超声二维斑点追踪技术对严重烧伤患者左心功能的评估价值
Evaluation Value of Two-Dimensional Speckle-Tracking Echocardiography for Left Heart Function in Patients with Severe Burns
目的 探讨超声二维斑点追踪技术对严重烧伤患者左心功能的评估价值。 方法 选取2022年6月至2024年6月郑州市第一人民医院67例严重烧伤患者为严重烧伤组(简称烧伤组),同时通过简单随机抽样法选择67例健康体检者为健康组,应用超声二维斑点追踪技术分析两组患者一般参数、左心室整体应变参数、左心室纵向应变(left ventricular longitudinal strain,LVLS)及组织运动瓣环位移(tissue motion annulus displacement, TMAD)的检测结果。 结果 烧伤组左室质量指数(left ventricular mass index, LVMI)、左心室后壁厚度(left ventricular posterior wall thickness, LVPWT)、左心室舒张末期室间隔厚度(left ventricular end diastolic interventricular septal thickness, LVEDIST)分别为(73.63 ± 6.91)g/m2、(8.64 ± 0.88)mm、(8.86 ± 0.95)mm,健康组LVMI、LVPW、LVEDIST分别为(75.59 ± 7.12)g/m2、(8.71 ± 0.73)mm、(8.82 ± 0.74)mm,差异无统计学意义(P > 0.05)。烧伤组左心室整体圆周应变(global circumferential strain, GCS)、整体纵向应变(global longitudinal strain, GLS)、整体径向应变(global radial strain, GRS)均低于健康组,差异有统计学意义(P < 0.05)。烧伤组左心室二尖瓣下壁纵向应变(LVLSIN)、左心室二尖瓣后壁纵向应变(LVLSIL)、左心室二尖瓣侧壁纵向应变(LVLSAL)、左心室二尖瓣前壁纵向应变(LVLSAN)、左心室二尖瓣前间隔纵向应变(LVLSAS)、左心室二尖瓣后间隔纵向应变(LVLSIS)均低于健康组,差异有统计学意义(P < 0.05)。烧伤组TMADIN、TMADIL、TMADAL、TMADAN、TMADAS、TMADIS、二尖瓣环整体位移(TMADglobal)均低于健康组,差异有统计学意义(P < 0.05)。 结论 超声二维斑点追踪技术可用于评估严重烧伤患者左心功能,并为临床安全有效治疗提供重要依据。
Objective The purpose of this study was to explore the value of two-dimensional speckle-tracking echocardiography in assessing left heart function in patients with severe burns. Methods A total of 67 severely burned patients from the First People’s Hospital of Zhengzhou from June 2022 to June 2024 were selected as the severe burn group. At the same time, a simple random sampling method was used to select 67 healthy individuals as the healthy group. 2D-STE technology was applied to analyze the global parameters, left ventricular global strain parameters, left ventricular longitudinal strain (LVLS), and tissue motion annulus displacement (TMAD) of the two groups of patients. Results The left ventricular mass index (LVMI), left ventricular posterior wall thickness (LVPW), and left ventricular end diastolic interventricular septal thickness(LVEDIST) in the severe burn group were (73.63 ± 6.91) g/m2, (8.64 ± 0.88) mm, and (8.86 ± 0.95) mm, respectively, while those in the healthy group were (75.59 ± 7.12) g/m2, (8.71 ± 0.73) mm, and (8.82 ± 0.74) mm, respectively, with no statistically significant difference (P > 0.05). The global circumferential strain (GCS), global longitudinal strain (GLS), and global radial strain (GRS) of the left ventricle in the severe burn group were all lower than those in the healthy group, and the differences were statistically significant (P < 0.05). The longitudinal strain of the inferior wall of the left ventricular mitral valve (LVLSIN), the longitudinal strain of the posterior wall of the left ventricular mitral valve (LVLSIL), the longitudinal strain of the lateral wall of the left ventricular mitral valve (LVLSAL), the longitudinal strain of the anterior wall of the left ventricular mitral valve (LVLSAN), the longitudinal strain of the anterior septum of the left ventricular mitral valve (LVLSAS), and the longitudinal strain of the posterior septum of the left ventricular mitral valve (LVLSIS) in the severe burn group were all lower than those in the healthy group, and the differences were statistically significant(P < 0.05). In the burn group, TMADIN, TMADIL, TMADAL, TMADAN, TMADAS, TMADIS, and the global mitral annular displacement(TMADglobal) were all significantly lower than those in the healthy control group(P < 0.05). Conclusion Two-dimensional speckle-tracking echocardiography could be used to assess left heart function in patients with severe burns, providing critical evidence for safe and effective clinical treatment.
