国际功能、残疾和健康分类视域下环境调控技术在康复医学中的应用与展望
Application and prospect of environmental regulation technology in rehabilitation medicine from the perspective of ICF
随着医学模式向生物-心理-社会模式转型,环境因素在康复中的作用日益凸显。本文基于世界卫生组织《国际功能、残疾和健康分类》理论框架,阐述环境调控技术在康复医学中的应用和发展前景。环境调控技术通过集成智能传感、自动化控制与新型材料,实现对声、光、温、湿、气味及空间布局等环境要素的精准干预。在临床应用方面,环境调控技术在神经康复、骨科康复及昏迷促醒等领域展现出显著潜力,临床应用包括通过节律性听觉刺激改善帕金森步态、利用多感官刺激促进意识恢复、构建安全物理环境以预防跌倒等。尽管目前面临技术标准缺失、评估工具不足及成本较高等挑战,但通过推动环境调控技术向智能化、个性化及社区化方向发展,将有效提升康复服务的精准性、人性化程度和覆盖范围,引领康复医学迈向新发展阶段。
With the ongoing shift of medical models toward the biopsychosocial approach, the role of environmental factors in rehabilitation has become increasingly prominent. Based on the world health organization's international classification of functioning, disability and health (ICF) framework, this paper systematically reviews the application and development prospects of environmental regulation technology in rehabilitation medicine. By integrating intelligent sensing, automated control, and novel materials, environmental regulation technology enables precise modulation of environmental elements such as sound, light, temperature, humidity, odor, and spatial layout. Clinically, it has demonstrated significant potential in neurological rehabilitation, orthopedic rehabilitation, and disorders of consciousness awakening. Key applications include rhythmic auditory stimulation for improving gait in Parkinson's disease, multisensory stimulation protocols to facilitate recovery of consciousness, and the design of safe physical environments for fall prevention. Although current challenges include the lack of technical standards, insufficient assessment tools, and high costs, advancing environmental regulation technology toward intelligent, personalized, and community-based solutions is expected to enhance the precision, human-centeredness, and accessibility of rehabilitation services, thereby guiding rehabilitation medicine into a new developmental phase.
