Mendelian randomization studies on cardiometabolic factors and intracranial aneurysms: A systematic literature analysis

摘要

目的 颅内动脉瘤(intracranial aneurysm,IA)起病隐匿,一旦破裂,致死率、致残率均较高。心血管代谢因素可能与其形成和破裂相关。本研究旨在总结孟德尔随机化(Mendelian randomization,MR)方法在IA心血管代谢因素研究中的应用,以期为阐明IA的病因和发病机制提供线索。 方法 检索PubMed、Embase、Web of Science、中国知网和万方数据知识服务平台建库至2024年2月21日收录的基于MR方法的IA研究相关文献,由2名研究者独立进行文献筛选、数据提取和质量评价,采用叙述整合方法对纳入文献进行定性系统综述。 结果 共纳入2017至2024年发表的11篇文献(11项研究),其中4项被判定为高质量研究。研究探讨了血压、血脂、血糖、肥胖相关指标和炎症因子与IA及其亚型的关系,但存在重复发表的问题。4项基于同一欧洲人群不同筛选标准的MR研究及 1项基于不同欧洲人群的MR研究均发现血压是IA及其亚型的危险因素;其他MR研究中血脂、血糖、肥胖相关指标和炎症因子与IA及其亚型关联的结果不一致。 结论 血压升高可增加IA及其亚型的发生风险,对于其他心血管代谢因素与IA及其亚型的关联需要进一步研究。MR研究仍存在局限性,需综合其他证据,谨慎进行因果推断。

Abstract

Objective Intracranial aneurysm (IA) has an insidious onset, and once ruptured, it carries high rates of mortality and disability. Cardiometabolic factors may be associated with the formation and rupture of IA. This study aims to summarize the application of Mendelian randomization (MR) methods in research on cardiometabolic factors and IA, providing insights for further elucidation of IA etiology and pathogenesis. Methods Literature about MR-based IA studies published up to February 21, 2024, was retrieved from PubMed, Embase, Web of Science, CNKI, and Wanfang. Two researchers independently performed literature screening, data extraction, and quality assessment. A narrative synthesis approach was used to conduct a qualitative systematic review of the included studies. Results A total of 11 MR-based studies on IA published between 2017 to 2024 were included, of which 4 were rated as high quality. These studies investigated the associations between blood pressure, blood lipids, blood glucose, obesity-related indicators, and inflammatory cytokines with IA and its subtypes, though issues of duplication were noted. Four MR studies based on the same European population but using different instrumental variable selection criteria, as well as another MR study in a different European cohort, consistently identified blood pressure as a risk factor for IA and its subtypes. Findings for blood lipids, blood glucose, obesity-related indicators, and inflammatory cytokines were inconsistent across MR studies. Conclusion Blood pressure appears to increase the risk of IA and its subtypes. Associations between other cardiometabolic factors and IA/subtypes require further in-depth investigation. Given the inherent limitations of MR studies, causal inferences should be made cautiously in combination with other lines of evidence.

颅内动脉瘤(intracranial aneurysm,IA)是一种常见的脑血管疾病,起病隐匿,如果发生破裂,会引发蛛网膜下腔出血,危及生命。IA病因研究对于针对性地进行预防和干预至关重要。心血管代谢因素可能与IA风险存在关联[1-3]。但由于混杂因素和时间序列效应的影响,目前仍无法确定它们之间的因果关系。孟德尔随机化(Mendelian rando-mization,MR)方法利用与暴露因素高度相关的遗传变异作为工具变量,可推断暴露与结局之间的因果关系,提供病因推断的补充性证据[4]。近年来,MR研究[5-15]探讨了心血管代谢因素与IA及其亚型的关联。本文对IA和心血管代谢因素的MR研究进行系统综述,以期为IA的病因与发病机制研究提供线索。

1 资料与方法

1.1 研究注册

本研究已在INPLASY(International Platform of Registered Systematic Review and Meta-analysis Protocols)数据库平台注册(注册编号:202470035)。

1.2 资料来源与检索策略

采用主题词与自由词相结合的检索方式,在PubMed、Embase、Web of Science等英文数据库和中国知网、万方数据知识服务平台等中文数据库进行系统检索,检索时间为建库至2024年2月21日,语种限定为中文和英文,检索策略见表1

1.3 文献的纳入和排除标准

纳入标准:1)MR研究设计;2)暴露为心血管代谢因素,包括血压、血脂、血糖、肥胖相关指标、炎症因子和尿酸等;3)结局为IA和/或其亚型[未破裂颅内动脉瘤(unruptured intracranial aneurysm,uIA)和动脉瘤性蛛网膜下腔出血(aneurysmal subarachnoid hemorrhage,aSAH)],uIA和aSAH患者分别根据国际疾病分类-10(International Classification of Diseases-10,ICD-10)代码I67.1和I60进行诊断。排除标准:1)评论、综述、共识和指南;2)会议摘要、书籍章节等研究数据不全的文献;3)重复发表文献。

1.4 文献筛选和数据提取

利用Endnote X9软件对检索到的文献进行管理,排除重复文献后,根据纳入和排除标准,对标题和摘要进行初步阅读筛选;对经过筛选的文章进行全文阅读,以确定最终是否纳入。对纳入的文献提取暴露(心血管代谢因素、数据来源、样本量、种族)、结局(IA及其亚型、数据来源、样本量、种族)、单核苷酸多态性(single nucleotide polymorphisms,SNPs)的筛选标准(显著性阈值Pr2F)、效应值及其置信区间等基本信息。由2名研究者独立进行文献筛选、数据提取和质量评价,并交叉核对,如有意见不统一,则讨论决定或交由第3人协助裁定。使用Excel 2016对每篇文献的相关信息进行详细记录。

1.5 质量评价

目前,MR研究尚无金标准的质量评价方法。参考《孟德尔随机化研究的报告规范》(Strengthening the Reporting of Observational Studies in Epidemiology Using Mendelian Randomization,STROBE-MR)[16]和既往MR研究的其他系统综述[17-18],从7个方面对MR研究质量进行评价,包括关联性、独立性、排他性、工具变量(质量控制、连锁不平衡、弱工具变量)、人群(样本重叠、人群分层)、敏感性分析和其他混杂,各部分的判定原则见表2。对于双样本MR,总分为10分,7分及以上为高质量研究;对于单样本MR,总分为8分,5分及以上为高质量研究。

2 结 果

2.1 文献基本特征和质量评价结果

共检索到相关文献226篇,剔除重复文献124篇,根据纳入和排除标准,最终纳入11篇文献(图1),均为英文文献。研究人群主要来自欧洲,包含血压、血脂、血糖、肥胖、炎症因子等常见心血管代谢因素,研究结局包含IA、uIA和aSAH。

11项研究中,有4项研究被判定为高质量MR研究(表3)。

2.2 心血管代谢因素与<bold>IA</bold>的<bold>MR</bold>研究

2.2.1 血压

有5项MR研究[5-9]探讨了血压与IA及其亚型的因果关联(附表1https://doi.org/10.57760/sciencedb. 28756),但存在重复发表的问题,其中有4项MR研究[6-9]是利用同一欧洲人群不同工具变量筛选标准进行的。文献[5]最先在欧洲人群采用逆方差加权法(inverse variance weighted,IVW)分析,结果表明收缩压每升高10 mmHg(1 mmHg=0.133 kPa),IA和aSAH的发生风险分别增加87%(OR=1.87,95% CI 1.61~2.17)和117%(OR=2.17,95% CI 1.82~2.58);舒张压每升高10 mmHg,IA和aSAH的发生风险分别增加192%(OR=2.92,95% CI 2.49~3.43)和221%(OR=3.21,95% CI 2.66~3.87);敏感性分析也支持IVW分析的结果。随后4项在另一欧洲人群中根据不同SNPs筛选标准的MR研究[6-9]也发现,收缩压和舒张压升高与IA、uIA和aSAH的发生风险增加相关(均 P<0.05)。文献[9]发现血压每升高1 mmHg,IA及其亚型的发生风险增加。文献[6]发现高血压与IA及其亚型的发生风险增加相关。

2.2.2 血脂

5项MR研究[5-7, 10-11]探讨了血脂与IA及其亚型的因果关联(附表2https://doi.org/10.57760/sciencedb. 28756)。文献[5]和文献[7]利用同一欧洲人群数据,根据不同SNPs筛选标准的MR研究发现,甘油三酯(triglyceride,TG)与IA、aSAH之间存在关联,但两者结果存在矛盾。文献[5]发现TG每增加1个标准差(standard deviation,SD),IA的发生风险增加19%(OR=1.19,95% CI 1.02~1.38)。而文献[7]发现TG每增加1个SD,aSAH的发生风险降低29%(OR=0.71,95% CI 0.53~0.94)。文献[6]在多种族人群中发现高总胆固醇(total cholesterol,TC)每增加1个SD,IA的发生风险降低14%(OR=0.86,95% CI 0.75~0.99)。但上述3项研究的敏感性分析差异均无统计学意义。文献[10]在欧洲和东亚队列中发现,低密度脂蛋白胆固醇(low density lipoprotein cholesterin,LDL-C)每增加 1 mmol/L,IA(OR=0.83,95% CI 0.73~0.94)和aSAH(OR=0.83,95% CI 0.70~0.97)的发生风险均降低17%。文献[11]在不同的欧洲人群中证实了文献[10]的结果,发现高密度脂蛋白胆固醇(high density lipoprotein cholesterol,HDL-C)每增加1 mmol/L,IA和aSAH的发生风险分别降低34%(OR=0.66,95% CI 0.43~0.99)和44%(OR=0.56,95% CI 0.35~0.91)。

2.2.3 血糖

4项MR研究[5-7, 12]分析了血糖与IA及其亚型的因果关联(附表3https://doi.org/10.57760/sciencedb. 28756),其中3项MR研究中的部分指标是在同一人群不同SNPs筛选标准下进行的。文献[5]发现空腹血糖每增加1 mmol/L,IA和aSAH的发生风险分别降低36%(OR=0.64,95% CI 0.43~0.95)和43%(OR=0.57,95% CI 0.34~0.94),糖化血红蛋白(hemoglobin A1c,HbA1c)、空腹胰岛素及2型糖尿病(type 2 diabetes,T2D)与IA和aSAH没有关联(P>0.05)。文献[7]发现空腹血糖每增加1 mmol/L,IA和aSAH的发生风险分别降低65%(OR=0.35,95% CI 0.18~0.68)和72%(OR=0.28,95% CI 0.12~0.64)。文献[6]发现HbA1c(OR=0.77,95% CI 0.60~0.98)、空腹血糖(OR=0.56,95% CI 0.36~0.87)以及T2D(OR=0.89,95% CI 0.83~0.95)与IA风险降低相关。而文献[12]在荷兰人队列中未发现T2D与IA的关联。

2.2.4 肥胖

4项MR研究[5-7, 12]探讨了肥胖相关指标与IA及其亚型的关联(附表4,https://doi.org/10.57760/sciencedb. 28756),其中3项MR研究中的部分指标是在同一人群不同SNPs筛选标准下进行的。文献[5]在欧洲人群中发现体重指数(body mass index,BMI)每增加1个SD,IA和aSAH的发生风险分别增加15%(OR=1.15,95% CI 1.01~1.32)和21%(OR=1.21,95% CI 1.03~1.41)。文献[6]发现体脂率与IA存在关联 (OR=1.29,95% CI 1.11~1.52)。文献[7]的IVW分析结果显示,BMI每增加1个SD,IA风险增加26%(OR=1.26,95% CI 1.07~1.49),并提示内脏脂肪和瘦体重与IA和aSAH风险之间可能存在关联。而文献[12]在荷兰人队列中未发现BMI及腰臀比与IA存在关联。

2.2.5 炎症因子

4项MR研究[6, 13-15]探讨了炎症因子与IA及其亚型的因果关联(附表5,https://doi.org/10.57760/sciencedb. 28756),但存在重复发表的问题,其中有2项MR研究是在同一欧洲人群不同SNPs筛选标准下进行的。文献[15]发现干扰素γ诱导蛋白-10(interferon γ induced protein-10,IP-10)与uIA的发生风险呈负相关(OR=0.63,95% CI 0.45~0.90);白介素-10(interleukin-10,IL-10;OR=1.19,95% CI 1.02~1.39)和血管内皮生长因子(vascular endothelial growth factor,VEGF;OR=1.12,95% CI 1.01~1.23)与aSAH的发生风险呈正相关;成纤维细胞生长因子(fibroblast growth factor,FGF;OR=0.62,95% CI 0.42~0.92)和肿瘤坏死因子相关凋亡诱导配体(tumor necrosis factor related apoptosis-inducing ligand,TRAIL;OR=0.89,95% CI 0.80~1.00)与aSAH的发生风险降低相关。而文献[14]发现VEGF(OR=1.15,95% CI 1.04~1.26)与aSAH的发生风险呈正相关,FGF(OR=0.62,95% CI 0.42~0.92)与aSAH的发生风险降低相关;FGF-9(OR=0.74,95% CI 0.62~0.89)和FGF-16(OR=0.84,95% CI 0.72~0.97)与aSAH的发生风险呈负相关。此外,文献[13]和文献[6]在不同欧洲人群中均未发现C反应蛋白(C-reactive protein,CRP)、IL-10和IL-18与IA及其亚型的关联。

3 讨 论

本研究总结了心血管代谢因素与IA及其亚型关联的MR研究相关证据,发现血压可增加IA及其亚型的发生风险,而血脂、血糖、肥胖相关指标和炎症因子虽然与IA及其亚型存在一定的关联,但研究结果不一,还需进一步深入研究。

既往基于不同人种的观察性研究[19-21]表明血压显著增加IA及其亚型的发生风险,4项基于同一欧洲人群不同SNPs筛选标准的MR研究及另1项基于不同欧洲人群的MR研究均提示血压是IA及其亚型的危险因素,支持血压与IA及其亚型风险存在因果关联。血流动力学应激是血压导致IA发生的机制之一[22]。血压升高导致血管壁的压力增加,特别是在血管分支或弯曲处。随着时间的推移,这种压力会使血管壁变弱,形成瘤样突起,导致IA的形成。高血压还可以促进炎症和氧化应激,进一步促进IA的形成[23]。因此,控制血压对IA及其亚型的预防及控制至关重要。然而,目前的MR研究主要集中于欧洲人群,尚无针对亚洲人群的相关MR研究。

血脂在IA中的作用尚不清楚。很少有研究阐明LDL-C在IA及其亚型中的作用,但多项流行病学研究[24-26]表明LDL-C与脑出血风险呈负相关。此外,日本的临床研究[27]表明血脂可能导致IA形成,但也可能降低IA破裂的风险。一项在欧洲人群中进行MR研究[11]支持HDL-C与IA和aSAH风险降低相关,2项在不同人群中进行的MR研究[10-11]支持LDL-C与IA和aSAH风险降低相关。HDL-C具有抗炎、抗凋亡、抗氧化和抗血栓特性[28-29],这些功能可以解释HDL-C可以降低IA破裂风险的原因。但是,目前关于LDL-C与IA和aSAH发生机制的关系尚不清楚。动物模型研究[30]显示,较低的胆固醇浓度与颅内血管壁脆性和通透性增加有关,从而导致血管神经紧张和微动脉瘤形成。尽管这些发现来自脑实质内出血的临床前模型,但似乎同样的机制也可能削弱血管壁,导致IA形成并最终破裂[31]。病理研究[32-33]表明脂质浸润的增加与IA破裂有关,推测这可能是由于脂质对IA的被动修复。为更全面地理解血脂在IA形成和破裂中的作用,有必要进行实验研究探讨内壁变性和破裂中脂质积累的生物学机制。

流行病学研究中血糖与IA及其亚型的关联相互矛盾。一些研究[3, 34]显示糖尿病与IA及其亚型风险降低相关;另一些研究[35-36]显示T2D与IA及其亚型的风险增加相关。一项基于多种族人群的MR研究[6]支持T2D与IA风险降低相关,其他不同数据来源的MR研究[5, 12]结果并不支持这一假设。此外,MR研究[5-6]还发现空腹血糖和HbA1c与IA和aSAH风险降低相关。血糖与IA和aSAH之间的机制尚不清楚,但有研究[37]表明糖尿病患者可能接受饮食或药物治疗并改变生活习惯,这可降低IA和aSAH风险。糖尿病患者在发生IA之前死于其他并发症的风险可能更高[37]。尽管这些研究提供了一些线索,但目前还不能得出一个明确的结论,需要更多的研究来阐明血糖在IA形成和破裂中的具体作用。

BMI是超重和肥胖最常用的衡量标准。观察性研究[38-39]表明BMI与IA的形成和破裂相关。2项在同一人群不同SNPs筛选标准下进行的MR研究[5, 7]支持高BMI增加IA和aSAH的风险,1项基于欧洲人群的MR研究[6]支持体脂率与IA的风险增加相关。目前对于肥胖参与IA和aSAH发生的机制尚不完全清楚,但有研究[40]表明肥胖引起血管内皮损伤和功能障碍,减少脑紧密连接蛋白的表达,与IA的形成和破裂有关;也有研究[41]表明脂肪细胞能够募集和激活炎症细胞,从而引发血管和内皮组织的炎症,增加IA的风险。但是,上述关系在uIA患者中未被发现,可能的解释是样本量有限,可能会降低统计学功效。因此,需要大样本研究进一步来揭示它们之间的关联。

炎症因子参与调节血管壁的炎症反应,可能促进IA的形成和发展[42-43]。2项在不同人群中开展的MR研究[6, 13]未发现炎症因子与IA及其亚型存在关联,另外2项在同一人群不同SNPs筛选标准下进行的MR研究[14-15]发现IP-10、IL-10、VEGF、bFGF、TRAIL、FGF-9及FGF-16与uIA或aSAH风险可能存在关联,表明以上这些炎症因子与IA发病机制之间存在潜在关联。然而,潜在的机制仍未完全阐明。IP-10可能通过促进抗炎巨噬细胞和CD4+ T细胞的转运,减少血管平滑肌细胞的表型转化,抑制IA的进展,从而促进动脉瘤壁炎症的消退[44-45]。IL-10的升高与低动脉瘤壁增强(aneurysm wall enhancement,AWE)有关,低AWE的动脉瘤更不稳定[46]。VEGF可能首先引起动脉瘤壁内皮细胞的损伤,进而影响内膜和中膜的结构稳定性,最终导致IA破裂[15]。FGF通过促进动脉瘤内壁血管平滑肌细胞的增殖,增强内壁稳定性,降低动脉瘤破裂的可能性[47]。TRAIL的保护作用可能与炎症的早期消退和动脉瘤壁血管平滑肌细胞增殖的刺激有关[48]。尽管如此,目前关于炎症生物标志物在IA形成和进展中作用的流行病学和实验证据仍然不足,需要未来的研究进一步阐明炎症因子与IA及其亚型的具体作用机制,为IA的预防和治疗提供新的策略。

MR研究结果的质量受诸多因素的影响。在研究设计严谨且工具变量符合核心假设的情况下,MR研究可以为心血管代谢因素与IA结局之间的潜在因果关联提供初步线索,并为实验室研究和机制探索提供理论支持。然而,MR分析本身也存在局限性,包括但不限于弱工具变量、多效性、连锁不平衡以及人群分层等问题。研究者应结合MR研究、既往观察性研究、生物学合理性等多方面证据对潜在的因果关系进行综合解读,为进一步阐明IA的病因和发病机制提供线索。

综上所述,本研究发现血压能够增加IA和aSAH的风险,其他心血管代谢因素与IA及其亚型的关联不一,需要更多的研究进一步探索这些因素与IA的关联,以便为临床实践和公共卫生政策提供更有针对性的指导。
