从胃肠积热态场论治小儿遗尿症经验

Managing pediatric enuresis through the lens of the gastrointestinal heat retention pattern field theory

摘要

小儿遗尿，虽多责之于膀胱约束无权，然其论治传统多囿于脾肾不足、肺失宣肃、膀胱虚寒等内因。团队根据多年临床经验及调查研究，提出创新性观点：当今小儿遗尿，多由饮食热量过高、肥甘厚腻摄入无度、饮食不节等外因，结合小儿“阳常有余”“脾常不足”的生理特点，导致食积难消，酿生胃肠积热。此热里郁下移，扰动膀胱，遂致睡中遗尿。本文深刻阐明了胃肠与膀胱在水液代谢、气机升降及经络相通等方面的密切联系，不仅为临床实践提供了“从胃肠积热论治”的新思想依据，也为从饮食习惯入手改善患儿体质提供了切实可行的指导方向。

Abstract

The disease of enuresis in children is caused by the inability of the bladder to restrain itself， and its treatment mainly starts from the deficiency of spleen and kidney， the impairment of lung promoting and purifying function， the deficiency of bladder cold and so on. According to many years of clinical experience and clinical investigation， our team proposes an innovative perspective： modern pediatric enuresis is frequently caused by external factors—such as excessive caloric intake， overconsumption of rich， sweet， and greasy foods， and dietary irregularities—combined with the physiological characteristics of children， specifically “yang is often in excess” and “spleen is often in deficiency”. These factors lead to indigestion and food accumulation， which transforms into heat in the stomach and intestines. This heat stagnates internally and transmigrates downward， disturbing the bladder and resulting in bedwetting during sleep. This article profoundly clarifies the close relationship between the stomach / intestines and the bladder regarding fluid metabolism， qi movement （ascending and descending）， and meridian connectivity. It not only provides a new theoretical basis for clinical practice—“ treating enuresis by clearing heat from the stomach and intestines”—but also offers practical guidance for improving children’s constitutions through dietary intervention.
