The application of different local anesthetic adjuvants in regional anesthesia

摘要

区域麻醉通过阻断神经传导实现手术镇痛,因其对全身影响小、术后恢复快等优势,已成为临床麻醉的重要策略。然而,传统局麻药存在作用时间短、高剂量易引发毒性反应等问题,制约了其临床应用。局麻药佐剂通过增强麻醉效果、延长作用时间及减少局麻药用量,降低了不良反应的发生风险。但其安全性、有效性及给药剂量目前仍存在争议。本文回顾了近年来国内外相关文献,阐述了肾上腺素、右美托咪定、糖皮质激素等常见佐剂的作用机制、临床效果及不良反应。文献复习结果表明,不同佐剂在区域麻醉中展现出显著的协同增效作用,但由于缺乏统一的剂量推荐标准,其临床应用仍面临个体差异显著及安全性不确定等挑战。未来研究亟需开展更多高质量的多中心随机对照试验,以明确各类佐剂的最佳配伍方案,并积极探索新型佐剂的开发,旨在提升区域麻醉的安全性与有效性,为临床实践提供更精准的指导。

Abstract

Regional anesthesia achieves surgical analgesia by blocking nerve conduction. Due to its advantages such as minimal impact on the whole body and rapid postoperative recovery, it has become an important strategy in clinical anesthesia. However, traditional local anesthetics have problems such as short duration of action and easy toxicity reactions at high doses, which restrict their clinical application. Local anesthetic adjuvants enhance the anesthetic effect, prolong the duration of action, and reduce the dosage of local anesthetics, thereby reducing the risk of adverse reactions. However, there are still controversies regarding their safety, efficacy, and dosage. This article systematically reviews the relevant domestic and foreign literature in recent years, elaborating on the mechanisms of action, clinical effects, and adverse reactions of common adjuvants such as epinephrine, dexmedetomidine, and glucocorticoids. The literature review shows that different adjuvants exhibit significant synergistic effects in regional anesthesia. However, due to the lack of unified dosage recommendation standards, their clinical application still faces challenges such as significant individual differences and uncertain safety. Future research urgently needs to conduct more high-quality multi-center randomized controlled trials to clarify the optimal combination schemes of various adjuvants and actively explore the development of new adjuvants, aiming to improve the safety and efficacy of regional anesthesia and provide more precise guidance for clinical practice.

区域麻醉是指将局部麻醉药注射到神经支配区域的周围,通过暂时阻断该区域神经冲动的传导,使身体对应部位产生感觉和运动消失的一种麻醉方法。局麻药的应用在区域麻醉中对于急性和慢性疼痛的管理有益,因此被广泛用于围术期的诊疗过程。尽管局麻药在疼痛管理方面带来诸多好处,但其在临床上的应用受作用时间限制,以及对心脏和中枢神经系统等可能产生的剂量相关性不良反应,如心律失常、呼吸抑制、中枢神经系统抑制等。为提高围术期安全并提升麻醉效果,局麻药常与佐剂联合使用,通过协同作用延长阻滞时间并减少局麻药总量。

1 局麻药佐剂

局麻药佐剂是指与局麻药联合使用,通过协同作用增强神经阻滞效果的一类药物。随着区域麻醉技术的普及和相关研究的深入,目前已知的局麻药佐剂有阿片类镇痛药物、肾上腺素、α2肾上腺素受体激动剂、地塞米松、咪达唑仑、NMDA受体阻滞剂等。

1.1 阿片类药物

阿片类药物作为常用的局麻药佐剂,在临床麻醉中扮演着十分重要的角色。研究证实,阿片受体存在于人类感觉神经的外周末梢上1。阿片类药物可能通过作用于G蛋白偶联受体,引起传入感觉神经元的超极化,从而降低神经元的兴奋性或减少神经递质的释放2

1.1.1 丁丙诺啡

丁丙诺啡是一种高脂溶性的μ受体部分激动联合κ/δ受体拮抗剂。对阿片受体的高亲和力以及良好的细胞膜穿透性,使其在鞘内注射及周围神经阻滞中展现出显著的应用潜力3。Candido K D等4研究发现,使用含有丁丙诺啡佐剂的布比卡因溶液进行臀下坐骨神经阻滞时,可以延长首次需要阿片类镇痛药物的时间约6 h。一项随机对照试验的Meta分析发现5,与单独使用局麻药相比,神经周围单独注射0.1~0.3 mg丁丙诺啡,可以显著延长镇痛时间。在剖宫产手术中,Ipe S等6对比了鞘内和硬膜外注射丁丙诺啡的临床效果,发现硬膜外注射0.3 mg丁丙诺啡与鞘内注射0.15 mg丁丙诺啡均能提供良好的术后镇痛,且2种给药方式对运动阻滞的时间均无显著影响。此外,该研究还观察到,接受丁丙诺啡治疗的患者与未接受丁丙诺啡治疗的患者在呼吸抑制、恶心或呕吐等不良反应方面无明显差异。这表明在剖宫产手术中,使用丁丙诺啡可以提供有效的镇痛效果,而且与未使用该药物的患者相比,使用丁丙诺啡的患者并未出现更多的不良反应。

1.1.2 吗啡

研究表明7,鞘内注射吗啡能够实现满意的麻醉效果。Frassanito L等8对52例接受全膝关节置换手术的患者进行了研究,比较了鞘内注射吗啡和单次股神经注射罗哌卡因的术后镇痛效果。结果发现,鞘内注射吗啡组在术后48 h内的吗啡使用量少于股神经阻滞组。还有研究显示,与采用患者自控镇痛(Patient-controlled analgesia,PCA)吗啡治疗的患者相比,接受硬膜外吗啡治疗的患者在1~6 h内的视觉模拟评分法(Visual analogue scale,VAS)评分较低9。尽管吗啡在椎管内阻滞中广泛使用,但其可能引起不良反应,如呼吸抑制、恶心、呕吐、瘙痒和尿潴留等,在大剂量使用下对老年患者构成较高风险。有证据表明10,与静脉注射和肌肉注射方式相比,吗啡辅助的周围神经阻滞并无明显优势。目前不建议在周围神经阻滞中使用吗啡,以避免可能引发的神经毒性反应。

1.1.3 芬太尼

1985年,Fontana P等11首次将芬太尼注射到老年患者的蛛网膜下腔内进行血管造影手术。这次成功的使用使芬太尼在椎管内的临床应用逐渐受到关注,并为后续的临床实践提供了宝贵的经验。尽管硬膜外给药是一种常见的给药方式,但芬太尼作为佐剂的应用并未展现出明显的优势。Nishikawa K等12对比了小剂量芬太尼分别经腋路臂丛神经阻滞联合给药(即加入利多卡因局麻药液中)与静脉注射给药的临床效应。发现与静脉注射相比,将芬太尼直接添加至利多卡因溶液中进行外周神经阻滞,能显著提高感觉阻滞的成功率,并有效延长感觉阻滞的持续时间。表明芬太尼在外周神经阻滞中可能具有局部辅助镇痛作用,其联合局麻药应用的效能优于单纯的全身性给药。Puri A等13发现,芬太尼作为局麻药佐剂用于锁骨上神经阻滞可能具有减轻疼痛的潜在优势,并为手术后的疼痛管理提供了更有效的选择。然而,芬太尼作为周围神经阻滞佐剂的效果存在较大争议。一些研究指出,在特定药物组合中芬太尼可能延长麻醉时间或增强镇痛效果,而其他研究者则未观察到类似益处14。这表明对芬太尼在不同治疗方案中的作用仍需进一步深入研究,以明确其在周围神经阻滞中的效果和适用性。

1.2 肾上腺素

多年来,肾上腺素作为局麻药的佐剂被广泛应用,其在提高麻醉效果、延长麻醉时间以及减少出血量等方面发挥着重要作用。其主要作用机制是通过α1受体介导的血管收缩作用来减少局部血流量,从而延长麻醉药在注射部位的停留时间,提高其局部浓度15。同时,肾上腺素减少了药物通过血液循环迅速分布到全身其他部位的风险。研究指出16,在周围神经阻滞中肾上腺素与不同的局麻药联合使用时可能产生不一样的效果。当它与短效或中效局麻药如利多卡因、甲哌卡因联合应用时,可显著延长局麻药的作用时间。然而,对于长效局部麻醉药,如罗哌卡因、布比卡因、左旋布比卡因,肾上腺素是否能延长其镇痛时间目前尚存争议。提示尽管肾上腺素在区域麻醉中的潜在益处被认可,但针对其对长效局部麻醉药镇痛时间的具体影响还需要进一步验证。需要注意的是,肾上腺素对血管的收缩作用会导致神经周围血流量减少,在神经已经受损的情况下(例如周围神经病变患者)可能引发神经毒性问题17。特别是对于已知存在神经损伤风险的糖尿病患者,使用肾上腺素作为佐剂时需要谨慎。

1.3 α2肾上腺素受体激动剂

1.3.1 可乐定

可乐定是一种具有血管收缩作用的非选择性α2肾上腺素受体激动剂。其作用机制是通过激活脊髓背角中的连接后α2肾上腺素受体来抑制伤害性冲动18。以往可乐定在椎管内阻滞中的使用常伴随着镇静、心动过缓和低血压等不良反应,这给临床工作带来一些问题。目前在鞘内应用小剂量范围的可乐定(15~40 μg),可以减少心血管不良反应的发生风险,同时确保患者在椎管内阻滞过程中的安全性19。既往研究显示,在不同人群中硬膜外应用25~50 μg/h剂量的可乐定,可以使吗啡的消耗量减少40%~60%20。Mohamed S A等21通过向手术区域局部浸润布比卡因和可乐定的混合液,发现可以有效降低乳房切除手术后慢性神经性疼痛的发生率。这与Tschernko E M等22研究一致,在局麻药中添加可乐定佐剂后,在进行肋间神经阻滞时显著改善了开胸手术后的疼痛和动脉氧合。此外,该研究还发现使用可乐定可以让术后FEV1/FVC的比值仅降至术前水平的60%~90%。这种联合应用不仅能够减少患者术后使用阿片类药物的总量,还能够降低VAS评分。需要注意的是,老年患者或有其他合并症的患者,即使使用较低剂量的可乐定,也可能引起与临床相关的低血压和镇静状态。

1.3.2 右美托咪定

右美托咪定是一种亲脂性的α羟甲基衍生物,其在α2和α1肾上腺素受体上的亲和力比为1 620∶1,远高于可乐定23。与其他α2激动剂类似,右美托咪定具有多重药理作用,包括镇静、催眠和镇痛。这些特性使其在临床上被广泛用于麻醉、术前镇静和术后镇痛等方面。研究发现,椎管内注射右美托咪定,可以降低患者对镇痛药物的需求24。一项关于鞘内注射右美托咪定的荟萃分析显示25,添加右美托咪定能够延长阻滞的持续时间,提供更长时间的镇痛效果,在低剂量右美托咪定(<5 μg)时,未观察到与右美托咪定相关的低血压或其他不良事件。近年来,右美托咪定作为局麻药佐剂在临床上的应用越来越受到关注。临床试验显示,右美托咪定与局部麻醉剂(如罗哌卡因、布比卡因)联合应用于不同部位的神经阻滞,可提高镇痛质量和持续时间23,将其作为局部麻醉的佐剂已被证实能提供更为优越的疗效和更好的术后恢复体验,未发现使用右美托咪定会导致神经毒性26-28。在动物脊髓损伤及缺血再灌注模型中,右美托咪定显示出显著的潜在神经细胞保护作用29。然而,在大鼠糖尿病模型中,神经周围使用高剂量的右美托咪定可能加剧局部麻醉药引起的神经损伤,表现为严重的脱髓鞘和轴突变性30。这种损伤可能是由于高剂量右美托咪定激活了α2受体介导的血管收缩效应,导致神经周围血流量减少,进而增强了局麻药的神经毒性。因此在进行特定区域的神经阻滞,例如腘窝后的坐骨神经阻滞时,应当谨慎使用。此外,为了确保右美托咪定在临床实践中的安全应用,需要进一步研究其剂量-效应关系及其潜在的作用机制。

1.4 地塞米松

地塞米松是一种具有抗炎作用的糖皮质激素,通过抑制磷脂酶A2和糖皮质激素受体激活发挥镇痛、免疫抑制和止吐作用。研究显示31,在骨科手术中,鞘内注射地塞米松能显著延长脊髓麻醉中感觉阻滞的持续时间,且未引起严重的不良反应。此外,地塞米松作为硬膜外局麻药的佐剂也能提供良好的镇痛效果19。Morita S等32发现,在神经周围注射地塞米松和布比卡因的混合液,能显著延长首次报告疼痛的时间,并减少所需镇痛药的剂量。关于地塞米松在神经周围使用时产生的镇痛效果是否源自其全身作用,这一点仍存在争议。地塞米松通过神经周围注射或静脉注射均能延长周围神经阻滞的持续时间33,但哪种途径更安全、有效仍存在不确定性。值得关注的是,地塞米松作为一种碱性溶液,pH为8.0,与罗哌卡因混合使用时,其碱化作用可能会导致非电离形式的罗哌卡因比例增加,对麻醉效果产生积极影响。然而,混合物pH值的升高同时增加了药物沉淀风险,在注射过程中需要谨慎处理,以避免可能发生的神经血管栓塞事件34。有研究35-36指出,地塞米松的抗炎和免疫抑制特性使其在全身应用时有助于延长镇痛效果的时间,并且减少呕吐的发生。这一发现可能促使人们采用更安全的静脉注射方式,替代神经周围应用。

1.5 其他

1.5.1 艾司氯胺酮

艾司氯胺酮是氯胺酮的右旋异构体,主要通过阻滞N-甲基-D-天冬氨酸(N-methyl-D-aspartic acid,NMDA)受体发挥镇痛作用,其作为局麻药佐剂在区域麻醉中具有潜力。Unlugenc H等37发现,在剖宫产手术中通过鞘内注射0.05 mg/kg艾司氯胺酮与布比卡因的混合液可以缩短感觉和运动神经阻滞的起效时间,并增强麻醉阻滞平面扩散,但未观察到延长镇痛效果的持续时间。然而,Martindale S J等38将0.5 mg/kg艾司氯胺酮作为布比卡因佐剂用于硬膜外注射,术后镇痛时间延长了6 h,且术后镇痛需求显著降低了50%以上。且在该研究中,硬膜外注射组与静脉注射组对比发现硬膜外注射艾司氯胺酮在提供镇痛效果方面更优。说明艾司氯胺酮可能通过作用于局部神经而非引起全身反应来实现更好的镇痛效果。在儿童患者下肢骨折的研究中,联合使用0.5 mg/kg的艾司氯胺酮与罗哌卡因进行坐骨神经阻滞和股神经阻滞,显示出优于单独使用罗哌卡因的镇痛效果39。目前关于艾司氯胺酮复合局麻药在周围神经阻滞中的应用研究相对较少,其临床效果和不良反应仍需进一步研究证实。

1.5.2 硫酸镁

硫酸镁是一种NMDA受体拮抗剂,其作用机制涉及二价正电荷对神经元膜电位的调节作用40。研究发现,硫酸镁无论是通过静脉还是鞘内给药,均能提升麻醉和镇痛效果41-42。此外,硫酸镁能降低周围神经兴奋性,并增强利多卡因提高A-β纤维兴奋阈值的能力43。然而,尽管硫酸镁具有这些潜在优势,但其在周围神经阻滞中的应用仍需谨慎。特别是,使用较高剂量的硫酸镁(如200 mg)可能会增加患者恶心呕吐的风险44

1.5.3 咪达唑仑

咪达唑仑是一种水溶性的苯二氮卓类药物,通过作用于A型氨基丁酸受体发挥抗伤害感受作用45。咪达唑仑硬膜外给药能有效延长术后镇痛时间,且无明显不良反应46。Ghai B等47发现在择期上腹部手术的儿童中,连续硬膜外输注布比卡因和咪达唑仑的混合液,不仅能降低术后镇痛的需求,还明显延长了患儿首次需要镇痛的时间。Tucker A P等48发现在常规治疗剂量下,椎管内使用咪达唑仑与不良的神经系统事件无关联。目前关于咪达唑仑作为局麻药佐剂对周围神经阻滞影响的研究相对有限。考虑其对神经毒性的潜在风险,在缺乏更多安全性证据的情况下,不建议在临床实践中使用咪达唑仑佐剂。

2 小结

在多种模式镇痛方案中,区域麻醉被广泛应用,旨在减轻围手术期疼痛、减少阿片类药物使用,并缩短住院时间。目前,局麻药佐剂的运用是麻醉学领域持续发展和创新的焦点。在临床工作中,众多研究致力于发现更有效的局麻药佐剂,以缩短起效时间、延长作用持续时间和提高区域麻醉质量。在各类佐剂中,阿片类药物仍然被广泛使用,而α2受体激动剂(如右美托咪定)也日益成为麻醉实践中的首选辅助药物。在选择局麻药佐剂时,应综合考虑现有证据和推荐的安全剂量范围,同时要做好手术麻醉并发症的处理。总之,区域麻醉作为一种有效的麻醉方式,为患者提供了良好的舒适度和术后恢复条件。随着技术的进步和新型麻醉药物的应用,区域麻醉的安全性和有效性将进一步提高,未来将得到更广泛的应用。
