摘要

目的 评估综合控制多种代谢危险因素与代谢功能障碍相关脂肪性肝病(MASLD)患者全因死亡风险的关联及其量化效应。 方法 采用回顾性队列研究设计,选取美国国家健康与营养检查调查(NHANES)2003—2018年数据,共纳入29 458例参与者,其中MASLD患者(MASLD组)12 528例,非MASLD患者(非MASLD组)16 930例。综合评估9项危险因素:高血压、糖尿病、肝纤维化[纤维化-4指数(FIB-4)]、肝酶比值[天冬氨酸氨基转移酶/丙氨酸氨基转移酶(AST/ALT)比值]、血清白蛋白、肾功能(血清肌酐)、全身性炎症(中性粒细胞计数)、吸烟和体力活动状况。根据综合危险因素控制程度,MASLD患者被进一步分为低控制组(n=1 107)、中控制组(n=7 042)和高控制组(n=4 379)。外部验证队列共纳入397例参与者,其中MASLD组266例,非MASLD组131例。计量资料多组间比较采用Kruskal-Wallis H检验。计数资料组间比较采用χ2检验。采用Cox比例风险模型对NHANES数据应用复杂抽样权重,分析危险因素控制程度与全因死亡率的关系,并辅以Kaplan-Meier生存曲线、Log-rank检验、限制平均生存时间(RMST)及人群归因分值(PAF)进行评价。在外部独立临床验证队列(n=397)中重复分析以检验结论的稳健性。 结果 基于NHANES数据库的队列研究,中位随访6.7年,12 528例MASLD患者中共1 171例死亡。死亡风险随危险因素控制数量增加而显著降低(趋势检验χ2=65.06,P<0.001)。与低控制组相比,中控制组[风险比(HR)=0.59,95%置信区间(CI):0.51~0.69]和高控制组(HR=0.36,95%CI:0.27~0.48)死亡风险依次降低;每多控制1个因素,死亡风险降低22%(HR=0.78,95%CI:0.74~0.82)。PAF分析显示,AST/ALT比值控制(PAF=19.2%)与体力活动(PAF=18.9%)是贡献最大的因素。基于最近邻匹配构建的子队列(最长随访13.3年)的生存分析显示,不同控制水平的MASLD组与非MASLD组之间的累积生存率存在显著差异(P<0.001);RMST分析进一步量化了各组生存获益:高控制组10年RMST最长(9.88年),优于非MASLD组(9.43年)、中控制组(9.33年)和低控制组(7.96年)。在完整队列中进行的多变量调整模型显示,与非MASLD组相比,低控制组MASLD患者的死亡风险显著升高,中控制组的风险与之相当,而高控制组患者的死亡风险显著低于非MASLD组。亚组分析结果显示,综合控制的保护作用在男性和女性中均显著存在(P值均<0.05),且交互作用分析提示其对女性的保护作用可能更强(P交互=0.031);保护效应在≥60岁患者中具有统计学显著性(P值均<0.05);保护作用在不同腰围分层人群中具有统计学显著性(P值均<0.05),但组间无显著交互作用(P交互=0.821)。外部验证队列397例参与者中有72例(18.1%)全因死亡。生存分析结果显示,MASLD患者的生存率随控制水平提高呈现显著的梯度改善(P=0.022)。RMST分析显示,中、高控制组RMST在5年时均优于非MASLD组(4.47年 vs 4.56年 vs 4.40年),至10年时生存优势更为明显(8.67年 vs 8.95年 vs 8.44年)。Cox回归分析结果显示,以MASLD患者低控制组为参照,中控制组(HR=0.41,95%CI:0.21~0.81)和高控制组(HR=0.32,95%CI:0.14~0.71)死亡风险显著降低;以非MASLD组为参照,低控制组风险显著升高(HR=2.02,95%CI:1.03~3.95),中控制组(HR=0.83,95%CI:0.48~1.44)和高控制组(HR=0.66,95%CI:0.30~1.38)风险与之相当,且效应点估计值与主研究高度吻合。 结论 MASLD患者代谢危险因素控制越好,死亡风险越低。达到最佳控制水平可消除超额死亡风险,使患者生存预后优于普通人群。

Abstract

Objective To investigate the association of comprehensive control of multiple metabolic risk factors with the risk of all-cause mortality in patients with metabolic dysfunction-associated steatotic liver disease (MASLD) and its quantitative effect. Methods A retrospective cohort study was conducted based on the data from National Health and Nutrition Examination Survey (NHANES) in 2003—2018. A total of 29 458 participants were enrolled, including 12 528 patients with MASLD and 16 930 non-MASLD controls. The control status of nine risk factors was assessed, i.e., hypertension, diabetes, liver fibrosis (assessed by fibrosis-4 index [FIB-4]), aspartate aminotransferase (AST)/alanine aminotransferase (ALT) ratio, serum albumin, renal function (assessed by serum creatinine), systemic inflammation (assessed by neutrophil count), smoking, and physical activity. According to the status of comprehensive risk factor control, the patients with MASLD were further divided into low-control group with 1 107 patients, moderate-control group with 7 042 patients, and high-control group with 4 379 patients. A total of 397 participants were enrolled in the external validation cohort, among whom there were 266 participants with MASLD and 131 participants without MASLD. The Kruskal-Wallis H test was used for comparison of continuous data between multiple groups, and the chi-square test was used for comparison of categorical data between groups. The Cox proportional hazards model incorporating complex sampling weights for NHANES data were used to investigate the association between the degree of risk factor control and all-cause mortality, and the Kaplan-Meier curve, the log-rank test, restricted mean survival time (RMST), and population attributable fraction (PAF) were used for further assessment. The robustness of the findings was validated in an independent clinical cohort (n=397). Results The cohort study based on the NHANES database showed that during the median follow-up time of 6.7 years, 1 171 deaths occurred among the 12 528 patients with MASLD. There was a significant reduction in mortality risk with the increase in the number of risk factors controlled (trend test: χ2 =65.06, P<0.001). Compared with the low-control group, the mortality risk decreased successively in the moderate-control group (hazard ratio [HR]=0.59, 95% confidence interval [CI]: 0.51 — 0.69) and the high-control group (HR=0.36, 95%CI: 0.27 — 0.48), and the mortality risk was reduced by 22% for each additional risk factor controlled (HR=0.78, 95%CI: 0.74 — 0.82). The PAF analysis showed that AST/ALT ratio control (PAF=19.2%) and physical activity (PAF=18.9%) were the largest contributors. The survival analysis of the sub-cohort constructed based on nearest neighbor matching (with a maximum follow-up time of 13.3 years) showed that there was a significant difference in cumulative survival rate between the MASLD groups with different control levels and the non-MASLD group (P<0.001). The RMST analysis further quantified the survival benefit of each group, and the high-control group had the longest 10-year RMST (9.88 years), which was superior to that of the non-MASLD group (9.43 years), the moderate-control group (9.33 years), and the low-control group (7.96 years). The multivariable-adjusted model analysis of the complete cohort showed that compared with the non-MASLD population, the MASLD patients in the low-control group had a significant increase in mortality risk, the moderate-control group had a comparable mortality risk, and the high-control group had a significantly lower mortality risk. The subgroup analysis showed a significant protective effect of comprehensive control in both male and female individuals (P<0.05), and the interaction analysis suggested a potentially stronger protective effect in female individuals (Pfor interaction=0.031). The protective effect was statistically significant in patients aged ≥60 years (P<0.05). There was a significant difference in protective effect across the groups stratified based on waist circumference (P<0.05), but there was no significant interaction between groups (Pfor interaction=0.821). In the external validation cohort, among the 397 participants, there were 72 cases of all-cause mortality (18.1%). The survival analysis showed a significant gradient increase in the survival rate of MASLD patients with the increase in the level of control (P=0.022). The RMST analysis showed that the moderate- and high-control groups had a better RMST than the non-MASLD group at 5 years (4.47 years vs 4.56 years vs 4.40 years), with a more significant survival advantage at 10 years (8.67 years vs 8.95 years vs 8.44 years). The Cox regression analysis showed that, with the low-control MASLD group as the reference, there was a significant reduction in mortality risk in the moderate-control group (HR=0.41, 95%CI: 0.21 — 0.81) and the high-control group (HR=0.32, 95%CI: 0.14 — 0.71); with the non-MASLD population as the reference, there was a significant increase in mortality risk in the low-control group (HR=2.02, 95%CI: 1.03 — 3.95), with a comparable mortality risk in the moderate-control group (HR=0.83, 95%CI: 0.48 — 1.44) and the high-control group (HR=0.66, 95%CI: 0.30 — 1.38), and the point estimates of the effects were highly consistent with the main study. Conclusion Among MASLD patients, better control of metabolic risk factors is associated with a lower mortality risk. Achieving optimal control can eliminate the excess mortality risk associated with MASLD, thereby helping patients achieve a better survival prognosis than the general population.

代谢功能障碍相关脂肪性肝病(metabolic dysfunction-associated steatotic liver disease, MASLD)以肝脂肪变性合并至少1项心脏代谢危险因素为特征,将肝脏健康与全身代谢状况明确关联1-2。该病在全球成人中的患病率接近三分之一,已成为重要的公共卫生问题3。其疾病谱可从单纯脂肪变性进展为脂肪性肝炎、肝纤维化甚至肝癌4-5。值得注意的是,MASLD患者的全因死亡风险显著升高,主要死因多为心血管事件等肝外并发症6
《代谢相关(非酒精性)脂肪性肝病防治指南(2024年版)》7强调,对代谢危险因素进行综合管理具有重要临床意义。然而,关于多重危险因素综合控制对预后的协同改善作用以及关键控制阈值及各因素的相对权重,目前仍缺乏系统的量化证据。
为此,本研究基于美国国家健康与营养检查调查(National Health and Nutrition Examination Survey,NHANES)数据库开展回顾性队列分析,旨在明确多重危险因素综合控制与死亡风险之间的关联,并在独立临床队列中进行验证,以期为MASLD人群的防控策略提供循证依据。

1 资料与方法

1.1 基于NHANES数据库的队列研究

1.1.1 研究对象

数据来源于2003—2018年NHANES数据库,采用MASLD作为核心术语,其定义依据2023年国际多学会共识1。该定义要求在确认肝脂肪变性的基础上,至少伴有1项心脏代谢危险因素,并排除其他主要肝病病因及过量饮酒。本研究的所有诊断与分组均基于此定义,识别标准如下:(1)肝脂肪变性采用脂肪肝指数(fatty liver index,FLI)进行评估,以FLI≥60定义为存在肝脂肪变性8-9。(2)心脏代谢危险因素要求满足至少以下1项:1)体重指数≥25 kg/m2或中心性肥胖(男性腰围≥94 cm,女性≥80 cm);2)空腹血糖≥100 mg/dL、餐后2 h血糖≥140 mg/dL、糖化血红蛋白≥5.7%或已确诊糖尿病;3)血压≥130/85 mmHg或已确诊高血压;4)空腹甘油三酯≥150 mg/dL;5)高密度脂蛋白胆固醇降低(男性<40 mg/dL,女性<50 mg/dL)1。(3)同时排除其他肝病及过量饮酒者(男性≥30 g/d,女性≥20 g/d)1。将所有参与者分为MASLD组和非MASLD组。

1.1.2 危险因素评估

系统评估9项临床常见且可干预的危险因素:糖尿病、高血压、肾功能、肝酶比值、肝纤维化、血清白蛋白、全身性炎症、吸烟状况及体力活动。各危险因素的目标或推荐水平基于NHANES数据库标准中推荐的水平及相关文献研究。

各风险因素的控制标准定义如下:(1)糖尿病及高血压控制:根据自我报告及用药史,确认为无糖尿病或高血压病史视为控制良好10-11。(2)肾功能控制:血清肌酐水平控制在男性0.67~1.17 mg/dL、女性0.51~0.95 mg/dL的参考范围内12。(3)肝酶比值控制:采用天冬氨酸氨基转移酶(aspartate transferase,AST)与丙氨酸氨基转移酶(alanine amino-transferase,ALT)比值进行评估,将AST/ALT<0.8定义为控制良好13。(4)肝纤维化控制:采用肝纤维化-4指数(fibrosis-4 index,FIB-4指数)进行评估,将FIB-4指数<1.3定义为控制良好14。(5)血清白蛋白控制:血清白蛋白水平维持在3.5~5.0 g/dL的正常参考范围内视为控制良好15。(6)炎症控制:中性粒细胞计数维持在(2.5~7.0)×109/L的参考范围内视为控制良好16。(7)吸烟控制:一生中累计吸烟量<100支的个体定义为控制良好。(8)体力活动控制:每周至少进行1次、每次持续至少10 min并能引起呼吸或心率略有增加的中等强度运动、健身或娱乐活动的个体定义为活动达标,即控制良好17

根据上述标准,计算每例MASLD患者达到控制状态的风险因素数量,并据此将研究人群分为3组:低控制组(≤3个因素控制)、中控制组(4~6个因素控制)和高控制组(≥7个因素控制),以用于后续的关联性分析。分组经限制性立方样条Cox回归验证,控制数量与死亡风险呈显著非线性关联,风险下降趋势以≤3、4~6、≥7为切点,且该分组赤池信息准则最小、一致性指数最高,优于其他方案。

1.1.3 协变量评估

从NHANES数据库的体格检查、实验室检测及问卷调查信息中提取以下协变量用于模型调整:年龄、性别、种族(白人/非白人)、教育水平(高中以下/高中或大学在读/大学毕业以上)、婚姻状况(稳定/非稳定)、贫困收入比、饮酒史(是/否)、腰围、体重指数、血小板计数、糖化血红蛋白、甘油三酯、γ-谷氨酰转移酶及每日久坐时间。

1.1.4 结局

本研究主要结局为全因死亡,通过链接NHANES死亡率档案(截至2018年12月31日)确定。随访时间自基线访视日起,至发生全因死亡或随访截止日(2018年12月31日)先到达者为随访终点。

1.2 外部验证

为验证基于NHANES数据库的队列研究结论,从内蒙古民族大学附属医院(2015—2020年)构建独立临床队列。MASLD依据腹部超声及相同代谢标准诊断7。样本量估算基于主研究中观察到的效应量,满足验证分析的要求。随访至2023年12月31日。危险因素定义、分组与主研究一致。

1.3 统计学方法

采用R 4.3.3软件进行统计学分析。计量资料以MP25P75)表示,多组间比较采用Kruskal-Wallis H检验。计数资料组间比较采用χ2检验。基于NHANES数据库的队列研究应用NHANES复杂抽样权重18-19。为可视化比较,采用1∶1最近邻匹配(匹配年龄、性别和种族)构建子队列,用于绘制Kaplan-Meier生存曲线、Log-rank检验及限制平均生存时间(restricted mean survival time,RMST)分析。为评估综合危险因素控制的效果,构建2个Cox比例风险模型:在MASLD人群内部,以低控制组为参照,评估中、高控制组的死亡风险;在整个研究人群中,以非MASLD组为参照,评估低、中和高控制组的死亡风险。模型结果以森林图直观呈现。为量化各危险因素的独立贡献,进行多变量因素分析,将9个危险因素(FIB-4指数、AST/ALT比值、血清白蛋白、中性粒细胞计数、高血压、糖尿病、肾功能、吸烟和体力活动)作为自变量同时纳入Cox模型,并调整1.1.3节所列的全部协变量。所有Cox模型均进行Schoenfeld残差检验验证比例风险假设。其中,血清白蛋白变量违反了比例风险假设(P<0.05),因此对其引入时间依存协变量项进行分析。计算人群归因分值(population attributable fraction,PAF)以量化各危险因素控制不佳对死亡风险的归因比例。进行亚组分析(按性别、年龄和腰围分层)及交互作用检验。验证队列采用相同核心方法(Kaplan-Meier、Cox模型)进行一致性分析,受样本量所限,多变量Cox模型仅纳入年龄、性别和血小板计数作为调整变量。P<0.05为差异有统计学意义。

2 结果

2.1 基于NHANES数据库的队列研究

2.1.1 基线分析

初始纳入80 312例参与者,在排除年龄<20岁、无法计算FLI及关键变量缺失的个体后,最终分析队列包含29 458例参与者,其中MASLD患者12 528例,非MASLD患者16 930例。MASLD患者中低控制组1 107例(8.8%)、中控制组7 042例(56.2%)和高控制组4 379例(35.0%)。基线分析(表1)显示,控制程度更高的MASLD患者群体呈现更健康的人口学、生活方式及临床指标。与低控制组相比,高控制组患者更年轻,社会经济状况更好,吸烟者比例更低;在临床指标方面,高控制组患者的合并症负担较轻,高血压和糖尿病的患病率较低;关键肝脏和代谢指标也表现更好,包括更优的FIB-4指数、AST/ALT比值、肾功能及血糖控制水平。

2.1.2 MASLD患者危险因素控制程度与全因死亡率

在中位随访6.7年期间,共记录到1 171例死亡。分析显示,危险因素综合控制程度与全因死亡率呈显著负相关,且存在剂量-反应关系(趋势检验χ²=65.06,P<0.001)。与低控制组相比,中控制组(风险比=0.59,95%置信区间:0.51~0.69)、高控制组(风险比=0.36,95%置信区间:0.27~0.48)的多变量调整死亡风险显著降低。将控制因素数量作为连续变量分析时,每增加1个控制因素,死亡风险均显著下降。各危险因素的PAF分析表明,AST/ALT比值控制(PAF=19.2%)与体力活动达标(PAF=18.9%)是贡献度最高的2项可干预因素(表2)。

2.1.3 MASLD组与非MASLD组的危险因素控制程度和全因死亡率

基于最近邻匹配构建的子队列(最长随访13.3年)的生存分析显示,不同控制水平的MASLD组与非MASLD组之间的累积生存率存在显著差异(P<0.001)(图1)。RMST分析进一步量化了各组生存获益:高控制组10年RMST最长(9.88年),优于非MASLD组(9.43年)、中控制组(9.33年)和低控制组(7.96年)。在完整队列中进行的多变量调整模型显示,与非MASLD组相比,低控制组MASLD患者的死亡风险显著升高,中控制组的风险与之相当,而高控制组患者的死亡风险显著低于非MASLD组(图2)。

2.1.4 MASLD患者危险因素控制程度与全因死亡率的亚组分析

为评估危险因素综合保护效应的普遍性,按性别、年龄和腰围进行亚组分析。亚组分析显示,综合控制的保护作用在男性和女性中均显著存在(P值均<0.05),且交互作用分析提示其对女性的保护作用可能更强(P交互=0.031)。年龄分层分析显示,保护效应在≥60岁患者中具有统计学显著性(P值均<0.05)。保护作用在不同腰围分层人群中具有统计学显著性(P值均<0.05),但组间无显著交互作用(P交互=0.821)(表3)。

2.2 外部验证

2.2.1 基线分析

共纳入397例参与者,其中MASLD 266例,非MASLD 131例。该队列基线特征与主研究呈现相似趋势:低控制组患者年龄更大,且高血压、糖尿病等代谢危险因素负担更重(表4)。

2.2.2 生存分析

随访至2023年12月31日,有72例(18.1%)全因死亡。生存分析结果进一步支持主研究的核心结论,MASLD患者的生存率随控制水平提高呈现显著的梯度改善(P=0.022),且中控制组的生存曲线已接近非MASLD对照组甚至略优(图3)。

2.2.3 RMST分析及Cox回归分析

RMST分析显示,中、高控制组RMST在5年时均优于非MASLD组(4.47年 vs 4.56年 vs 4.40年),至10年时生存优势更为明显(8.67年 vs 8.95年 vs 8.44年)。Cox回归分析复现了主研究核心关联模式:以MASLD患者低控制组为参照,中控制组(风险比=0.41,95%置信区间:0.21~0.81)、高控制组(风险比=0.32,95%置信区间:0.14~0.71)死亡风险显著降低;以非MASLD人群为参照,低控制组风险显著升高,高控制组风险与之相当,且效应点估计值与主研究高度吻合(图4)。

3 讨论

本研究采用大规模人群队列与独立临床验证相结合的设计,系统评估了多重代谢危险因素综合控制与MASLD患者全因死亡率的关系。既往研究多聚焦于单个代谢因素(如体力活动、血压或血糖)对MASLD预后的影响,或仅局限于某一类因素(如心血管相关因素)进行组合评估。本研究首次综合评估9项关键危险因素,不仅证实了控制数量与死亡风险之间的剂量-反应关系,还通过PAF分析量化了各因素的相对贡献,为建立MASLD的精准综合管理策略提供了重要依据。

基于NHANES数据库的队列显示,危险因素控制程度与全因死亡风险之间存在显著的剂量-反应关系:每多控制1个因素,死亡风险降低22%(HR=0.78);实现高水平控制(≥7个因素)的患者死亡风险较低控制组降低64%(HR=0.36),其风险甚至低于非MASLD人群。上述核心关联在独立临床队列中得到了验证。对于“高控制组风险低于非MASLD人群”这一发现,分析原因可能与能够实现高度综合控制的患者可能具有更强的健康意识和治疗依从性有关;其次,多种危险因素的综合控制可能产生协同增效,全面改善心血管-肝脏-代谢轴功能21,从而带来超越普通人群的生存获益。这一发现具有重要临床意义,提示通过积极全面的风险管理,MASLD患者不仅可改善疾病相关的生存劣势,甚至可能获得优于一般人群的生存结局,为将MASLD纳入慢病综合管理体系提供了有力证据。

尽管验证队列与主研究结论高度一致,但仍存在部分差异。验证队列样本量较小,统计效能有限,高控制组与非MASLD人群比较的风险比(HR=0.66)未达统计学显著性,但其点估计值与主研究(HR=0.65)高度接近,提示真实效应量可能相似。验证队列中中等控制水平患者的生存预后已接近非MASLD人群,可能是该临床队列基线风险较高,对综合干预产生了更显著的早期获益。

本文结果显示,AST/ALT比值控制(PAF=19.2%)与体力活动(PAF=18.9%)是贡献最大的2个可干预因素。AST/ALT比值是肝纤维化进展的标志物,控制该指标将带来最大死亡风险下降归因,提示抑制肝脏炎症与纤维化是改善预后的核心环节13。体力活动能够显著降低心血管疾病风险,并干预代谢综合征22。高血压控制(PAF=12.4%)与吸烟改善(PAF=12.2%)同样是重要的干预方向23-24。近期证据表明,代谢异常与脂肪肝患者的心脏舒张功能受损密切相关25,进一步支持了血压管理在维护MASLD患者心血管健康中的关键作用。

亚组分析显示,综合风险控制在女性患者中可能具有更强的保护作用(交互作用P<0.05),可能与性别相关的激素水平、体脂分布、健康行为依从性等因素有关。在年龄分层中,保护效应在≥60岁患者中尤为显著(HR=0.35),提示在衰老背景下有效的危险因素控制可产生更大的相对健康收益。在不同腰围亚组中,保护作用均持续存在,表明无论是否伴有腹型肥胖,积极的多因素干预均能带来获益。

MASLD患者管理应从肝脏转向多学科协作,重点关注心血管代谢风险的全面控制7。本研究结果量化了多重危险因素综合控制的累积效应,为指南中“加强代谢风险管理”的推荐提供了实证依据,也提示临床实践中应建立系统的评估框架和明确的控制目标。本研究结果与近期学术观点一致26,支持通过综合管理显著降低MASLD患者死亡风险。相关研究显示,非肝硬化MASLD患者的主要死因为心血管疾病及肝外恶性肿瘤,而非肝脏本身并发症627。因此,临床管理需要整合心血管风险管理、恶性肿瘤筛查与肝脏专科随访。同时,肝纤维化仍是MASLD患者全因死亡及肝脏相关死亡的重要预测指标429。对于已出现显著肝纤维化的患者,在强化风险因素管理的同时需结合抗纤维化治疗与密切监测30

本研究存在一定局限性:(1)MASLD诊断基于FLI而非肝活组织检查金标准,可能存在误分类偏倚31-32;(2)作为观察性研究,尽管调整了已知混杂因素,但残余混杂仍无法完全排除;(3)危险因素控制状态基于基线评估,未能反映随访期间的动态变化;(4)尽管数据库中包含药物使用情况与饮食等信息,但因缺失比例较高,为保证模型完整性与结果稳健性,本研究未将其纳入最终的多变量分析模型,可能对结果产生一定影响;(5)外部验证队列为单中心回顾性设计且样本量有限,虽核心结论与主研究高度一致,但其统计效能与外推性仍受限;(6)本研究基于美国NHANES数据库,结论应用于其他人群时需谨慎。

综上所述,本研究结合大规模人群分析与独立临床验证,证实了危险因素控制程度与MASLD患者全因死亡率呈显著负相关。达到最佳控制水平的患者,其死亡风险甚至低于非MASLD人群。上述发现提示,对MASLD患者实施全面、系统的危险因素管理可为其长期预后带来重要获益。
