基于“以经释案”结合翻转课堂的中医临床教学模式探讨

基于“以经释案”结合翻转课堂的中医临床教学模式探讨
Discussion on the clinical teaching model of traditional Chinese medicine based on“Case-Based Explanation of Classical Texts” combined with flipped classroom
目的 探讨“以经释案”结合翻转课堂的教学模式在中医内科学课程中对提升本科生中医临床思维能力和实践能力的效果,并与传统教学模式进行比较,以验证其有效性。 方法 建立在线的中医内科学“以经释案”病例库,采用对照研究设计将4个平行教学班148名学生随机分为试验组(n=78)与对照组(n=70)。试验组采用“以经释案”结合翻转课堂的教学模式,对照组采用传统教学模式。通过迷你临床演练评估(TCM-Mini-CEX)、期末考核成绩、学生满意度问卷等方法,收集并分析两组学生的学术表现、临床能力及其对教学模式的反馈。 结果 试验组学生成绩(72.72±8.75)分显著高于对照组(69.46±9.67)分。TCM-Mini-CEX评估显示,试验组教学后总分显著提升,特别是体格检查、辨证思路、理法方药方面均明显上升(P<0.05)。学生反馈显示,78.1%的学生认为课程对学习中医有帮助,87.2%认为对建立中医思维有帮助。此外,81.3%的学生认为本科阶段开设该课程有必要,78.3%愿意将课程推荐给他人。课程优点包括中医思维纯正性(85.5%)和授课思路清晰度(81.8%)。 结论 “以经释案”结合翻转课堂教学模式有效增强了学生的中医临床思维和实践能力,获得了较高的学生满意度,显示出在中医教育中的应用潜力。
Objective To explore the effectiveness of integrating the “Case-Based Explanation of Classical Texts” with a flipped classroom teaching model in enhancing undergraduate students’ clinical thinking and practical skills in the course of Traditional Chinese Internal Medicine, and to compare it with the traditional teaching model to verify its efficacy. Methods An online case database for “Traditional Chinese Internal Medicine” using the “Case-Based Explanation of Classical Texts” was established. A controlled study design was employed, 148 students were randomly dividing four parallel teaching classes into the experimental group (n=78) and control group (n=70). The experimental group adopted the “Case-Based Explanation of Classical Texts” combined with a flipped classroom teaching model, while the control group followed the traditional teaching model. Students’ academic performance, clinical skills, and feedback on the teaching models were collected and analyzed through the TCM-Mini-CEX assessment, final exam scores, and student satisfaction surveys. Results The experimental group’s scores (72.72±8.75) points were significantly higher than those of the control group (69.46±9.67) points. TCM-Mini-CEX assessments revealed a significant improvement in the total score for the experimental group after the intervention, with notable improvements in physical examination, diagnostic thinking, and the application of treatment principles and formulas (P<0.05). Student feedback showed that 78.1% found the course helpful in learning TCM, and 87.2% believed it contributed to the development of TCM clinical thinking. Moreover, 81.3% students felt it was necessary to offer this course at the undergraduate level, and 78.3% were willing to recommend it to others. The advantages of the course included the authenticity of TCM thinking (85.5%) and clarity in teaching (81.8%). Conclusion The “Case-Based Explanation of Classical Texts” combined with a flipped classroom teaching model effectively enhances students’ clinical thinking and practical skills in TCM, achieving high student satisfaction. It demonstrates potential for broader application in TCM education.
