摘要

背景 中轴型脊柱关节炎(axial spondyloarthritis,axSpA)的主要表现是腰背痛以及脊柱活动受限,严重影响生活质量,体力活动(physical activity,PA)是改善病情的重要干预手段之一。 目的 分析axSpA患者中PA强度与心理健康、生活质量、整体病情的关联,明确中高强度体力活动(moderate-to-vigorous physical activity,MVPA)的临床意义。 方法 本研究为横断面调查研究,纳入2024年8月— 2025年3月于解放军总医院第一医学中心风湿免疫科门诊部与住院部就诊的axSpA患者,采用华为手环7监测7天PA。以7天MVPA ≥ 150 min为达标组,<150 min为未达标组。使用焦虑自评量表(self-rating anxiety scale,SAS)、抑郁自评量表(self-rating depression scale,SDS)、多维度疲劳评估量表(multidimensional assessment of fatigue,MAF)、匹兹堡睡眠质量指数(Pittsburgh sleep quality index,PSQI)评估心理状态,使用健康指数(Assessment of Spondyloarthritis International Society Health Index,ASAS-HI)评估生活质量,使用疾病活动度评分(axial spondyloarthritis disease activity score,ASDAS)、疾病活动指数(Bath ankylosing spondylitis disease activity index,BASDAI)、功能指数(Bath ankylosing spondylitis functional index,BASFI)、活动衡量指数(Bath ankylosing spondylitis metrology index,BASMI)评估病情。采用logistic回归分析PA与ASDAS、ASAS-HI的关联关系。以周体力活动量(MVPA volume)为自变量,ASDAS、BASFI为中介变量,SAS、SDS为因变量,年龄、性别、体质量(body mass index,BMI)为协变量,构建结构方程模型,探索ASDAS和BASFI在PA与心理健康的关联之间的中介效应。 结果 共纳入166例axSpA患者,其中男性133例,女性33例,中位年龄35(IQR:29 ~ 40)岁。中位MVPA为19.4(IQR:11.6 ~ 30.2) min/d,PA达标75例(45.2%)、不达标91例(54.8%)。PA达标与较低的SAS(P<0.001)、SDS(P<0.001)、MAF(P<0.001)、PSQI(P=0.019)、ASAS-HI(P<0.001)、ASDAS (P=0.02)、BASDAI(P=0.002)、BASFI(P<0.001)、BASMI(P=0.015)均显著相关。多元logistic回归分析显示,MVPA与疾病活动评分(OR=0.96,P=0.002)以及生活质量(OR=0.97,P=0.021)独立关联。修正的结构方程模型拟合优秀,结果显示,ASDAS在MVPA volume与SAS、SDS之间的总中介效应占总效应的35.4%(SAS)和35.8%(SDS)。 结论 MVPA达标(≥150 min/w)的axSpA患者心理状态更健康,生活质量更高,疾病活动度更低,功能状态更好。建议运动处方以中强度体力活动为基础、高强度体力活动为补充。

Abstract

Background Axial spondyloarthritis (axSpA) is characterized primarily by low back pain and restricted spinal mobility, affecting quality of life severely. Physical activity (PA) serves as one of the key interventions for improving disease outcomes. Objective To analyze the association between objectively quantified PA intensity and mental health, quality of life, and overall disease status, and clarify the clinical significance of moderate-to-vigorous physical activity (MVPA). Methods This cross-sectional study enrolled axSpA patients attending the outpatient and inpatient departments of Rheumatology and Immunology at the First Medical Center of PLA General Hospital from August 2024 to March 2025. PA was monitored for 7 days using the Huawei Band 7. Participants were stratified into "guideline-met" group (≥150 min/week of MVPA) and "guideline-unmet" group (<150 min/week). Mental health was assessed using the self-rating depression scale (SDS), self-rating anxiety scale (SAS), multidimensional assessment of fatigue (MAF), and Pittsburgh sleep quality index (PSQI). Quality of life was evaluated using the Assessment of Spondyloarthropathy International Society Health Index (ASAS-HI). Disease status was assessed using the ankylosing spondylitis disease activity score (ASDAS), Bath ankylosing spondylitis disease activity index (BASDAI), Bath ankylosing spondylitis functional index (BASFI), and Bath ankylosing spondylitis metrology index (BASMI). Logistic regression analysis was used to examine the associations between PA and ASDAS, and between PA and ASAS-HI, respectively. A structural equation model (SEM) was constructed with weekly MVPA volume as the independent variable, ASDAS and BASFI as mediators, and SDS and SAS as dependent variables, adjusting for age, sex, and body mass index (BMI), so as to explore the mediating effects of ASDAS and BASFI on the association between PA and mental health. Results A total of 166 axSpA patients were included, comprising 133 males (80.1%) with a median age of 35 (IQR: 29 - 40) years. The median daily MVPA was 19.4 (IQR: 11.6 - 30.2) min. Totally 75 patients (45.2%) met the PA guidelines, while 91 (54.8%) did not. Meeting PA guidelines was significantly associated with lower scores on SAS (P<0.001), SDS (P<0.001), MAF (P<0.001), PSQI (P=0.019), ASAS-HI (P<0.001), ASDAS (P=0.02), BASDAI (P=0.002), BASFI (P<0.001), and BASMI (P=0.015). Multivariable logistic regression analysis revealed that MVPA was independently associated with disease activity scores (OR=0.96, P=0.002) and quality of life (OR=0.97, P=0.021). The revised structural equation model showed a good fit. Mediation analysis showed that the total mediating effect of ASDAS accounted for 35.4% (SAS) and 35.8% (SDS) of the total effect between MVPA volume and mental states, while the total mediating effect of BASFI accounted for 33.2% (SAS) and 32.8% (SDS). Conclusion AxSpA patients achieve the recommended MVPA level (≥150 min/week) exhibit a healthier comprehensive mental status, higher quality of life, lower disease activity, and better functional status. Exercise prescriptions should be based on moderate-intensity physical activity (MPA) and supplemented by vigorous-intensity physical activity (VPA).

中轴型脊柱关节炎(axial spondyloarthritis,axSpA)是一种以脊柱和骶髂关节慢性炎症为主要特征的慢性风湿性疾病,临床表现为持续性腰背痛、晨僵、脊柱活动受限及功能障碍,还常伴随焦虑、抑郁、疲劳、睡眠障碍等心理症状,使患者面临生理、心理双重挑战[1-5]。目前,axSpA主要依赖非甾体抗炎药、生物制剂等药物干预,但往往无法完全控制疾病进展,且存在不良反应[6]。多项研究证实,规律的PA可能通过调节炎症反应、改善关节功能和增强肌肉力量来控制疾病活动、改善功能状态和提升生活质量[7-8]。因此,探索有效的非药物干预措施,特别是体力活动(physical activity,PA),对于改善axSpA患者的长期预后具有重要意义。
现有研究存在诸多局限,PA评估方法多依赖于自我报告,多集中于体力活动的总体水平与健康结果的关联,缺乏对中强度体力活动(moderate physical activity,MPA)和高强度体力活动(vigorous physical activity,VPA)分别作用的深入探讨[9-10]。此外,关于PA、心理健康、病情及整体健康的相互关系研究较为有限。因此,本研究通过横断面研究,采用智能手环客观监测PA,定量分析不同强度PA与axSpA患者心理、生活质量、疾病活动度及功能状态的相关性,为axSpA患者的临床管理提供更精确的依据。

1 对象与方法

1.1 研究对象与数据采集

本研究采用单中心、横断面设计。2024年8月— 2025年3月连续招募于风湿免疫科门诊部与住院部就诊的axSpA患者。纳入标准:(1)符合2009年国际脊柱关节炎评估协会分类标准;(2)年龄18 ~ 65岁;(3)有一定阅读理解能力。排除标准:(1)合并严重心、肝、肾功能不全和血液系统疾病等;(2)合并其他风湿免疫性疾病。研究方案经解放军总医院第一医学中心医学伦理委员会批准(批号:S2022-738-02),所有调查对象均知情同意,且自愿参与本研究。

1.2 PA数据及其他指标采集流程

为评估患者的真实体力活动水平,本研究采取先监测、后评估的分阶段数据采集模式。

设备发放(第0天):研究者对符合纳入标准的患者进行初步筛查,指导其佩戴校准后的智能手环[11-12](华为手环7),嘱其在8 d内保持原有生活与运动习惯,不刻意改变行为模式。

手环适应(第0 ~ 1天):后台不收集体力活动数据,旨在让患者熟悉设备佩戴并逐渐忽略监测,以确保7天数据准确。

体力活动监测(第2 ~ 8天):患者连续佩戴智能手环7 d(含工作日与休息日),此期间不收集临床结局变量。

临床资料与结局评估(第9天):患者返院,回收智能手环并导出体力活动数据,同时通过医院电子病历系统及脊柱关节炎智能管理系统[13]提取临床数据,通过标准化量表评估心理状态,由经过培训的研究员现场采集人口学信息。

1.3 暴露因素及协变量

本研究分析的暴露因素是PA。使用智能手环和问卷采集VPA、MPA、中高强度体力活动(moderate-to-vigorous physical activity,MVPA)、睡眠(sleep,SLP)和久坐(sedentary behaviour,SED)时长(单位min/d)指标。其中,VPA、MPA与SLP由智能手环的心率、加速度传感器结合算法判定,MVPA=VPA+MPA,SED由问卷获取[14],参照量表计算一周MVPA总量,即MVPA volume = (MPA × 4+ VPA × 8)× 7/60 (单位MET·h/w)[15-16]

根据欧洲抗风湿病联盟(European Alliance of Associations for Rheumatology,EULAR)指南推荐,将每周MVPA ≥ 150 min归为达标组,<150 min归为未达标组,进行组间比较,计算统计值。

协变量包括年龄、性别、体质量指数(body mass index,BMI)、婚姻状况、吸烟史、饮酒史、就业状况、经济状况等指标。

1.4 评价指标

1.4.1 心理健康和生活质量

焦虑情绪、抑郁程度、疲劳感、睡眠状况和生活质量分别由焦虑自评量表(Zung’s self-rating anxiety scale,SAS)、抑郁自评量表(Zung’s self-rating depression scale,SDS)、多维度疲劳评估量表(multi-dimensional assessment of fatigue,MAF)、匹兹堡睡眠质量指数(Pittsburgh’s sleep quality index,PSQI)和国际脊柱关节炎评估协会健康指数(assessment of spondyloarthritis international society health index,ASAS-HI)评估。

1.4.2 病情

疾病活动度指标通过中轴型脊柱关节炎疾病活动度评分(axial spondyloarthritis disease activity score,ASDAS)、巴斯强直性脊柱炎疾病活动指数(Bath ankylosing spondylitis disease activity index,BASDAI)评估,功能状态指标通过巴斯强直性脊柱炎功能指数(Bath ankylosing spondylitis functional index,BASFI)、巴斯强直性脊柱炎活动衡量指数(Bath ankylosing spondylitis metrology index,BASMI)测量,此外还包括病程、HLA-B27、指地距等指标。

对ASDAS以2.1为界分组、ASAS-HI以6为界转换为二分类变量,纳入暴露因素和协变量分别进行单因素、多因素logistic回归分析,探索独立关联因素。

1.5 统计学分析

采用R 4.5.2软件进行统计学分析。计量资料经Shapiro-Wilk检验符合正态分布时以均值±标准差表示,组间比较使用独立样本t检验,否则以中位数(四分位距)表示,采用Mann-Whitney U检验;计数资料以频数(百分比)表示,组间比较采用卡方检验,若任何单元格期望频数<5则使用Fisher精确检验;所有统计检验均为双侧检验,以P<0.05为差异有统计学意义。使用Spearman相关性分析探索PA与其余指标的关系。将检验水准设定为P<0.15,对ASDAS和ASAS-HI分别进行单因素、多因素logistic回归分析。参照前人研究及Spearman相关性分析结果,假设体力活动量直接及间接影响心理状态路径,考虑到潜在的双向影响和误差项相关问题,纳入变量之间的残差方差项,采用lavaan构建含协变量的结构方程模型,使用BCa自助法估计参数(bootstrap=5 000),报告标准化路径系数及拟合指标。

2 结果

2.1 axSpA患者一般资料

本研究共纳入166例axSpA患者,男性133例,女性33例,以中青年为主,中位年龄为35(IQR:29 ~ 40)岁,中位BMI为24.9(IQR:22.3 ~ 27.0) kg/m2。见表1

2.2 axSpA患者病情

病情方面,axSpA患者中位病程为9.2(IQR:4.4 ~ 16.3)年,HLA-B27阳性率为84.9%(141/166)。疾病活动度方面,ASDAS中位得分为1.6(IQR:1.1 ~ 2.4)分,高疾病活动度评分(ASDAS≥2.1)占比34.3%(57/166);BASDAI中位得分为2.2(IQR:1.3 ~ 3.6)分,高疾病活动度指数(BASDAI≥4)占比20.5%(34/166)。活动功能方面,BASFI中位得分为1.4(IQR:0.5 ~ 2.9)分,功能受限(BASFI≥4)占比18.1%(30/166);BASMI中位得分为1.0(IQR:0 ~ 3.0)分,脊柱结构损伤(BASMI≥4)占比23.5%(39/166);指地距中位数为10.0(IQR:0 ~ 22.0) cm。见表1

2.3 axSpA患者心理健康和生活质量

心理健康方面,SAS中位得分为42.0(IQR:36.0 ~ 49.5)分,焦虑状态(SAS≥50)占比25.3%(42/166),其中轻度焦虑(50≤SAS≤59)占比16.3%(27/166),中度焦虑(60≤SAS≤69)占比7.2%(12/166),重度焦虑(SAS≥70)占比1.8%(3/166)。SDS为48.0(IQR:38.0 ~ 56.5)分,抑郁状态(SDS≥50)占比49.4%(82/166),其中轻度抑郁(50≤SDS≤59)占比31.9%(53/166),中度抑郁(60≤SDS≤69)占比11.4%(19/166),重度抑郁(SDS≥70)占比6.0%(10/166)。MAF为24.8(IQR:17.3 ~ 32.1)分,疲劳(MAF≥18.7)占比72.9%(121/166);PSQI为7.0(IQR:5.0 ~ 9.0)分,睡眠障碍(PSQI≥8)占比38.6%(64/166)。生活质量方面,ASAS-HI中位得分4(IQR:2 ~ 6)分,低生活质量(ASAS-HI≥6)占比33.7%(56/166)。见表1

2.4 axSpA患者MVPA情况及PA达标组与未达标组组间比较

166例患者MVPA中位时长为19.4(IQR:11.6 ~ 30.2) min/d,VPA为11.0(IQR:5.3 ~ 20.8) min/d,MPA为6.0(IQR:3.0 ~ 12.8) min/d,中位MVPA volume为13.95(IQR:8.71 ~ 24.01) MET·h/w。MVPA达标组75例(45.2%),未达标组91例(54.8%)。达标组男性及未婚比例更高,学历更高,差异均有统计学意义(P<0.05)。达标组的疾病活动、功能状态、生活质量和整体心理健康水平均显著优于未达标组(P<0.05)。见表1

2.5 MVPA时长与病情、心理状态、生活质量指标相关性分析

Spearman相关性分析显示,在心理健康方面,MVPA、VPA与SDS、SAS、MAF、PSQI均显著负相关(P<0.01),MPA与SAS、MAF、PSQI显著负相关(P<0.05)。在生活质量方面,MVPA、VPA、MPA与ASAS-HI显著负相关(P<0.05)。在整体病情方面,MVPA与ASDAS、BASDAI、BASFI、BASMI均显著负相关(P<0.05),MPA与ASDAS、BASDAI、BASFI显著负相关(P<0.01),VPA与ASDAS、BASDAI、BASFI、BASMI均显著负相关(P<0.01)。见图1

2.6 MVPA时长与疾病活动度和生活质量的关联分析

以ASDAS与ASAS-HI为因变量,MVPA时长和其他协变量为自变量,分别进行logistic回归分析。单因素分析显示MVPA、性别、BMI、吸烟史等与ASDAS相关(P<0.05)。多元logistic回归分析显示,MVPA与ASDAS独立关联(OR=0.961,P=0.002)(表2)。单因素分析显示MVPA、性别、BMI、经济状况与ASAS-HI相关(P<0.05),多元logistic回归显示MVPA(OR=0.971,P=0.021)和经济状况(OR=0.290,P=0.004)与ASAS-HI独立关联(表3)。

2.7 axSpA患者体力活动、病情和心理健康的结构方程模型构建

在既往文献研究[17-19]的基础上,参考Spearman相关性分析结果,提出假设:周体力活动量直接影响抑郁、焦虑状态,疾病活动度、功能指数分别在周体力活动量与抑郁、焦虑状态之间起中介作用。以周体力活动量(MVPA volume)为自变量,ASDAS、BASFI为中介变量,SAS、SDS量表得分为因变量,年龄、性别、BMI为协变量,考虑到双向影响以及潜在的遗漏测量偏差,故纳入ASDAS与MVPA volume残差协方差,考虑到误差项相关,故纳入SAS与SDS残差协方差,构建假设模型(图2)。初始模型存在不显著路径,在剔除后再次构建假设模型,适配度检验结果显示,χ2/df=1.563,拟合优度指数(gamma goodness-of-fit index,GFI)为0.965,比较拟合指数(comparative fit index,CFI)为0.983,增量拟合指数(incremental fit index,IFI)为0.984,非规范拟合指数(tucker-lewis index,TLI)为0.958,近似根平均平方误差(root mean square error of approximation,RMSEA)为0.058,结构方程模型拟合结果良好,因此在本次分析中,建立的结构方程模型(图3)基本达到稳定水平,不再进行模型修正。周体力活动量与疾病活动度的残差相关具有统计学显著性,但其标准化系数接近于零(βstd=0.001,P=0.001),表明两者间未被模型解释的双向共变效应微弱。

路径分析结果显示,周体力活动量对焦虑和抑郁的影响主要通过疾病活动度和功能指数的中介路径实现。具体而言,周体力活动量通过疾病活动度→功能指数→抑郁/焦虑的间接路径,以及疾病活动度→抑郁/焦虑的直接路径,共同构成了体力活动对心理健康的显著影响。其中,体力活动量对焦虑、抑郁的总效应分别是-0.33、-0.30,病情(疾病活动度+功能指数)对体力活动量→焦虑、抑郁路径的中介效应分别为44.6%、44.7%,疾病活动度的总中介效应占总效应的35.4%(焦虑)和35.8%(抑郁),功能指数的总中介效应占总效应的33.2%(焦虑)和32.8%(抑郁)(P<0.05)。

3 讨论

作为一种慢性炎症性疾病,axSpA以慢性炎症和关节僵硬为主要病理生理特征,可严重影响患者的生活质量[1]。axSpA患者心理障碍并发率高,近年来,全球axSpA管理趋势已从单纯的疾病控制转向全面的生活质量与心理状态优化[20]。PA作为非药物管理的核心要素,其与疾病活动度、功能状态、生活质量和心理健康的关联日益受到关注。本研究axSpA人群中位MVPA时长为19.4(IQR:11.6 ~ 30.2) min/d,45.2%达标,MVPA时长与整体心理健康和病情均显著相关,与疾病活动度、生活质量独立关联,MVPA总量通过双路径与心理状态关联。

通过对166例axSpA患者的客观监测,本研究证实了达到MVPA推荐量(≥150 min/w)与更低的疾病活动度、更好的功能状态及生活质量显著相关。未达标组病情重,可能阻碍达标锻炼,故体育锻炼需以规范的药物治疗为前提;反之,充分的MVPA可能利于控制病情、改善心理健康和生活质量,是药物治疗无法替代的补充[21]

生活质量(ASAS-HI)是axSpA管理的重要目标,多因素回归分析显示MVPA是其独立关联因素,这一发现与全球axSpA管理趋势一致[20]。在临床实践上,建议将ASAS-HI纳入患者教育模块,强调MVPA达标作为预防“生活失能”的策略。胡等[22]发现活动能力与性健康体验呈显著正相关,共同强调了优化运动干预在提升整体生活质量方面的临床价值。此外,经济稳定亦是提高生活质量的独立相关因素,提示其可能通过改善医疗资源可及性、心理压力水平或运动条件而间接提高生活质量。

在心理健康方面,MVPA和较低水平的焦虑、抑郁、疲劳和睡眠障碍显著相关。这种关联既可能通过降低BMI和改善疾病认知,减轻中央敏化现象,缓解疼痛感知而间接促进心理健康[19,23],也可能源于直接改善[24]

本研究发现VPA与良好的整体病情和心理健康的显著相关均强于MPA,提示其可能通过更高效的方式改善身心状况。axSpA以慢性炎症和关节僵硬为主要病理生理特征,炎症因子如TNF-α和IL-17的升高导致关节软骨破坏和纤维化,引发疼痛和功能受限[25]。MPA主要通过降低炎症因子水平、提高柔韧性,改善身体功能和生活质量,提高自我效能感和积极应对能力,缓解疾病对心理的负面影响[18,26]。VPA则显著增强肌肉力量和耐力,帮助延缓甚至改善功能障碍,长期坚持可有效提升全面身心状态[27]。但Zhang等[28]发现患者对PA认知不足和缺乏运动动机是限制其参与的主要障碍,强调了加强患者教育和个体化运动指导的重要性。但VPA可能对耐受能力较低的患者造成脊柱、关节的损伤[29]。因此,结合对心功能不全、脑卒中和多系统硬化症患者运动逐步增量的建议[30-32],我们认为对axSpA患者运动处方的设计必须阶梯化:MPA作为基础,VPA作为补充以优化功能增益,并且合理选择运动方式,如规避危险的对抗性运动。

本研究的路径分析揭示了MVPA改善axSpA患者心理健康的潜在机制,其核心在于明确了疾病活动度与功能指数所扮演的不同中介角色。结果表明,体力活动对抑郁和焦虑症状的积极影响主要通过降低疾病活动度来实现,其次通过改善功能指数来实现。这提示,体力活动可能首先通过直接的抗炎等生物学途径有效抑制疾病的核心进程,进而一方面直接缓解由疾病活动本身引发的情绪困扰,另一方面通过改善躯体功能间接促进心理健康。这一发现对临床实践具有明确指导意义:将降低疾病活动度视为改善其心理状态的优先靶点,在制定康复计划时,不仅应鼓励患者进行体力活动以维持功能,更需向患者充分阐释其在控制内在病情、从而从源头改善情绪方面的关键价值,这有助于增强患者的治疗信心与依从性,实现身心双重获益。

本研究的主要优势在于:客观测量设备真实可靠,减少了传统问卷的回忆偏倚[33],证实MVPA与整体病情和心理健康的显著相关性,聚焦于不同运动强度的差异化效应,并解析了疾病活动度和功能指数对体力活动影响心理健康的中介作用。本研究的局限性在于:尽管在结构方程模型中使用了残差相关来处理潜在的双向性问题,但横断面设计不能确立因果方向,手环监测可能低估非步态活动(如骑车),未控制药物使用、社会支持等混杂因素,未涉及分子、神经机制,可能削弱结果的普适性与临床转化价值,未来研究需开展随机对照试验以验证运动干预的动态效应。

综上,MVPA作为axSpA非药物管理的核心要素,达标运动有助于改善整体病情与心理健康,推荐所有患者加强MPA,对可耐受的患者逐步引入VPA,保持MVPA时长≥150 min/w。通过结构方程模型揭示的体力活动改善心理健康的“双路径机制”,为把运动干预纳入axSpA多维度管理策略提供了更多循证依据,即单纯控制炎症无法充分解决心理负担,而将功能状态监测纳入运动干预评估体系(在疾病活动度改善后重点关注功能状态的提升),方能进一步提升心理获益。
