摘要

2025年6月骨整合学会和美国牙周病学会联合制定的关于预防和管理种植体周病和状况的专家共识发布,该共识基于8篇系统评价,重点关注种植体周黏膜炎、种植体周炎、种植体周软组织缺损以及种植体周边缘骨丧失的危险因素和防治策略,分析了全身疾病、行为因素、患者因素、种植位点局部条件、手术因素和修复设计等对种植体周病和状况发生的影响并对其风险程度进行分级,随后基于这些风险因素对不同种植体周病和状况的防治提出了指导意见。本文拟结合我国国情对该共识进行要点解读,梳理循证医学证据和专家共识意见,以提高国内医生对种植体周病和状况防治的理解,从而提高种植体留存率。

Abstract

The article “Academy of Osseointegration/American Academy of Periodontology consensus on the prevention and management of peri-implant diseases and conditions: summary report” was published in June 2025. This consensus, supported by eight systematic reviews, focused on the prevention and management of peri-implant diseases and conditions. It placed a particular focus on the risk factors and therapeutic strategies for peri-implant mucositis, peri-implantitis, peri-implant soft tissue deficiency, and peri-implant marginal bone loss. It analyzed the effects of systemic diseases; behavioral, patient-related, and surgical factors; implant site conditions; and prosthetic design on the occurrence of peri-implant diseases and conditions and stratified their risk levels. Clinical guidelines for the prevention and management of different peri-implant diseases and conditionswere proposed on the basis of the above risk factors. This article aims to provide a critical interpretation of the consensus in the context of China’s clinical practice, synthesizing evidence-based recommendations and expert opinions to enhance clinicians’ understanding of peri-implant disease and condition management, thereby improving long-term implant survival rates.

近年来种植修复呈爆发式增长,其中国内市场的增长非常明显,2020年超过7亿美元,约占全球市场的19.4%[1]。种植体周病(peri-implant diseases,PID)的患病率和种植体周相关状况的发生率较高,严重影响了种植体使用寿命,因此预防和管理PID的需求日益迫切。2023年,欧洲牙周病学会(European Federation for Periodontology,EFP)曾发布PID防治S3级临床指南[2],该指南主要关注PID的初级、一级、二级、三级预防,并对PID治疗提出了指导性建议。2024年,英国牙周病和种植学会(British Society of Periodontology and Implant Dentistry,BSP)根据英国医疗现状,对EFP指南进行了补充[3]。2025年6月,骨整合学会/美国牙周病学会(Academy of Osseointegration/American Academy of Periodontology,AO/AAP)发布了种植体周病和状况防治的专家共识[4],与之前的指南相比,此次共识更侧重于种植体周病和状况的危险因素、临床治疗和管理。本文拟对此共识进行要点梳理和解读,以期为国内种植体周病和状况的防治提供参考。

1  共识制订的背景和方法

1.1  制订背景

全球不同地区的流行病学研究[5-9]表明,43%~47%接受种植治疗的患者会发生种植体周黏膜炎,20%~22%的患者会发生种植体周炎。而关于种植体周软组织缺损(peri-implant soft tissue deficiency,PSTD)以及种植体周边缘骨丧失(marginal bone loss,MBL),由于不同研究在诊断标准、纳入样本和随访时长上存在显著差异,各项流行病学指标不具有直接可比性,其发生率仍待进一步研究[10-11]。与牙周疾病相比,PID的治疗成功率更低[3,12-13],这可能和种植体周独特的解剖及生理特点有关[14-15]。因此,在种植体植入前尽早识别危险因素、制定预防策略[16-17]尤为重要。

本质上,PID的发生是菌斑和宿主免疫失衡的结果。大量科学证据已将各种全身疾病、行为因素和种植位点局部条件列为PID发生的可能危险因素,如牙周炎病史[18-20]、糖尿病、肥胖[21-22]、吸烟[23-24]、大量饮酒[24-25]以及未定期复查[26]。近年的研究[16,26-28]强调了手术变量(如种植体植入位置不佳)在PID发生和进展中的重要作用。此外,基台的高度[29-30]、角度和外形[31-33]也与发生MBL的风险增加有关。为了综合现有的种植体周病和状况的流行病学、危险因素和疾病管理的最佳证据,使临床医生通过确定关键的危险因素来预防PID[7,34]以及PSTD[12-13,35]的发生,并对已经发生的种植体周病和状况进行规范化治疗,AO/AAP于2024年8月联合组织了共识会议。

1.2  制订方法

专家们分为2组:第一组侧重于确定PID和种植体周状况发生、发展的关键危险因素,并为风险管理和疾病预防提出临床建议[36];而第二组则是根据当前的科学证据和临床最佳实践制定风险分层治疗方案,以指导PID的治疗和管理,并建立个性化维护方案[37]

1)证据的系统评价。每个小组在共识会议召开前对重点问题进行系统评价,并在可能的情况下进行meta分析。

2)共识意见分级。44名专家对他们所提交发言的同意程度进行了投票。共识程度根据小组成员的意见分为:一致同意(100%同意)、强烈共识(95%<同意<100%)、共识(75%~95%同意)、多数同意(50%~74%同意)、未达成共识(<50%同意)。

2  PID和种植体周状况的危险因素和临床对策

本部分针对以下4个问题进行了系统评价和meta分析:1)PID的总体患病率和患者全身状况、行为、相关危险因素,共纳入102项研究[38];2)PID发生/发展中的手术和种植体相关因素,共纳入33项研究[39];3)修复体设计对MBL的影响,共纳入93项研究[40];4)PSTD流行情况及其风险和保护因素,共纳入221项研究[11]。最后,在表1中对危险因素进行汇总,根据风险等级分为高、中、低风险,同时基于风险因素对不同PID和种植体周状况的预防提出临床对策。

2.1  种植体周黏膜炎的危险因素和临床对策

2.1.1  危险因素

meta分析结果显示,5、10、20年种植体周黏膜炎的患病率分别为46%、61%、53%[38]。基于投票结果,专家共识[4]提出的危险因素主要包括以下因素。1)全身因素:①肥胖[中风险(多数同意)];②肥胖和/或代谢综合征[中风险(未达成共识)]。2)行为因素:①吸烟是最受关注的行为因素,无论是当前吸烟还是近10年内有吸烟史,都可能是种植体周黏膜炎的高风险因素,但未达成共识;②饮酒量:低风险(未达成共识)。3)患者因素:①牙周炎病史[高风险因素(共识)];②未控制的牙周炎也被认为是高风险因素,其差异具有统计学和临床意义,但未达成共识;③薄软组织表型[中风险(共识)]。

2.1.2  预防种植体周黏膜炎的临床对策

1)尽量延期种植,尽量使用手术导板,尽可能采用不翻瓣的方法植入种植体。笔者提醒,在无数字化技术辅助的情况下,应谨慎采用不翻瓣种植;2)对于美学区和薄软组织表型的部位,在种植体植入前或术中进行软组织增量;3)强烈建议在种植前戒烟,对于吸烟者,要注意定期随访,并进行彻底的菌斑控制;4)牙周炎患者应接受彻底的牙周治疗,并在植入前控制牙周炎症。应定期复查,并进行牙周支持照护(supportive perio-dontal care,SPC);5)对于肥胖和/或代谢综合征患者,应进行体重监测,推广健康生活方式,加强种植体周的维护治疗。

2.2  种植体周炎的危险因素和临床对策

2.2.1  种植体周炎的危险因素

meta分析显示,5、10、20年种植体周炎的加权平均患病率分别为12%、14%和22%[38],其危险因素[4,39]包括以下4种。1)全身因素:①未控制或控制不佳的糖尿病[高风险(共识)];②抑郁症或服用抗抑郁药物[中风险(共识)];③代谢综合征[中风险(共识)];④少数专家认为高血压(34%同意)、肥胖症(44%同意)与种植体周炎的风险较高有关。2)行为因素:①吸烟或吸烟史[高风险(共识)];②饮酒也可能是种植体周炎的重要风险指标(比值比2.07),然而证据有限。3)患者因素:①牙周炎病史和未经控制的牙周炎是种植体周炎的高风险因素(共识);②软组织表型也是种植体周炎的重要影响因素(共识)。4)种植体和种植位点相关因素:①种植体宏观和微观设计可能是影响种植体周炎的发生发展的危险因素(共识),但未发现任何设计特征在预防PID发生、发展方面具有优势;②种植体近远中向间距<3 mm[中风险(共识)],冠根方向上,种植体平台距离邻牙颊侧正中釉牙骨质界(cemento-ena-mel junction,CEJ)>6 mm[中风险(共识)];③植入部位的骨条件,植入部位骨宽度不足[中风险(共识)],也有少数专家认为骨增量部位的种植体更容易发生种植体周炎(36.5%同意);④修复设计,种植体基台/修复体界面与牙槽骨距离过近[中风险(共识)];⑤种植体周黏膜炎是种植体周炎的易感因素(共识)。

2.2.2  预防种植体周炎的临床对策

1)应制定合适的种植治疗计划;2)使用导板或计算机导航技术是有益的,尤其是对复杂病例而言;3)种植体间距应>3 mm,种植体基台与邻牙颊侧正中CEJ的垂直向距离应<6 mm;4)在骨宽度不足位点植入种植体时,应在治疗计划中考虑未来的骨改建,可考虑植入窄直径的种植体;5)软组织水平种植体或穿龈基台有助于避免MBL的发生;6)牙周炎患者应接受彻底的牙周治疗,在牙周炎症控制后方可植入种植体;7)对于糖尿病患者,应积极和相关医生配合,控制血糖,对于未控制或控制不佳的糖尿病患者,定期复诊和严格的菌斑控制至关重要;8)强烈建议在种植治疗前戒烟,定期复诊和严格的菌斑控制对吸烟患者至关重要;9)体重监测和健康生活方式推广应是口腔护理的一部分,种植治疗也不例外;10)应及时发现患者的抑郁或慢性压力迹象,并采取相应的治疗措施,如转诊至心理医生处,必要时,应推迟种植治疗。术后应增加这些患者的维护治疗频率。

2.3  功能负载后种植体周MBL的危险因素和临床对策

2.3.1  危险因素

在连接穿龈部件后的1年时间内,种植体周边缘骨可能会进行生理性重建[41]。但某些因素可能会导致种植体周出现额外的MBL,包括但不限于修复体设计、手术设计和生物膜沉积。此处仅讨论和修复体、手术设计相关的因素[40],主要包括以下几点。1)基台的选择和设计:①使用平台匹配基台(platform-matched abutment)[高风险(共识)];②穿龈高度<2 mm[高风险(共识)];③穿龈轮廓角度>30°[高风险(共识)];④凸形基台穿龈轮廓会增加骨水平种植体发生MBL的风险,反之,凹形设计会降低骨水平种植体发生 MBL 的风险(暂无循证医学证据,但已达成共识)。2)修复方案:①种植体支持式联冠修复[中风险(共识)]。3)黏膜下粘接剂残留[高风险(虽具有统计学和临床意义,但未达成共识)]。

2.3.2  预防功能负载后种植体周MBL的临床对策

1)对于骨水平种植体,不应植入过浅,至少预留2 mm的穿龈基台高度;2)植入间距应≥3 mm;3)最好采用螺丝固位,如需采用粘接固位,则应使用氧化锌或玻璃离子粘接剂以获得X线阻射影,必须清除残留的粘接剂。

2.4  PSTD的危险因素和临床对策

2.4.1  危险因素

PSTD是一个新兴的临床问题,即种植体周软组织缺损,导致植体/基台的暴露[35],增加PID发生和发展风险[42-44]。PSTD的发生率在1年内为33.3%,3~5年为38.3%,5年后为64%,其危险因素如下。1)种植或修复设计:①即刻植入[中风险(差异有统计学意义,但未达成共识)];②凸形基台穿龈轮廓[高风险(共识)]。2)种植体植入位置:①植入位置偏颊侧[高风险(共识)];②种植体轴向过于偏颊[中风险(共识)]。3)局部解剖异常:①颊侧骨开裂[高风险(统计和临床意义重大,但未达成共识)];②角化龈宽度<2 mm[高风险(共识)];③颊侧黏膜厚度不足[高风险(共识)]。4)邻面MBL[高风险(统计和临床意义重大,但未达成共识)]。5)种植体植入超过5年[中风险(有统计和临床意义,但未达成共识)]。

2.4.2  预防PSTD的临床对策

1)种植体应在合适的三维位置植入;2)建议进行计算机辅助种植;3)必要时,在种植体植入前或同时处理颊侧骨板过薄和颊侧骨开裂的问题;4)延期种植可以最大程度地减少一些危险因素的影响;5)在植入种植体时或最终修复前行软组织增量;6)可考虑不翻瓣种植;7)为了保持黏膜的长期稳定性,应尽量保持邻面骨水平。

3  PID和种植体周状况的管理

为了评估PID及种植体周状况的管理方法,共进行了4项系统综述,包括:1)种植体周黏膜炎的非手术治疗,共纳入24项临床试验[45];2)种植体表面清创策略,共纳入121项研究[46];3)种植体周炎的非再生性手术的有效性评估,共纳入15项随机对照研究(randomized controlled trial,RCT)[47];4)再生治疗的有效性评估,共纳入40篇文献(18篇RCT和22篇非RCT)[48]

3.1  种植体周黏膜炎的治疗

3.1.1  治疗方法

种植体周黏膜炎的治疗方法主要包括针对病因的机械清创和一系列辅助治疗[45]

1)病因治疗:主要采用对种植体表面友好的器械(如钛刮治器、超声波器械和喷砂装置)通过非手术方式去除生物膜,同时避免损伤种植体[49-50];虽然部分专家建议采用氯己定龈下冲洗作为非手术机械清创治疗种植体周黏膜炎的辅助手段,但未达成共识;不建议碘制剂龈下冲洗作为辅助手段(共识),也不建议将过氧化氢龈下冲洗作为辅助手段(多数同意)。

2)辅助治疗。①药物辅助治疗,不建议将含过氧化氢的抗菌漱口水和含碘抗菌漱口水作为治疗辅助药物(多数同意),不建议将局部抗菌药物作为辅助治疗(多数同意),也不建议将全身性抗菌药物作为辅助治疗(共识);但对于顽固病例,可考虑使用局部抗菌药物,如米诺环素软膏或多西环素凝胶。②激光辅助治疗,激光不能作为种植体周黏膜炎的单一疗法,有25%的专家认为激光可以作为治疗种植体周黏膜炎的辅助手段,但未达成共识;在无法采用其他非手术疗法的情况下,39%专家同意激光作为辅助手段用于治疗,但当种植体周探诊深度(probing depth,PD)达到6 mm,且没有骨丧失时,本共识的专家们仍然推荐激光作为机械清创的辅助治疗,常用的激光有Nd:YAG激光、CO2激光、半导体激光、Er激光、Cr:YSGG激光、Er:YAG激光等。③益生菌辅助治疗,临床证据不支持使用益生菌辅助治疗种植体周黏膜炎(多数同意),但在某些情况下,含有乳杆菌和双歧杆菌的辅助益生菌可能有助于重建口腔微生物群稳态,减少种植体周软组织炎症。

3)维护治疗。维护计划有助于降低疾病进展或复发风险;患者自我菌斑控制和医生的专业维护相结合,有助于实现种植体周的长期健康。

3.1.2  疗效

虽然很难完全实现种植体周黏膜炎的治愈[51-52],但单纯清创可显著降低种植体周PD和探诊出血(bleeding on probing,BOP),是一种有效的干预措施,再加上患者教育和适当的维护,可以有效实现菌斑控制,防止种植体周黏膜炎的发展或复发[45]。但辅助治疗的额外收益难以确定,因此并不支持在治疗种植体周黏膜炎时常规使用辅助疗法。值得注意的是,对于吸烟者来说,没有一种辅助疗法能明显改善临床效果[45]

3.2  种植体周炎的非手术治疗

种植体周炎的一线疗法为微创的非手术治疗,目的是减少微生物负荷,减轻局部炎症,同时保护种植体和周围骨组织[53-54]。其核心方法是使用钛刮治器、塑料或者碳纤维的超声波刮治器或喷砂装置等不损伤种植体表面的器械进行清创,而当螺纹暴露时,可使用钛刷等工具。辅助疗法,如化学制剂(氯己定、过氧化氢、生理盐水或碘制剂等)、局部或全身应用抗菌药物,可能有助于减少微生物负荷[14,55-56]。激光(如Nd:YAG激光、CO2激光、半导体激光、Er激光、Cr:YSGG和Er:YAG激光)和光动力疗法也可用于种植体表面去污。此外,患者教育至关重要,应强调正确使用牙刷和邻面清洁工具(牙线和牙间隙刷)来控制菌斑。每3~4个月复查并进行维护对于监测病情并及时调整治疗计划至关重要。然而,如无好转或疾病复发,则可能需要手术治疗。

3.3  种植体表面清创方法和骨再结合潜力

3.3.1  种植体表面清创方法及其疗效

种植体表面清创对消除种植体周炎症和实现骨再结合是必不可少的[57]。理想的清创应在不遗留材料残余且不改变种植体宏观和微观形态的情况下,有效地去除菌斑和牙石,但目前并无标准的种植体表面清创方法(共识)。

目前最常用的种植体表面清创方法是采用各种手工或者超声刮治器进行清创,但通常会造成种植体表面损伤或材料残留。喷砂、Er:YAG激光(能量低于70 MJ/脉冲)、电化学处理和冷常压等离子体既能大量减少生物膜,又能将对种植体表面的损伤降到最低,但高水平证据有限[46]。值得注意的是,仅使用全身抗菌药物无法实现充分的种植体表面清创。另外,在种植体周炎的手术治疗中,通常会用到种植体成形术,种植体成形术是治疗伴有骨缺损的种植体周炎的有效辅助手段(多数同意)。

3.3.2  种植体周的骨再结合

骨再结合是指骨-种植体界面的重建,可以在适当的清创后实现,但其报告值差异很大(9%~85%),垂直骨增量从最小值(<1 mm)到超过2 mm不等[46]。值得注意的是,喷砂和激光清创通常会促进良好的骨再结合,但单独采用种植体成形术、聚醚醚酮包被的超声工作尖和钛刷的疗效尚缺乏高级别证据。辅助使用化学制剂(氯己定、柠檬酸、过氧化氢、次氯酸钠、乙二胺四乙酸)并不能提高骨再结合[46]

3.4  种植体周炎的非再生性手术治疗

3.4.1  术式

1)翻瓣术:翻瓣术为种植体表面清创提供了入路。通常翻全厚瓣,可做垂直切口。可同期或分阶段进行前庭沟加深术、角化龈增宽术等膜龈手术。2)骨切除术:当种植体周为不利骨缺损时,通常需要进行骨切除手术。此时,翻瓣范围通常大于单纯的翻瓣术,可增加垂直切口,以获得良好的手术视野。通常需要根向复位,所以与翻瓣术相比,会暴露更多的种植体。

3.4.2  疗效

对于种植体周炎,在非手术治疗失败后,需要对有保留价值的种植体采用手术治疗,包括翻瓣术和骨切除术。两者都能减少PD和BOP,且没有显著差异[47]。唯一显著的区别在于边缘骨水平,翻瓣组更利于骨量保存。另外,吸烟和牙周炎病史会影响手术疗效。

3.5  种植体周炎的再生性手术治疗

3.5.1  术式

无论是否有垂直切口,都应采用全厚瓣入路暴露种植体周缺损,然后去净肉芽组织,并对种植体表面进行彻底的清洁。可以使用自体骨移植或骨替代材料来填充骨缺损。在可能的情况下,应尽量将种植体完全埋入(共识)。屏障或膜的应用虽然显示出积极趋势(平均增加0.56 mm)[48],但影响较小,然而专家们还是推荐在骨移植物上覆盖膜(一致同意)。瓣可根据手术需要复位于不同位置。对于种植体周黏膜过薄者,应考虑同期行软组织增量。埋入式愈合可能效果更佳,但尚待进一步确认。当种植体周骨丧失严重、骨结构或形态不良、种植体周炎反复发作或种植体松动时,应考虑拔除种植体。

3.5.2  疗效和治疗建议

与非再生性手术相比,再生性手术旨在获得骨增量并获得种植体周的骨再结合[58-59],疗效更佳[60-62]。因此,可能的情况下,尽量采用再生性手术(共识),尤其是出现有利骨缺损时,强烈建议采用再生性手术(强烈共识)。但笔者提醒,再生性手术有很高的技术敏感性,受多种因素影响,其中种植体表面的彻底清洁和术区的彻底清创是关键步骤,而术后良好的患者自我菌斑控制是手术愈合的重要前提,因此要严格掌握其适应证。无论采用哪种治疗方式,随着时间的推移,这些临床参数都会出现轻微但具有统计学意义的复发。基线PD、MBL和角化龈宽度都和预后密切相关,而缺损形态并非关键因素。

3.6  软组织手术

本共识没有进行系统评价来回答有关软组织管理的问题,但对以下关键问题进行了投票。1)手术时机:软组织手术可与种植体周炎手术治疗同期进行或在其后进行,具体视病例情况而定(共识)。2)适应证:再生性手术和非再生性手术的软组织修复适应证不同(多数同意)。当主要治疗目的是增加角化龈时,可考虑游离龈移植(共识),也可考虑使用软组织替代物(多数同意)。当主要治疗目的是增加种植体周软组织的宽度或厚度时,可考虑自体结缔组织移植(强烈共识),也可考虑使用软组织替代物(一致同意)。当主要治疗目的是加深前庭沟时,可考虑自体软组织移植(共识),也可考虑使用软组织替代物(多数同意)。3)手术方法:基于自体结缔组织移植的方法是种植体周所有软组织移植手术的标准方法(共识)。治疗美学区的种植体周炎时,可考虑结合手术测量进行结缔组织移植(共识)。

3.7  种植体周维护治疗(supportive peri-implant maintenance therapy,SPiT)

SPiT主要包括:1)患者口腔卫生检查和健康教育;2)种植体及其周围组织的检查,应使用种植体专用器械,评估软组织和种植体基台界面,并对边缘骨的稳定性进行影像学监测;3)风险评估;4)必要时进行治疗。SPiT对保持种植体周的长期健康至关重要。其复诊间隔应根据患者的整体风险状况确定。

本共识没有进行任何分析研究来回答相关问题,但对关键问题进行了投票,总结如下:1)对于接受过种植体周黏膜炎治疗的患者,SPiT的间隔时间应更短(共识),高风险者建议每3~4个月进行1次SPiT(共识),低风险者SPiT间隔时间为5~6个月(共识);2)对于接受过种植体周炎治疗的患者,建议SPiT间隔时间为3~4个月(共识),也可稍微延长,但每5~6个月复诊1次是最低要求;3)治疗第一年后应重新评估并根据患者情况制定个性化的SPiT间隔(共识);4)规律且有效的患者自我菌斑控制是预防PID和维护种植体长期健康的关键(共识)。

3.8  其他共识问题和投票结果——与系统评价不一致

另外,还有一些与系统评价的结果不一致、但专家们仍然很倾向的观点,包括:1)种植体周黏膜炎是发生种植体周炎的诱因/易感因素:强烈共识(97%同意);2)软组织表型对种植体周黏膜炎的风险有影响:共识(94%同意)——结果与第 1 组投票略有不同;3)钛的降解可能是PID发生、发展的原因之一:多数同意(61%同意);4)微生物生物膜是PID的主要致病因素:共识(91%同意)。

3.9  PID的治疗流程总结

种植体周炎的具体治疗流程,在图2中进行了详细总结。

4  展望和总结

虽然本次会议对很多问题达成了共识,但仍有以下问题尚待解决。因此,专家们建议未来应:1)开展设计良好的纵向研究,以确定新的危险因素并评估原有危险因素的作用;2)进行适当规模的RCT,至少随访1年,以确定非手术和手术治疗PID的疗效;3)通过临床研究确定辅助疗法的效果;4)确定种植体设计和表面特征对PID疗效的影响;5)确定种植体周炎不同治疗方法的复发率、种植体周组织稳定性和长期成功率;6)采用合适的参数组合为PID分级提供基于证据的定义;7)评估不同疗法治疗PID的成本效益;8)探讨在严重病例中种植体拔除的标准;9)开发高灵敏度和高特异性的工具用于PID的诊断和监测;10)制定标准化的种植体周炎治疗的结果衡量标准,包括影像学骨水平变化、软组织迁移、疾病缓解、患者满意度和黏膜退缩等;11)强烈推荐以上原创研究提高原始数据可用性;12)建议将这些结果纳入研究生课程。

PID尤其是种植体周炎是一项重大的临床挑战。临床医生应识别并分析每个患者的危险因素,并据此制订合理的个性化治疗方案。治疗方面,种植体周黏膜炎的治疗仍以非手术清创为主;而种植体周炎通常可能需要手术,应尽可能行再生性手术,以期获得种植体周的骨再结合,但治疗时应严格掌握其适应证。无论是何种PID或种植体周状况,其治疗后的长期维护都至关重要。另外,临床医生应时刻牢记,PID的病因是菌斑,因此,患者的自我菌斑控制至关重要,应贯穿治疗始终。
