摘要

在老年人群中,高血压和衰弱都有很高的流行率,两者常同时存在并相互影响,对老年人的健康和生活质量构成共同威胁。目前的主流医学共识认为,在启动老年高血压患者的降压治疗前,必须进行衰弱评估。然而,针对老年高血压合并衰弱的药物治疗,目前尚无标准化的方案,其核心原则是治疗目标的个体化和用药策略的个性化。各类降压药对衰弱的影响尚存争议,而其他类别药物的疗效与安全性也仍在研究中。中医药治疗或可成为破解该领域治疗瓶颈的突破口。

Abstract

Hypertension and frailty are both highly prevalent conditions in the elderly population. They often coexist and mutually exacerbate each other, posing a joint threat to the health and quality of life of older adults. Current mainstream medical consensus holds that frailty assessment must be conducted before initiating antihypertensive therapy in elderly hypertensive patients. However, there is no standardized protocol for the pharmacotherapy of hypertension combined with frailty in the elderly. The core principles are individualized treatment goals and personalized medication strategies. The impact of various antihypertensive drugs on frailty remains controversial, and the efficacy and safety of other drug classes are still under investigation. Traditional chinese medicine treatment may serve as a breakthrough to address the therapeutic bottlenecks in this field.
