机构(GI = 0.35)、技师(士)(GI = 0.29)的根据人口分布的医疗卫生资源洛伦兹曲线远离绝对公平线. 结果说明粤港澳大湾区内医疗资源配置公平性仍待进一步提升,可通过创新粤港澳互联互通机制解决港澳医疗资源短缺现状,同时完善分级诊疗制度以缓解异地就医耗竭医疗资源现况.
The equitable distribution of medical and healthcare resources is a crucial factor in achieving parity in medical services and an essential prerequisite for the development of a 'Healthy Bay Area'. By conducting an analysis of the current state and challenges pertaining to resource allocation within the Guangdong-Hong Kong-Macao Greater Bay Area, this study proposes a strategy aimed at optimizing resource allocation within the region. Through data collection and data statistics, the material resources allocation, human resources allocation and financial resources allocation of cities in the Guangdong-Hong Kong-Macao Greater Bay Area were compared and analyzed. At the same time, the Lorenz curve and Gini coefficient were calculated for the health institutions and medical human resources in cities to obtain the fairness of resource allocation. The Lorentz curve of medical resources based on population distribution was far from the absolute equality line in health centers (GI = 0.71), emergency institutions (GI = 0.54), specialized disease prevention institutions (GI = 0.35) and technicians (GI = 0.29). The fairness of medical resource allocation in Guangdong-Hong Kong-Macao Greater Bay Area still needs to be further improved, and the innovation of Guangdong-Hong Kong-Macao connectivity mechanism is conducive to solving the shortage of medical resources in Hong Kong and Macao. The exhaustion of medical resources in different places can be alleviated by improving the hierarchical diagnosis and treatment system.
