摘要

目的 探讨本体感觉神经肌肉促进技术 (PNF) 对老年股骨颈骨折患者髋关节置换术后关节功能和平衡能力的影响。方法 采用方便抽样法选取2022年10月—2023年12月南京医科大学附属江宁医院收治的老年股骨颈骨折并行髋关节置换术患者100例, 随机数字表法分为对照组和观察组, 每组各50例, 对照组采用常规康复训练, 观察组在对照组的基础上增加PNF。分别于术后1周、2周、4周采用Harris评分评估关节功能, 采用Berg平衡量表 (BBS) 评估平衡能力, 采用起立—步行计时评估行走能力。结果 术后1周、2周、4周, 观察组Harris评分、BBS评分均高于对照组 (P<0.05); 观察组行走时间短于对照组 (P<0.05)。且两组术后2周、4周Harris评分、BBS评分均高于术后1周, 行走时间均短于术后1周 (P<0.05), 且术后4周较术后2周差异均有统计学意义 (P<0.05)。结论 在老年股骨颈骨折患者髋关节置换术后康复中, PNF能够有效促进关节功能恢复与平衡能力提升, 为临床康复治疗提供了一种有效的方法。

Abstract

Objective To investigate the effect of proprioceptive neuromuscular facilitation (PNF) on joint function and balance ability in elderly patients with femoral neck fracture after hip arthroplasty. Methods A total of 100 elderly patients with femoral neck fracture who underwent hip replacement in Jiangning Hospital Affiliated to Nanjing Medical University from October 2022 to December 2023 were selected by convenient sampling method. They were divided into control group (n = 50) and observation group (n= 50) by random number table method. The observation group was added PNF on the basis of the control group. The patients were evaluated at 1 week, 2 weeks and 4 weeks after operation. Harris score was used to evaluate joint function, Bery balance scale (BBS) score was used to evaluate balance ability, and walking ability was evaluated by timed up and go. Results At 1 week, 2 weeks and 4 weeks after operation, Harris score and BBS score in observation group were higher than those in control group (P<0.05); walking time in observation group was shorter than that in control group (P<0.05). And Harris score and BBS score at 2 and 4 weeks after operation in both groups were higher than those at 1 week after operation, walking time was shorter than that at 1 week after operation (P<0.05), and there were significant differences at 4 weeks after operation compared with 2 weeks after operation (P<0.05). Conclusion In the rehabilitation of elderly patients with femoral neck fractures after hip replacement surgery, PNF can effectively promote the recovery of joint function and the improvement of balance ability, providing an effective method for clinical rehabilitation treatment.
