多能干细胞在肺纤维化治疗和研究中的应用

摘要

肺纤维化(pulmonary fibrosis,PF)是一种慢性、进展性肺病,现有疗法仅能一定程度缓解临床症状,但不能逆转纤维化进展乃至治愈该病。对比传统的PF模型与药物筛选系统,多能干细胞衍生的模型和系统可以更好模拟纤维化生理特性及微环境,也可为后续纤维化的治疗提供新的手段。综述基于多能干细胞的PF模型与药物筛选系统的贡献以及治疗研究进展,并探讨其临床转化过程中面临的挑战及解决方案。

Abstract

Pulmonary fibrosis (PF) is a chronic, progressive lung disease for which current treatments only partially alleviate clinical symptoms but cannot reverse fibrosis progression or cure the disease. Compared with traditional models and drug screening systems for pulmonary fibrosis, models and systems derived from pluripotent stem cells can better simulate the physiological characteristics and microenvironment of fibrosis, and also provide new means for subsequent treatment of fibrosis. This review summarizes the contributions of pluripotent stem cells‑based models and drug screening systems for pulmonary fibrosis and their progress in treatment research, and discusses the challenges and solutions for their clinical translation.

肺间质纤维化(pulmonary fibrosis, PF),是一种慢性、进行性的肺部疾病,主要表现为肺间质的炎症和纤维化。这种疾病会导致肺组织逐渐硬化,失去弹性,进而影响肺部的正常功能。肺纤维化对患者的生活质量和生存期都有严重影响。随着肺功能的逐渐减退,患者会出现呼吸衰竭、肺心病等严重并发症,甚至导致死亡1。近年来,针对肺纤维化的治疗研究取得了新的进展,尤其是干细胞治疗等新型治疗方法的涌现,为患者带来了希望。其中,多能肝细胞(pluripotent stem cells,PSC)因其独特的生物学特性,在PF的治疗中显示出巨大潜力。PSC不仅能够修复损伤的组织,还具备免疫调节功能,可以减轻炎症反应,同时还能发挥抗纤维化作用,阻止病情的进一步发展2。此外,基于PSC建立的肺纤维化模型和药物筛选系统,能够更准确地模拟纤维化的生理特性和微环境,为深入研究PF的发病机制和寻找有效治疗药物提供了有力的工具。这些模型和系统的应用,不仅推动了PF的基础研究的进展,也为后续临床治疗提供了新的思路和手段。

1 PSC的生物学基础

PSC是未分化的前体细胞,具有无限自我更新和多项分化潜能两个基本特性,它包括了胚胎干细胞(embryonic stem cell,ESC)和诱导多能干细胞(induced pluripotent stem cell,iPSC)。ESC来源于囊胚期胚胎的内细胞团,1981年,科学家首次成功分离小鼠ESC,此后在1998年,James Thomson团队成功构建出世界首份人类胚胎干细胞系(human embryonic stem cell,hESC)3。而iPSC是人工诱导的PSC,最早是由日本科学家山中伸弥等于2006年向小鼠的成纤维细胞中引入了4个基因(Oct4Sox2Klf4c‑Myc),成功地诱导这些细胞变成了PSC。这些细胞后来被命名为iPSC。2007年这一技术迅速被应用于人类细胞,成功诱导出人源诱导多能干细胞4。这种技术通过将完全分化的成年体细胞重编程至胚胎样状态,从而使体细胞获得了自我更新的能力,并具备了分化成身体内所有细胞类型的能力。

2 PF的病理生理机制

组织修复是维持生存的关键功能5。当某个器官经历慢性和持续的损伤而无法自我修复时,就可能会导致组织纤维化6。慢性纤维化的主要特点是TGF‑β家族成员的异常高表达,以及细胞外基质(extracellular matrix,ECM)成分如胶原蛋白的过度沉积。这种过度积累会引起器官的异常重塑,最终导致器官功能的损害78

从细胞层面来看,肺纤维化的发病机制根本上来说是肺泡上皮损伤死亡,然后被激活的具有高度增殖活性的成纤维细胞取代9。许多治疗方法都是主要针对肺泡上皮细胞。肺泡上皮细胞Ⅱ型(type Ⅱ alveolar epithelial cells, AT2)是肺泡的主要成分之一,它们能够分泌表面活性物质,维持肺泡的稳定性,防止肺泡萎陷。肺泡上皮细胞Ⅰ型(Type I alveolar epithelial cells, AT1)负责整个身体的气体交换。AT2可以增殖和分化,自我更新并分化为AT1而替代受损的AT1,从而参与肺组织的修复1011。有多项研究表明,AT2在PF发生发展中起到关键作用。在家族性间质性肺炎中,经常会发现AT2表达的表面活性物质相关基因突变9。不仅如此,动物实验中也显示,AT2受到特定损伤的小鼠也会罹患PF12。因此,AT2被认为是PF治疗的关键细胞。

在特发性肺纤维化(idiopathic pulmonary fibrosis, IPF)的发病机制中,巨噬细胞的动态极化过程及其功能异质性也备受关注。根据微环境信号的差异,巨噬细胞可呈现经典活化(M1型)和替代活化(M2型)两种表型特征。研究显示,M1型巨噬细胞在肺泡上皮损伤早期通过促炎反应启动修复程序,而M2型巨噬细胞则通过抗炎因子分泌介导修复终止及炎症消退1314。当肺组织遭受持续性损伤时,这种精密调控的修复级联反应发生紊乱,导致ECM异常沉积并最终形成纤维化病理改变。在此过程中,巨噬细胞不仅通过释放炎症介质直接参与纤维化进程,其分泌的细胞外囊泡(extracellular vesicle, EV)更被证实可通过细胞间信号传递发挥潜在抗纤维化作用15。这种双向调控特性提示巨噬细胞在IPF中可能同时扮演"致病驱动者"与"修复调控者"的双重角色。

3 PSC在PF治疗中的应用

3.1 hESC在PF治疗中的应用

过去已经有研究证实了从hESC分化得到的AT2在PF的动物模型中显示出治疗潜力1617。并且研究发现,通过调节Wnt/β‑catenin信号通路,可以促进hESC向AT1分化,从而在小鼠肺纤维化模型中减少胶原沉积,改善肺功能18

免疫和基质调节细胞(immunity‑and matrix‐regulatory cell, IMRC)是一种由hESC定向诱导而来的细胞类型,具有独特的免疫调节和EMC调节能力。有研究针对IMRC的安全性以及有效性进行了评估,结果发现IMRC在体内和体外都表现出强大的免疫调节和抗纤维化的功能,并且将其在新冠肺炎患者中进行了使用,发现其在改善了病情的同时,患者也没有出现不良反应及任何异常1920

另外研究发现,IMRC或其培养基具有促进组织再生和免疫调节的功能。这些物质通过抗氧化和抗炎机制,抑制博来霉素(bleomycin,BLM)诱导的氧化应激、ECM沉积和上皮间质转化(epithelial to mesenchymal transition, EMT),从而减轻肺部纤维化的程度并阻止其进展21。研究还表明,IMRC分泌的EV通过气管给药或静脉注射均能有效治疗IPF,改善肺功能22。此外,hESC分泌的EV中的miR‑17‑5p能直接结合Thbs2,来调节炎症和纤维化,保护肺免受BLM毒性23

3.2 iPSC在PF治疗中的应用

多项研究表明,iPSC可通过多种机制减轻BLM诱导的PF。iPSC已被证明可以显著减少由BLM诱导的IGF1、IGF2Irs1基因表达升高,这些基因在纤维化和EMT过程中起关键作用24。iPSC还可以通过影响Wnt信号通路来减少肺部炎症和纤维化,这对于组织修复和再生至关重要25。此外,iPSC通过抑制TGF‑β/Smad2/3信号通路,从而减少肺部ECM沉积和EMT26。动物实验中,通过静脉注射iPSC衍生的间充质干细胞(iPSC‑derived mesenchymal stem cell,iPSC‑MSC)治疗BLM诱导的PF小鼠模型。结果显示,这些干细胞抑制了TGF‑β1信号通路和成纤维细胞沉积导致的纤维化,并促进了胶原蛋白的降解,证实了iPSC‑MSC在PF治疗中的潜力27

iPSC也可以通过分泌肝细胞生长因子(hepatocyte growth factor,HGF)和TGF‑β1等因子发挥治疗作用。研究发现iPSC‑CM中含有高水平的HGF,阻断HGF的作用可以部分逆转iPSC‑CM的抗纤维化效果,表明HGF是iPSC‑CM抗纤维化作用的关键因子28。此外,iPSC‑CM 还能降低肺组织和支气管肺泡灌洗液中的TGF‑β1水平,抑制Smad‑2和Smad‑3的磷酸化,阻断TGF‑β1/Smad信号通路29,并抑制TGF‑β诱导的AT2的EMT26。研究还表明,iPSC‑EV通过miR‑302a‑3p/TET1轴抑制M2型巨噬细胞,减轻纤维化,提示iPSC‑EV可能是PF的潜在治疗剂30

综上所述,PSC在PF的治疗中显示出显著潜力。它们通过分化为功能性肺上皮细胞、IMRC,以及分泌生长因子和 EV,能够修复肺损伤、减少炎症和胶原蛋白沉积,改善肺功能。这些干细胞通过调节相关信号通路,如Wnt/β‑catenin、IGF、TGF‑β1/Smad等,发挥抗氧化、抗炎和抗纤维化作用,为PF治疗提供了新的策略和希望。并且在临床试验已初步证实其安全性和有效性,为未来的临床应用奠定了基础(表1总结了第3部分中部分文献所涉及的实验方法以及机制)。

4 PSC在PF研究中的应用

4.1 疾病模型的构建

4.1.1 体外肺类器官模型

体外肺类器官(human lung organoid,LO)是一种自组织的3D细胞结构,比2D培养更能模拟肺组织的微环境31。这种基于PSC的技术不仅为个性化器官移植提供了可能,还有助于疾病模拟,尤其是对于肺部疾病,因其能更准确地反映人类疾病的特征3233。且LO构建的PF模型因其更接近患者真实的病理状态和特征而更加方便研究细胞间的相互作用34,并在呼吸系统发育、再生和疾病模型研究中显示出潜在应用3536。此外,利用iPSC或患者来源的PSC制备的LO,也为PF的研究以及药物筛选提供了重要工具37

近年来,研究人员已经成功建立了将人类多能干细胞(human pluripotent stem cell, hPSC) 在体外分化为LO的技术方案38。2015年有研究团队首次通过操控hPSC的发育信号通路,通过激活WNT和FGF信号同时抑制BMP/TGF‑β信号,成功地分化出肺样结构。虽然并未观察到分支形态发生,但LO在转录组上与人类胎儿肺相似,是研究人类肺发育、成熟和疾病的优秀模型39。进一步有研究利用hPSC和原代人胎儿肺成纤维细胞构成了具有多种肺泡上皮细胞表型的依赖成纤维细胞的肺泡类器官(fibroblast‑dependent alveolar organoid,FD‑AO)4041,并发现这些类器官通过与胎儿肺成纤维细胞的3D共培养,更有效地分化为肺泡上皮细胞40。现已有研究通过这种三维细胞结构发现了TAZ蛋白在肺泡上皮细胞分化中的关键作用,通过抑制TAZ可以抑制AT2向AT1转化42,这一发现揭示了TAZ蛋白在肺泡上皮细胞分化中的关键作用,为治疗这一疾病提供了新的思路和潜在的治疗靶点。而原代的肺泡上皮细胞分离和离体培养比较困难,后续有研究者通过对患者来源的iPSC进行分化,得到具有AT2的三维结构,可以在体外稳定培养,这一创新策略可能克服现有的障碍,并实现为肺纤维化的治疗提供新的精准有效的手段43。BLM处理能够诱导FD‑AO使其在体外模拟肺纤维化中的多种病理变化44,因此有利于尤其是针对肺泡上皮细胞的病因找到新的治疗靶点。同样,TGF‑β1处理的hPSC分化的LO也展现了明显的纤维化特征,适用于模拟PF和评估药物的抗纤维化效果45。含有巨噬细胞的肺泡类器官(alveolar organoid,AO)在TGF‑β1处理后能产生炎症和纤维化反应,这些反应可被抗纤维化药物缓解,也显示了其在药物筛选中的潜力46。最近的研究通过使用AO,并利用促纤维化因子混合物刺激,成功使AO表现出纤维化的关键特征,从而建立了一个新颖的药物筛选平台,为PF的治疗研究提供了重要工具47

最新研究发现,持续激活缺氧诱导因子(hypoxia‑inducible factor,HIF)信号通路驱动异常上皮细胞群的产生,在抑制HIF2后,可以减轻AO的纤维化程度,减少异常上皮细胞的出现,并可以促进气道祖细胞向肺泡细胞的分化,并增强AT2的成熟标志物48。尽管这些AO模型在PF的研究中取得了进展,但还需优化以更真实地模拟人类肺泡环境,包括整合巨噬细胞和血管细胞等与肺功能密切相关的细胞49

4.1.2 多能干细胞相关的气‑液界面模型

气‑液界面模型(air‑liquid interface model,ALI)技术通过拟人类肺部环境,使细胞在空气和液体交界处生长,一侧接触液体培养基,另一侧暴露于空气,从而更真实地再现肺内环境5051,该技术能够模拟肺部气体交换及细胞间交流,适用于研究呼吸道上皮细胞与液体和空气的相互作用52,也能保持细胞的特定功能、结构和细复杂性,可用于模拟人类呼吸系统的不同部分5354。相较于传统2D培养,ALI培养有效克服了组织结构丧失、细胞分化不足和屏障完整性破坏等挑战,对于呼吸系统疾病研究尤为重要。此外,ALI培养还能促使气道上皮细胞形成假复层结构、实现粘液纤毛分化和粘液分泌,具备屏障功能,有效模拟肺上皮细胞的体内特征5556

研究利用hiPSC分化出AT2,并在ALI培养条件下建立体外模型,模拟肺泡环境。通过PF相关细胞因子混合物刺激,模型展现了PF患者肺内的促纤维化特征并表达了纤维化的特异性基因57。另一研究采用类似方法,在微型96‑Transwell ALI模型中培养hiPSC衍生的AO,证实其能模拟PF的病理现象,如表达和分泌促纤维化生物标志物58。说明该模型既可以用于筛选和评估针对PF的潜在药物,也可以用于研究PF的分子机制。

4.2 药物筛选和新疗法开发

目前,已经有一些研究使用此类方法,发现了一些有价值的新工具。一项研究通过渐进性纤维化模型筛选出抗纤维化小分子AA559。另一研究利用hESC构建的3D minilungs模型,发现了维生素D类似物可通过减少DNA损伤来减轻肺纤维化60。波形蛋白中间丝组装抑制剂WFA也被证实可增强自噬、抑制纤维化相关基因,降低肺泡球体侵袭性,成为潜在治疗药物61。此外,还有研究团队利用hPSC衍生的LO发现IL‑11是纤维化关键因子,敲除IL‑11可阻止特定突变的LO的纤维化,表明其作为治疗靶点的潜力62表2总结了第4部分中部分文献所涉及的实验方法以及机制)。

5 讨论

在该领域中,研究者们取得了显著进展。他们成功提取和培养了包括hESC和hiPSC在内的PSC,并在体外诱导其分化为肺泡上皮细胞,为PF治疗提供了新思路。同时,建立了PF疾病模型,有助于理解发病机制和药物筛选。研究者们通过动物实验和临床前研究,初步证明了PSC移植治疗PF的可行性,并对其安全性进行了评估,为临床试验的开展提供了重要的安全数据。此外,通过研究PSC在PF治疗中的作用,研究者们揭示了多种分子信号通路和细胞间相互作用,为深入理解疾病和治疗提供了科学依据。

然而,尽管取得了这些显著进展,该领域的研究仍面临一些挑战和问题。首先,临床转化过程中,就包含着细胞移植的效率、安全性以及免疫排斥等问题。其次,尽管有很多研究都在解决PSC向特定肺细胞类型的分化效率不高、分化过程难以精确控制的问题186365,但这一问题仍面临着巨大挑战,这限制了其在临床治疗中的应用。再者,PSC治疗的长期效果和潜在风险尚不明确,因此需要使用高度富集的细胞产品来降低风险66,并需要更多的长期随访数据来评估。不仅如此,安全性问题还涉及到遗传和表观遗传异常的出现,要非常注意质量控制和和排除标准。此外,伦理和法律问题也是PSC研究必须面对的挑战,尤其是在ESC的使用上。标准化和规模化生产的问题同样值得关注。PSC的培养和分化缺乏统一的标准,且难以实现规模化生产,这增加了临床应用的难度和成本。最后,由于干细胞疗法因全流程从生产、储存到输送,都需要专门的设备和人员进行操作,因此价格昂贵。费用可能会限制到治疗对患者的可及性67。因此,未来的研究者和政策制定者需要共同努力,克服这些障碍,以推动PSC在PF治疗领域的进一步发展。
