Abstract

Objective This study aimed to investigate the effects of a mobile health-based lifestyle intervention for gestational weight management on metabolic indicators, including fasting blood glucose (FBG), blood lipids [total cholesterol (TC), triglycerides (TG), high-density lipoprotein cholesterol (HDL-C), low-density lipoprotein cholesterol (LDL-C)], fasting insulin (FINS), and glycated hemoglobin (HbA1c), in overweight and obese pregnant women during the second trimester of pregnancy. Methods A randomized controlled trial (RCT) was conducted at Weifang Maternal and Child Health Hospital (Shandong Province) and Tongzhou District Maternal and Child Health Hospital (Beijing), each enrolling 100 overweight or obese pregnant women. Participants were stratified by pre-pregnancy body mass index (BMI), age, and parity, and then randomly assigned to an intervention group or a control group. The intervention group received management through a WeChat-based platform aimed at improving dietary behaviors and increasing physical activity, focusing on six components: health education, dietary goal setting, dietary behavior monitoring, exercise monitoring, weight tracking, and evaluation feedback. The intervention began at 10-14 gestational weeks, with face-to-face or telephone follow-ups every two weeks. Data were collected on pre-pregnancy weight, FBG, TC, TG, HDL-C, and LDL-C at 8-14 and around 28 gestational weeks. Residual serum samples from these two time points were used to measure FINS and HbA1c. Maternal fasting weight was recorded during the routine oral glucose tolerance test (OGTT) at 24-28 weeks. The control group received routine prenatal care according to standard institutional procedures. A generalized linear mixed model (GLMM) was applied to assess the effects of the intervention on gestational weight gain (GWG) and metabolic indicators. Mediation analysis was conducted using the “Mediation” package in R. Results Among the 178 pregnant women included in the analysis, compared with the control group, the intervention group had significantly lower gestational weight gain at 24-28 weeks, with an adjusted mean difference of -1.63 kg (95% CI: -2.80 to -0.46, t = -2.736, P = 0.007). The intervention effectively reduced FBG change, with an adjusted mean group difference of -0.34 mmol/L (95% CI: -0.53 to -0.14, t = -3.409, P < 0.001). Mediating results showed that weight gain in the second trimester mediated 31.0% (P = 0.036) of the changes in FBG before and after the intervention. Conclusion The lifestyle intervention based on mobile health technology to control weight gain during pregnancy can effectively reduce the level of FBG in overweight and obese pregnant women in the second trimester, and gestational weight gain (GWG) exerted partial mediating roles in reducing FBG.
