神经电生理监测技术在脊柱手术中的应用

神经电生理监测技术在脊柱手术中的应用
Application of neuroelectrophysiological monitoring technology in spinal surgery
目的 探讨神经电生理监测技术降低脊柱外科术中神经损伤的应用价值。 方法 回顾性分析赣南医科大学第一附属医院2021年6月至2024年6月305例脊柱手术中应用日本光电MEE-1200C神经功能测试仪监测体感诱发电位(Somatosensory evoked potential, SEP)、运动诱发电位(Motor evoked potential, MEP)波幅变化及神经传导速度异常情况。 结果 305例脊柱手术患者中,62例判定为阳性预警,阳性预警率为20.3%。其中颈椎、胸椎、腰椎手术阳性预警率分别为25.0%、12.0%和18.9%。 结论 神经电生理监测技术在脊柱外科手术中能够及时发现神经传导速度异常情况,为手术医师操作防止神经损伤起到有效提示作用,最大程度降低对神经系统的损害。
Objective To explore the application value of neurophysiological monitoring in reducing intraoperative neurological injuries during spinal surgery. Methods A retrospective analysis was conducted on 305 spinal surgeries performed at the First Affiliated Hospital of Gannan Medical University from June 2021 to June 2024, where the Nihon Kohden MEE-1200C neurofunctional tester was used to monitor somatosensory evoked potentials (SEP), motor evoked potentials (MEP) amplitude changes, and abnormal nerve conduction velocities. Results Among 305 patients who underwent spinal surgery, 62 were identified as positive early warnings, with a positive early warning rate of 20.3%. Among them, the positive early warning rates for cervical spine, thoracic spine, and lumbar spine surgeries were 25.0%, 12.0%, and 18.9% respectively. Conclusion Neurophysiological monitoring during spinal surgery can promptly detect abnormal nerve conduction velocities, effectively alerting surgeons to prevent neurological injuries and minimizing damage to the nervous system.
