数字化导板引导口腔种植修复术治疗牙列缺损的美学效果及影响因素分析

数字化导板引导口腔种植修复术治疗牙列缺损的美学效果及影响因素分析
Analysis of Aesthetic Effects and Influencing Factors of Digital Guide Plate Guided Oral Implant Restoration Surgery for the Treatment of Dental Defects
目的 探讨数字化导板引导的口腔种植修复术治疗牙列缺损的美学效果及影响因素。 方法 采用前瞻性随机对照研究,选取2021年7月至2024年7月漯河市中心医院收治的166例牙列缺损患者,按随机数字表法分为研究组(83例,数字化导板引导种植修复)与对照组(83例,常规种植修复)。比较两组患者种植精确度、牙周健康指数、美学效果、并发症发生情况的差异,并通过线性回归方程分析术后美学效果的影响因素。 结果 研究组深度偏差[(0.61 ± 0.16) vs. (1.06 ± 0.25)] mm、颈部距离偏差[(0.69 ± 0.16) vs. (1.07 ± 0.24)] mm、角度偏差(3.05° ± 0.92°) vs. (6.59° ± 1.56°)、根尖部距离偏差[(0.86 ± 0.16) vs. (1.45 ± 0.16)] mm均显著小于对照组(P < 0.05)。术后1周,两组龈沟出血指数(Sulcus Bleeding Index,SBI)、牙龈指数、牙菌斑指数均显著降低(P < 0.05),研究组更低于对照组(P < 0.05)。术后6个月,两组白色美学评分(White Esthetic Score,WES)、红色美学评分(Pink Esthetic Score,PES)、总分均显著升高(P < 0.05),研究组更高于对照组(P < 0.05)。研究组并发症总发生率(4.82%)显著低于对照组(16.87%)(P < 0.05)。线性回归方程显示,年龄、治疗方式、术前SBI、术后1周SBI、术前牙菌斑指数、术后1周牙菌斑指数均为术后美学效果的影响因素(P < 0.05)。 结论 数字化导板引导口腔种植修复术治疗牙列缺损,可提高种植精确度,改善牙周健康指数,增强美学效果,降低并发症,具有临床推广价值。
Objective To explore the aesthetic effect of digital guide plate and analyze the influencing factors. Methods A prospective randomized controlled study was conducted on 166 dental defect patients from July 2021 to July 2024. Patients were divided into a study group (n = 83, digital guide plate guided surgery) and a control group (n = 83, conventional surgery). Implant accuracy, periodontal indices (SBI, gingival index, plaque index), aesthetic scores (WES, PES), and complications were compared. Linear regression was used to analyze factors influencing postoperative aesthetics. Results he study group showed significantly smaller deviations in depth [(0.61 ± 0.16) vs. 1.06 ± 0.25)] mm, cervical distance [(0.69 ± 0.16) vs. (1.07 ± 0.24)] mm, angle (3.05° ± 0.92°) vs. (6.59° ± 1.56°), and apical distance [(0.86 ± 0.16) vs. (1.45 ± 0.16)] mm (P < 0.05). At 1 week postoperatively, the study group had lower SBI, gingival index, and plaque index (P < 0.05). At 6 months, WES, PES, and total aesthetic scores were higher in the study group (P < 0.05). The total complication rate was lower in the study group (4.82% vs. 16.87%, P < 0.05). Age, treatment modality, preoperative SBI, 1-week postoperative SBI, and preoperative plaque index were independent predictors of aesthetic outcomes (P < 0.05). Conclusion Digital guide plate guided oral implant restoration surgery for dental arch defects can improve implant accuracy, improve periodontal health index, enhance aesthetic effect, and reduce complications. Age, treatment method, preoperative SBI, 1-week postoperative SBI, and preoperative plaque index are all factors affecting postoperative aesthetic effect,demonstrating significant clinical value.
