摘要
骨质疏松症(osteoporosis,OP)作为最常见的年龄相关性代谢性骨病,其发病与骨内稳态失衡息息相关。自噬是维持细胞活力的重要过程,骨相关细胞自噬参与维持及调控骨内稳态。玄府是中医微观层面最基本的结构和功能单位,骨玄府开阖通利的性能是保持中医骨系统正常运行的先决条件。该文通过对细胞自噬与玄府在结构和功能上进行对比,认为骨玄府稳态失衡-自噬失调是OP发生的中医微观病机,并提出佐以风药辅助治疗OP,增强中医宏观层面上脏腑辨证相应的疗效,完成中医药治疗OP时微观层面与宏观层面的整合。
Abstract
As the most common age-related metabolic bone disease, osteoporosis (OP) is closely related to the imbalance of bone homeostasis. Autophagy is a crucial process for maintaining cellular vitality, and autophagy in bone-related cells is involved in maintaining and regulating bone homeostasis. Xuanfu (pore) is the most fundamental structural and functional unit at the microcosmic level in Traditional Chinese Medicine (TCM). The opening and closing of bone xuanfu to ensure smooth passage is a prerequisite for maintaining the normal functioning of the bone system in TCM. By comparing the structural and functional aspects of autophagy and xuanfu, this article posits that the imbalance of bone xuanfu homeostasis and autophagy dysregulation represent the microscopic pathogenesis of OP from a TCM perspective. Furthermore, it proposes using wind-dispelling herbs as adjunctive therapy for OP to enhance the efficacy of organ differentiation and pattern differentiation and treatment on a macroscopic level, thereby achieving integration between the microscopic and macroscopic dimensions in TCM treatment of OP.
